    Notes: Abstract We report a rare case of recurring idiopathic ileoileal intussusception in an adult. Diagnosis was established with abdominal computed tomography (CT) and enteroclysis, which led to a spontaneous reduction of the invagination. After a short period of physical improvement, a follow-up CT showed a recurrence. Surgery proved the diagnosis, but no predisposing factor was found.

    Notes: Abstract We present a 26-year-old male patient with Gaucher disease who presented with epigastric pain and a palpable mass in the left abdomen. Ultrasound, abdominal computed tomography, and magnetic resonance imaging showed massive splenomegaly with multiple splenic nodules up to 7 cm in diameter. Splenic nodules should be included in the differential diagnosis of splenic masses. Follow-up is necessary because of the increased incidence of hematologic malignancies in Gaucher disease.

    Notes: Abstract. The purpose of this study was to assess the accuracy of multi-parameter measurements with color-coded duplex sonography (CCDS) for the diagnosis of venous leakage in patients with erectile dysfunction. Sixty patients with repeated unsatisfactory reactions after intracavernous injection of vasoactive substances underwent CCDS. Following intracavernous injection of prostaglandin E1, peak systolic velocity (PSV), enddiastolic velocity (EDV), time averaged velocity (TAV), resistance index (RI), and pulsatility index (PI) were measured in the cavernous arteries over 30 min (one measurement per minute). The results were compared with independent measurements based on dynamic pharmaco-cavernosometry/cavernosography (DPCC). Dynamic pharmaco-cavernosometry/cavernosography revealed venous leakage in 33 patients. Of 48 patients with normal PSV ( 〉 25 cm/s), 25 had veno-occlusive dysfunction and the remainder presented normal venous function. No statistically significant differences between these groups were found in EDV, RI, and PI measurements. In contrast, differences in TAV were significant between patients with (mean 9.4 ± 4.6 cm/s) and without venous leakage (mean 5.5 ± 2.2 cm/s; p = 0.001). Analysis of relative frequencies revealed a broad overlap of EDV, TAV, RI, and PI measurements between both groups. Sensitivities and specificities determined from receiver-operating-characteristic curves were 〉 80 % and 〉 50 % for a TAV threshold of 5 cm/s, and an RI threshold of 1.0. Measurements of EDV, TAV, RI, and PI in patients with repeated unsatisfactory reactions on intracavernous prostaglandin injection are poor predictors of venous leakage and should not replace DPCC in the investigation of vasculogenic impotence.

    Notes: Abstract. The aim of this study was to determine the accuracy in quantifying right and left ventricular volumes using a 1.0-T system and commercially available, standard equipment. For exact comparison of MRI measurements and real volumes we used an animal heart model ex vivo. Eight pig hearts were explanted and prepared by removal of the atria. Aorta and pulmonary truncus were cannulated. Definable volumes were injected into the ventricles. Magnetic resonance imaging was performed at 1.0 T (Gyroscan T10 NT, Philips, Eindhoven, The Netherlands); sequence: fast field echo–echo planar (multishot EPI); body coil; MR software: Cardiac Application Package (Philips). Statistical analysis correlated the real volumes and MR measurements separately for both ventricles and two investigators (SAS, ANOVA). For both ventricles and both investigators the correlation between real volumes and MR measurements was greater than 0.99. There was no significant systematic false estimation for both ventricles. Magnetic resonance imaging at 1.0 T using standard hardware and software equipment enables the quantification of right and left ventricular volumes with high approximation to the real volumes in vitro. There is a clear restriction in translating these data into a clinical application because under experimental conditions no motion-induced artifacts existed.

    Notes: Abstract. We report in vitro T1 and T2 relaxation studies for the open-chain complexes Gd-DTPA and Gd-DTPA BMA. Measurements were performed on phantoms containing aqueous and plasma solutions of different concentrations by MR imaging in a 1.5T superconducting whole-body scanner. Longitudinal relaxation times T1 were evaluated from serial turbo-FLASH experiments for concentrations less than 1 mM, whereas for larger concentrations the values were obtained from a standard inversion recovery (IR) sequence. Transverse relaxation times T2 were determined using multi-echo spin-echo MRI protocols. The T1 and T2 relaxivities of the nonionic Gd-DTPA BMA are similar to those of the Gd-DTPA. The temperature dependencies of the relaxivities were determined over a temperature interval ranging from 21 to 50 °C and were found to be slightly different for the two contrast agents. In the case of Gd-DTPA BMA a larger deviation of the expected temperature behavior of the relaxivities was observed as compared with Gd-DTPA. Deviations from a strictly linear dependence of relaxation times on temperature were found at lower concentrations in aqueous solutions. In plasma solutions a high T1/T2 ratio was observed for low concentrations, which decreased monotonically with increasing concentrations.

