    Notes: Summary The intravascular transit of malignant tumor cells constitutes an important step in the formation of distant metastases. The development of tumors in extravascular tissues depends upon the exit of these cells from the circulation by crossing the barriers formed by endothelium and basement membrane and their growth in the extravascular environment. Cytostatic drugs may disturb the function of these barriers and some of them, as well as antiemetic drugs, are given as i.m. injections. Thus these both mechanically and chemically induced endothelial and extravascular tissue changes might facilitate tumor cell transit to extravascular tissues. These aspects are discussed in this case report of a young female patient with a mediastinal germ cell tumor. At 4 years after complete remission induced with chemotherapy and radiotherapy she developed recurrent germ cell tumor in the form of a s.c. gluteal abscess. This gluteal region is the most common site of i.m. injections during or after cancer chemotherapy.

    Notes: Summary The binding of a panel of lectins to histological sections of seminomas was studied. The findings were correlated with different clinical parameters. Concanavalin A and wheat germ agglutinin stained all seminomas whereas horse gram (DBA) and peanut agglutinin did not stain the tumor cells. A varying staining pattern was found with lectins from castor bean (RCA I), soy bean (SBA) and gorse (UEA I) indicating a heterogeneity of the tumor cell population. In the seminomas that were derived from undescended testis there were more cases that showed positive staining with soy bean agglutinin, which shows that the intra-abdominal location of the seminoma might cause changes in the cellular metabolism resulting in glycoconjugates different from those in descended tumors. No correlation was found between the lectin staining and the prognosis, stage or metastasis of the tumor.

    Notes: Summary The effect of actinomycin D on the ultrastructural changes induced by 5 mg of progesterone in the immature chick oviduct epithelium was studied at 0–24h after injection. The height of the epithelium increased during the experimental period. When actinomycin D was given half an hour before progesterone, it inhibited the formation of secretory granules, whereas it could not prevent the formation of the organelles involved in protein synthesis (polysomes, rough endoplasmic reticulum, Golgi apparatus) and the increase in the amount of mitochondria. On the other hand, actinomycin D enhanced the formation of progesterone induced secretory granules if it was administered after the steroid. Apical vacuoles were present within 2h after a combined treatment with progesterone and actinomycin D. Actinomycin D did not affect the changes in the plasma membrane caused by progesterone. The formation of microvilli and cilia began 7 and 12h, respectively, after progesterone. The epithelial cell plasma membranes developed a marked interdigitation, first observable 2h after progesterone administration. The intercellular spaces were markedly wider after actinomycin D, but not after progesterone only. There were significant changes in nuclear morphology after actinomycin D and progesterone injections. There was marked nuclear shrinkage as well as a significant decrease in the amount of nuclear pores by 2h after actinomycin D, administered either before or after progesterone. The amount of perichromatin granules (35–45 nm) increased following actinomycin D administration. There were no changes in the other nuclear granules. With prior actinomycin D administration, the amount of nucleoli per nucleus was decreased 7h after progesterone, whereas their amount increased within 2h if the actinomycin D treatment followed progesterone injection. Actinomycin D and progesterone treatment increased the incidence of nucleolonemata. Detachment of heterochromatin from the nuclear membrane was slightly delayed by actinomycin D, when given before progesterone. The results can be explained on the basis of transcriptional and post-transcriptional effects of actinomycin D.

    Notes: Abstract We describe a young woman with Burkitt's lymphoma, treated with intravenous adriamycine and cyclophosphamide and intrathecal cytarabine. She developed a reversible posterior leukoencephalopathy syndrome (RPLS) with typical MRI findings. Diffusion-weighted images during the first days after the onset of symptoms predicted a small irreversible lesion in the frontal lobe, verified on T2-weighted images 1 month later. The patient showed full recovery after high-dose steroid treatment.

    Notes: Summary Prostate-specific antigen (PSA) and prostatic acid phosphatase (PAP) serum levels were measured in 117 patients with prostatic adenocarcinoma, in 9 patients with prostatic hyperplasia and in 14 patients with other malignancies to compare the clinical usefulness of the PSA and PAP levels. PSA was elevated (PSA+) in 14 of 18 untreated patients (78%) with prostatic cancer. PAP was elevated (PAP+) only in 3 of these untreated cases (17%). Also in previously treated patients PSA was more often positive than PAP. PSA was positive in 40 of the 99 treated patients (40%), PAP was elevated only in 21 cases (21%). There was a significantly (P〈0.001) higher tendency towards elevated PSA in the prostatic cancer patients: 32 (27%) patients with PSA+ and PAP- compared with only 2 cases (2%) with PAP+ and PSA-. The PSA+/PAP- patients were analyzed further. In seven of them the PSA level also returned to its normal level after orchiectomy or/and radiotherapy. In two patients the PSA levels indicated tumor progression earlier than PAP, their PAP levels did not rise until bone metastasizing was evident. There were also progressive disease in some patients evidenced only by increased PSA levels. In addition to cancer patients the PSA level was increased in three (30%) of the prostatic hyperplasia patients. It was also elevated in three patients with other malignancies. However, these three patients also had prostatic hyperplasia and the increase in the PSA level is considered more likely to be due to that. According to these findings it is suggested that PSA is more sensitive than PAP in local and advanced prostatic cancer and may be more useful in monitoring responses and recurrence after therapy.

    Notes: Summary The direct influence of androgens on prostatic development was studied by culturing explants of urogenital sinuses of human embryos and fetuses aged 6 to 13 weeks in the presence of either 10-7 mol/l testosterone or dihydrotestosterone for 6 days. The differentiation of prostatic epithelial cells started both in control and androgen-treated explants in the ninth week, and androgens accelerated the differentiation of the secretory pathway organelles in the epithelial cells. The amount of granular endoplasmic reticulum and the number of Golgi complexes increased in the presence of androgens, especially in the older fetuses. Incipient secretory activity appeared in some cells during the culture and the differentiation occurred sooner in the androgen-treated explants than in the controls or in vivo. The differentiation and maintenance of the urogenital mesenchyme was also better in the presence of the hormone. The direct epitheliomesenchymal cell contacts seen earlier in vivo were also seen during the differentiation in vitro. These contacts may indicate an inductive role of the mesenchyme in the glandular morphogenesis. Androgens were not able to induce the epithelial differentiation in vitro before the first signs of mesenchymal differentiation were evident in vivo. The results suggest either that the mesenchymal changes are not solely determined by androgens or that the early explants may require additional factors for proper differentiation.
