Zusammenfassung Bei 962 Paraproteinämien wurde die Klassenzugehörigkeit des Paraproteins ermittelt: IgG-Paraproteinämie 627 Fälle=65%, IgA-Paraproteinämie 175 Fälle=18%, IgM-Paraproteinämie 118 Fälle=13%, Doppelparaproteinämie 13 Fälle=1%, Bence-Jones-Plasmocytom 29 Fälle=3%. Berücksichtigt man nur die IgG-, IgA- und IgM-Fälle, so erhält man als Prozentrelationen IgG=68%, IgA=19% und gM=13%. Somit entspricht die Häufigkeit der einzelnen Paraproteinämie-Klassen der relativen Serumkonzentration der einzelnen Immunogloubline bei altersentsprechendem Normalkollektiv. Bei 717 Paraproteinen wurde der Leichtkettentyp bestimmt. Für die Gesamtparaproteinämien betrug das Verhältnisx/λ ca. 1,7:1, für IgG-Paraproteinämie 1,7:1, für IgA 1:1, für IgM 4:1, für mikromolekulare Paraproteine 1,7:1 und für Doppelparaproteinämien 2,5:1. Diese Angaben stimmen mit denen der normalen Immunglobuline überein, mit Ausnahme der IgM-Paraproteine. Zwischen derx-λ-Verteilung der IgM-und den übrigen Paraproteinämien besteht ein signifikanter Unterschied. Die clektrophoretische Häufigkeitsverteilung unserer IgG- und IgM-Paraproteine sowie der normalen IgM-und IgG-Globuline lassen sich, aufgrund der durchgeführten statistischen Teste, durch eine angepaßte Normalverteilungskurve zweiter Art und die IgA-Paraproteine und die normalen IgA-Proteine durch eine Normalverteilung erster Art darstellen. Die elektrophoretische Verteilung der Paraproteine erstreckt sich wie die der normalen Immunglobuline vom α2 bis in den langsamenγ-Bereich. Die angepaßte elektrophoretische Häufigkeitsverteilung zeigt mit denen der entsprechenden normalen Immunglobulin-Klassen eine deutliche Ähnlichkeit. Nach strenger statistischer Testung besteht ein signifikanter Unterschied zwischen der nichtangepaßten Verteilungskurve der IgG- bzw. IgA-Paraproteine und der der normalen IgG-bzw. IgA-Globuline, nicht jedoch zwischen der Verteilungskurve der IgM-Paraproteine und der der normalen IgM-Globuline. Zwischen dem Leichtkettentyp des Paraproteins und seiner elektrophoretischen Mobilität und sedimentationsanalytischen Eigenschaft besteht keine Bezichung. Aufgrund unserer heutigen Kenntnisse über die normalen Immunglobuline und Paraproteine sind wir berechtigt anzunehmen, daß Paraproteine keine strukturell abnormen, funktionell inerten sondern, exzessiv vermehrte, physiochemisch und immunologisch einheitliche Immunglobuline sind.

Summary In 962 paraproteinemias the class distribution revealed the following data: IgG paraproteinemia 627 cases (65%), IgA paraproteinemia 175 cases (18%), IgM paraproteinemia 118 cases (13%), double paraproteinemia 13 cases (1%), Bence Jones plasmacytoma 29 cases (3%). With single respect to the main classes IgG, IgA and IgM, the percentage changes to 68%, 19% and 13% respectively. By this the frequency distribution of the main paraproteinemia classes equals the relative serum concentration of the corresponding normal immunoglobulins of an age-adequate normal collective. The light chain type was determined in 717 paraproteins. The followingx/λ ratios were found: total of paraproteins 1.7:1, IgG paraproteins 1.7:1, IgA p. 1:1, IgM p. 4:1, micromolecular p. 1.7:1 and double p. 2.5:1. These ratios agree with the figures of normal immunoglobulines except for the IgM paraproteins. There is a significant difference between thex/λ ratio of IgM paraproteins and paraproteins of other classes. Following statistical analysis, the electrophoretical frequency distribution of our IgM and IgG paraproteins as well as of a pool of normal IgG and IgM immunogloublins are better represented by a fitted normal distribution curve of second mode (asymmetrical normal distribution curve). In comparison to this the IgA paraproteins and corresponding normal immunoglobulins are more similary distributed following a normal Gaussian distribution curve of first mode (symmetrical d. c.). Like normal immunoglobulins, the electrophoretical distribution of paraproteins extends from α2 to the slowγ range. The fitted electrophoretical frequency distribution of paraproteins reveals an obvious similarity with that of the corresponding normal immunoglobulins. By severe statistical analysis, however, there was a significant difference between the non-fitted distribution curves of IgG and IgA paraproteins and the normal IgG and IgA immunoglobulins, respectively, in contrast to the distribution curves of IgM paraproteins and IgM normal immunoglobulins. No correlation was found between the light chain type of paraprotein and its electrophoretical mobility or sedimentation characteristics. By our actual knowledge about normal immunoglobulins and paraproteins the assumption is allowed that paraproteins are not structurally abnormal neither functionally inert but excessively increased, physicochemically and immunologically homogeneous immunoglobulins.