    Notes: Abstract. Gaucher's disease type I is the most prevalent lysosomal storage disorder caused by an autosomal-recessive inherited deficiency of glucocerebrosidase activity with secondary accumulation of glucocerebrosides within the lysosomes of macrophages. The storage disorder produces a multisystem disease characterized by progressive visceral enlargement and gradual replacement of bone marrow with lipid-laden macrophages. Skeletal disease is a major source of disability in Gaucher's disease. Extraosseous extension of Gaucher cells is an extremely rare manifestation of skeletal Gaucher's disease. This is a report on the MRI and histopathological findings of an extraosseous Gaucher-cell extension into the midface in a patient with Gaucher's disease.

    Notes: Abstract On a 1.5-T MR unit conventional spin-echo (CSE) and fast spin-echo (FSE) images were obtained in 70 consecutive children ranging from 4 days to 13 years in age. They suffered from developmental retardation, neurodegenerative, ischemic or inflammatory diseases. In our prospective study artifacts, lesion conspicuity and general impression were compared between double-echo (proton density- and T2-weighted) CSE and single-echo (T2-weighted) FSE images. Phase artifacts from flowing blood were seen rarely on the FSE images, while motion artifacts appeared more frequently. Assessment of myelination in 43 children with unfinished myelination revealed no difference between FSE and CSE. The lesion conspicuity in 20 children with focal abnormalities compared favorably between FSE and CSE. There was no diagnostically relevant difference between FSE and CSE, although in FSE spin density images were missing. FSE sequences can be used in MR imaging of the pediatric brain without disadvantage and with a time reduction of 67–75%.

    Notes: Abstract This prospective study included 43 patients undergoing renal transplantations. Magnetic resonance imaging (MRI) and X-rays of the hip joints were produced 3 and 12 months after transplantation. In 6 hip joints of 4 patients (9.3%), we discovered femoral head necroses just 3 months after transplantation. Three of the hip joints affected were symptomatic and 3 painless. The MR images taken 12 months after transplantation revealed no additional femoral head necrosis. A core decompression was performed on 3 joints. In contrast to those with core decompression, the femoral heads without core decompression showed a progression of the necrosis in 2 of 3 cases. All 4 patients with femoral head necroses were younger than 50 years and exhibited a premature conversion of the haematopoietic marrow to fatty marrow in the area of the proximal femoral metaphysis. A similar premature conversion to fatty marrow was seen in 6 of the 22 (27.2%) patients younger than 50 years and without femoral head necroses. The doses in long-term steroid medication and the steroid doses during the rejection periods of the patients with femoral head necroses were not significantly higher than those for the patients with premature conversion to fatty marrow. The latter had also not received significantly higher doses than the patients whose MRI findings were inconspicuous.

    Notes: Zusammenfassung Zur Strahlentherapie von malignen Tumoren im HNO-Bereich ist die genaue Kenntnis des Zielvolumens Voraussetzung. Die Bedeutung des Computertomogramms (CT) für die Strahlenthera-pieplanung soll am Beispiel des Nasopharynxcarcinoms (NPC) aufgezeigt werden. Es werden Planungs- und Fallbeispiele der häufigsten Primärtumorsituationen unter Einschluß der regionären Lymphbahnen mit Hilfe dieser Anordnung demonstriert. Für das NPC sind dies: 1. Primärtumor auf den Nasopharynx begrenzt (T1–2Nx). 2. Ausdehnung des NPC in die Nasenhaupthöhle (T3NaseNx) und/oder 3. den Mesopharynx (T3MesoNx), ggf. unter Einschluß der Retroman-dibulärgrube oder fossa pterygopalatina. 4. Destruktion der Schädelbasis mit/ohne Hirnnervenbeteiligung (T4Nx). 5. Einbeziehung der regionären Lymphabflußwege in Abhängigkeit ihrer N-Kategorien (TxN0–3). Mit Hilfe einer rechnergestützten Planung kann die Bestrahlung der individuellen Tumorausdehnung sowie der jeweils vorliegenden regionären Lymphknotenmetastasierung angepaßt werden.
