    Notes: Summary The neuroendocrine effects (changes in plasma CRF, ACTH and cortisol) of single and multiple (t.d.s. for 2 days) doses of ipsapirone (BAY Q 7821) 5 and 10 mg have been investigated in 6 healthy male volunteers. The study followed a balanced complete block, placebo-controlled and double blind design with two baseline phases (pre and post-treatment). Volunteers were investigated on identical days during 5 successive weeks. The results do not show a specific effect of ipsapirone on the hypothalamic-pituitary-adrenal axis when doses in the range of 5–30 mg per day were given.

    Description / Table of Contents: Summary Platelet counts were investigated in 26 patients withP. falciparum malaria and 39 patients withP. vivax malaria before and after treatment. Before schizontocidal treatment 22 of 26 (85%) patients withP. falciparum malaria and 30 of 39 (72%) patients withP. vivax malaria had depressed platelet counts below 150,000/Μl blood. There was a correlation between low platelet counts and high counts of malarial plasmodia (parasitized red blood cells) inP. falciparum andP. vivax infections (p 〈 0.001). Platelet survival, studied by malonaldehyde formation in three patients during the period of decreasing parasitaemia, revealed a shortened life span to 2–3 days in comparison to 7–10 days in normal controls. In all patients platelet counts rose to threefold the initial values within 5 days after clearance of parasites. The results demonstrate that, first, thrombocytopenia is a common feature in human malaria, second, thrombocytopenia induced by malaria is due to shortened life span in the peripheral blood and, third, some interaction is present between platelets and malaria plasmodia or parasitized red cells.
    Notes: Zusammenfassung Bei 26 Patienten mitMalaria tropica und 39 Patienten mitMalaria tertiana wurden die Thrombozytenzahlen vor und nach Behandlung untersucht. Vor der spezifischen Malariabehandlung zeigten 22 von 26 Patienten mitMalaria tropica (85%) und 30 von 39 Patienten mitMalaria tertiana (72%) erniedrigte Plättchenzahlen unter 150 000/Μd Blut. Zwischen niedrigen Thrombozytenzahlen und hoher Parasitämie bestand eine eindeutige Korrelation sowohl bei Infektionen durchP. falciparum als auch durchP. vivax (p 〈 0,001). Bei drei Patienten wurde während der abklingenden Parasitämie die Lebenszeit der Thrombozyten mit der Methode der Malonaldehyd-Bildung untersucht. Sie zeigte eine auf 2–3 Tage verkürzte Lebenszeit der Thrombozyten im Vergleich zu 7–10 Tagen bei normalen Kontrollpersonen. In allen Patienten stiegen die Thrombozytenzahlen nach beendeter malariaspezifischer Behandlung innerhalb von 5 Tagen bis auf das Dreifache der Ausgangswerte an. Die Ergebnisse lassen darauf schlie\en, da\ erstens eine Thrombozytopenie regelmä\ig bei der menschlichen Malaria vorhanden ist, zweitens die durch Malaria induzierte Thrombozytopenie bedingt ist durch eine verkürzte Lebenszeit der Thrombozyten im peripheren Blut, und drittens, da\ eine Interaktion zwischen Thrombozyten und den Malariaplasmodien bzw. den parasitierten Erythrozyten besteht.

    Notes: Summary An analysis of erythroblast cell kinetics utilizing quantitative 14C-autoradiography has been performed in five cases of acute Plasmodium falciparum malaria prior to and, in four patients, 3 or 6 days after the onset of antimalarial therapy. Associated with no or only moderate anemia were changes of erythroblast morphology, a considerable shift in the frequency of red and white blood cell precursors in the bone marrow, and a reduced rate of erythroblast proliferation. There was a marked loss of polychromatic erythroblasts, which was smaller but still detectable during the therapeutic phase. The results provide some quantitative data on the extent of “parenchymal damage” of bone marrow and stress the impact of ineffective erythropoiesis and reduced rate of erythropoietic proliferation on the emergence of anemia in Plasmodium falciparum malaria.

    Notes: Summary Fibrin(ogen) degradation products, platelet counts, antithrombin III, and the components of the Factor VIII complex were studied in a total of 80 patients with Plasmodium falciparum, Plasmodium vivax or Plasmodium ovale infections. The haemostatic findings were correlated to the numbers of parasitized erythrocytes and to each other. The results indicate that haemostatic changes in malaria correlate with the degree of parasitaemia. Evidence for moderate hyperfibrinolysis was found in patients with high P. falciparum parasitaemias only. Thrombocytopenia closely corresponded to parasitaemia and to von Willebrand factor levels, but appeared not to be linked to a consumption of coagulation factors. It was concluded that thrombocytopenia in malaria is not indicative of disseminated intravascular coagulation (DIC) but may relate to endothelial damage.

    Description / Table of Contents: Zusammenfassung Probenecid bewirkt bei Ratten einen dosisabhängigen Anstieg der Konzentration von zyklischem Adenosin-3′, 5′-Monophosphat (cAMP) im zisternalen Liquor. Maximaler Anstieg durch Sättigung des Anionencarriers erfolgt 2 h nach einer Dosis von 150 mg/kg Körpergewicht. Am isolierten Plexus chorioideus läßt sich eine überwiegend kompetitive Hemmung der Aufnahme von cAMP nachweisen. Probenecid hemmt ferner nichtkompetitiv eine für cAMP hochaffine Phosphodiesterase des Rattenhirns mit einer Inhibitorkonstante von 3.4×10−3M. Nachdem Probenecid als Testsubstanz in der neurologisch-psychiatrischen Diagnostik für die Untersuchung des Dopamin- und Serotonin-Umsatzes im Liquor bereits vielfach eingesetzt wird, stützen diese Befunde seine Verwendbarkeit auch für cAMP als second messenger.
    Notes: Summary In rats, probenecid exhibits a dose-dependent increase in the concentration of cyclic adenosine 3′, 5′-monophosphate (cAMP) in cisternal cerebrospinal fluid (CSF). Maximal accumulation is reached 2 h after IP administration at a dosage of 150 mg/kg body weight. Serum levels of cAMP are unchanged after 200 mg/kg probenecid. In vitro investigations show an inhibitory effect of probenecid on the uptake of cAMP into the isolated choroid plexus of the rabbit. A non-competetive inhibition of probenecid on a high affinity fraction of cyclic nucleotide phosphodiesterase from rat brain homogenates is demonstrated with an inhibitor constant of 3.4×10−3M. The results appear to validitate the “probenecid test” for cAMP in clinical diagnostics.
