    Description / Table of Contents: Summary 1. A simple method for the simultaneuous estimation of plasma volume and total number of erythrocytes using51Cr and125I-albumin is described and its advantages over other methods of simultaneous estimation are discussed. 2. The relation between body haematocrit-perioheral haematocrit values is investigated in 35 people. In the control group made up of anaemis of different origin and severity but without enlargment of the spleen and polycycthamics the results were comparable with those using varims other methods of blood volume estimations. 3. A highly significant deviation (p〈0,001) from the norm was shown in 10 patients with splenomegaly of various origin. Different results were obtained in 3 splenectomized patients. 4. The importance of the changes with regard to the anaemia of patients with splenomegaly and the value of bloodvolume estimations in various diseases of the blood is discussed in conclusion.

Eine einfache Methode zur gleichzeitigen Bestimmung von Plasmavolumen und Gesamterythrozytenmenge unter Verwendung von51Cr und125I-Albumin wird beschrieben und die Vorteile gegenüber anderen Simultanbestimmungen diskutiert. 2. Die Relation zwischen Körperhämatokrit und peripheren Hämatokritwerten wurde bei 35 Personen untersucht. In der Kontrollgruppe, bei Anämien verschiedener Ursache und Schwere ohne Milzvergrößerung und bei Polyzythämikern wurden mit anderen Blutvolumsbestimmungen vergleichbare Resultate erhalten. 3. Eine von der Normalgruppe hochsignifikante (p〈0,001) Abweichung dieser Relation wurde lediglich bei 10 Patienten mit Splenomegalie verschiedener Ursache nachgewiesen, während bei 3 splenektomierten Kranken unterschiedliche Ergebnisse erhalten wurden. 4. Die Bedeutung der Veränderungen im Hinblick auf die Anämie von Splenomegaliepatienten und der Wert von Blutvolumsbestimmungen bei verschiedenen hämatologischen Erkrankungen wird abschließend besprochen.

    Description / Table of Contents: Zusammenfassung Der Antimetabolit 6-Aminonicotinamid (6-AN) erzeugt bei Ratten eine irreversible spastische Parese Durch die mangelnde Spezifität einer Glykohydrolase des endoplasmatischen Reticulums werden aus NAD und NADP durch Austauschreaktion von 6-AN gegen Nicotinsäureamid abnorm strukturierte Nucleotide gebildet. 6-Aminonicotinsäureamid-Adenin-Dinuclcotid-Phosphat (6-ANADP) ist ein sehr wirksamer Hemmstoff der 6-Phosphogluconat-Dehydrogenase, der eine Blockade des Pentose-Phosphat-Weges mit Akkumulation von 6-Phosphogluconat bewirkt. Dies hat eine Funktionsstörung der Phosphoglucoseisomerase zur Folge, die eine Schlüsselstellung im Embden-Meyerhof-Weg einnimmt. Bei den mit 6-AN behandelten Tieren fanden sich charakteristische Abweichungen im Kohlenhydratstoffwechsel des Zentralnervensystems. Die Unterschiede gegenüber den Veränderungen nach Einwirkung einer kurzfristigen Ischämie waren eindeutig und bestätigen die celluläre Genese der experimentellen Myelopathie. Die höchsten Konzentrationen von 6-Phosphogluconat wurden in den Regionen des Zentralnervensystems festgestellt, die auch die stärksten Zellschädigungen aufwiesen. Im Rückenmark betrugen sie fast das 400fache der Norm. Elektronenmikroskopische Untersuchungen des Rückenmarks ergaben eine primäre Schädigung der Oligodendro- und Astroglia. Die Topographie der Neurogliazellen begünstigt die Aufnahme von 6-AN und erklärt die celluläre Läsion, die eine Störung der funktionellen Beziehungen zwischen Glia- und Nervenzelle in der spinalen Grisea zur Folge hat.
    Notes: Summary The antimetabolite 6-aminonicotinamide (6-AN) produces an irreversible spastic paresis in rats. By a lack of specificity of a glycohydrolase of the endoplasmic reticulum, abnormally structured nucleotides are synthesized from NAD and NADP by exchange reaction of 6-AN for nicotinamide. 6-Amino-nicotinamide-adenine-dinucleotide-phosphate (6-ANADP) is a very effective inhibitor of the 6-phosphogluconate dehydrogenase which causes a blockade of the pentose phosphate pathway with accumulation of 6-phosphogluconate. This leads to a functional disorder of the phosphoglucose isomerase which occupies a key position in the Embden-Meyerhof pathway. In the animals treated with 6-AN, characteristic deviations were found in the carbohydrate metabolism of the central nervous system. There were clear differences as compared with the changes after the effect of a temporary ischaemia which confirm the cellular genesis of the experimental myelopathy. The highest concentrations of 6-phosphogluconate were observed in those regions of the central nervous system which also exhibited the strongest cell lesions. In the spinal cord they showed to be almost 400 times the standard value. Electron microscopic studies of the spinal cord revealed a primary lesion of the oligodendroglial and astroglial cells. The topography of the neuroglial cells favours the uptake of 6-AN and explains the cellular lesion which results in a disturbance of the functional relationship between glial and nerve cells in the spinal gray matter.

    Notes: Summary Seven patients with cardiac dysrhythmias received a single, oral, therapeutically effective dose of the antiarrhythmic drug propafenone. The serum levels correlated well with changes in atrioventricular conduction times during the postabsorptive elimination phase (r=0.6810, p〈0.001). The mean half-lives of the serum level and the electrocardiographic effect (PQ-time) were 3.6±0.2 h and 3.8±0.6 h, respectively.

    Notes: Abstract Cell-mediated immune reactions play the most important role in the pathogenesis of chronic viral and auto-immune hepatitis. Cellular cytotoxicity (CC) of peripheral blood lymphocytes against autologous hepatocytes isolated from liver biopsies was studied in 29 children with different types of hepatitis B surface antigen (HBsAg)-positive hepatitis. Children with chronic hepatitis B showed higher cytotoxicity than control patients. However, a correlation of cytotoxicity to serum amino-transferases, HBeAg-/Anti-HBe-status, and hepatitis B virus DNA in serum could not be found. Children with a higher percentage of hepatitis B core antigen (HBcAg) expression in their liver tissue presented lower CC values, and vice versa. This supports the hypothesis that virus elimination occurs via T-cell attack against HBcAg expressing hepatocytes. Furthermore, children with a longer duration of chronic hepatitis B had considerably higher cytotoxicity values. Possibly, CC plays a role in perpetuating liver damage after infection with hepatitis B virus.

    Notes: Abstract The effect of different feeding regimens, notably the use of hydrolysed cow’s milk formulas, on the development of allergic reactions and the development of cow’s milk protein-IgG antibodies is still disputed. We prospectively compared the development of allergic manifestations and cow’s milk protein-IgG antibodies in a total of 702 infants who were divided into six groups: 1. exclusively breast milk for at least 4 weeks (n = 206). 2. Breast milk plus initial partially hydrolysed formula (n = 104). 3. Breast milk plus extensively hydrolysed formula (n = 50). 4. Breast milk plus initial conventional cow’s milk formula (n = 73). 5. Conventional cow’s milk with or without breast milk throughout (n = 187). 6. Extensively hydrolysed cow’s milk formula for 2 months, followed by conventional cow’s milk (n = 82).
