    Notes: Abstract In order to identify relevant genetic lesions in gastric carcinoma, we searched for tumor suppressor gene inactivation and K-ras gene mutations by analyzing tumor and control DNAs from 34 patients. These were from an epidemiologically defined area of Italy characterized by one of the world's highest incidences of stomach cancer. Allele losses were investigated by the Southern blotting procedure at 16 polymorphic loci on 11 different chromosomes. Our data demonstrate that chromosomal regions 5q, 11p, 17p and 18q are frequently deleted, and that 7q and 13q chromosome arms are also involved, although at a lower frequency. Loss of heterozygosity (LOH) at region 11p was not found during other surveys carried out on patients of different geographic origins. No specific combination of allelic losses could be recognized in the samples analyzed, the only exception being that tumors with 17p allelic loss also showed LOH on the 18q region. When matching frequent LOH events and the stage of progression of the tumors, we observed a trend of association between advanced stages and allelic losses on 17p and 18q chromosome arms. The analysis of K-ras, carried out by the polymerase chain reaction and denaturing gradient gel electrophoresis, demonstrated transforming mutations in only 3 out of 32 cases. Colorectal tumorigenesis proceeds by the accumulation of genetic alterations, including K-ras mutations and inactivation of tumor suppressor genes on the 5q, 17p and 18q regions. Our data indicate that, although gastric and colorectal neoplasias share common genetic alterations, they probably progress through different pathways.

    Notes: Abstract This prospective, multicenter, epidemiological study was carried out in 99 Italian ICUs, distributed throughout the country, from April 1993 to March 1994. In the study, we applied the new ACCP/SCCM classification system for sepsis (SIRS, sepsis, severe sepsis and septic shock) and determined the prevalence, incidence, evolution and outcome of these categories in critically ill patients. The preliminary analysis of 1101 patients showed that on admission SIRS accounted for about half of the diagnoses (52%) with sepsis, severe sepsis and septic shock accounting for 4.5%, 2.1% and 3% of patients, respectively. Patients with severe sepsis or septic shock more frequently had high SAPS scores than patients without sepsis. Mortality rates were similar in patients with SIRS (26.5%) and without SIRS or infection (24%), but rose to 36% in patients with sepsis, to 52% in those with severe sepsis and to 81.8% in those with septic shock. Sepsis, severe sepsis and septic shock were more common in patients with medical diagnoses, and neither severe sepsis nor septic shock was observed in trauma patients. With respect to evolution, the incidence of septic shock was progressively higher in patients admitted with more severe “sepsis-related” diagnoses, while only a trivial difference in rates of incidence was observed between SIRS patients and those admitted without SIRS or any septic disorder (nil). The breakdown of the various ACCP/SCCM “sepsis-related” diagnoses at any time during the study was: SIRS in 58% of the population, sepsis in 16.3%, severe sepsis in 5.5% and septic shock in 6.1%. It seems reasonable to expect from the final evaluation of our study answers to the questions raised by the ACCP/SCCM Consensus Conference about the correlations between “sepsis-related” diagnosis, severity score, organ dysfunction score and outcome.

    Notes: Summary A controlled study of the clinical pharmacology of the biliary tract has been made. The rate of gallbladder emptying induced by a fatty meal was taken as a parameter for assessment of the inhibitory effect of indoprofen, a new analgesic-anti-inflammatory drug, pentazocine and morphine. The compounds were administered as single doses by iv (indoprofen and pentazocine) or im (morphine) injection. Indoprofen up to 400 mg had no effect, whereas morphine and pentazocine exerted a significant inhibitory effect on emptying of the gallbladder. Adverse reactions of clinical significance were associated with morphine and pentazocine, but were negligible with indoprofen.

    Notes: Abstract Superficial siderosis of the central nervous system is a rare condition characterized by deposition of haemosiderin in the leptomeninges and in the subpial layers of the brain and spinal cord. With the widespread use of magnetic resonance imaging, an increasing number of cases of superficial siderosis are being discovered, secondary forms being more frequent than idiopathic ones. We report a 78-year-old man in oral anticoagulant therapy, who presented neurosensory hearing loss, gait ataxia and spastic paraparesis. Magnetic resonance imaging suggested the diagnosis of superficial siderosis of the central nervous system, without an evident bleeding source.

    Notes: A battery of visual and spatial perception tests (VOSP) has been administered to a sample of 25 mild Alzheimer's disease (AD) patients and 25 age- and education-matched controls in order to assess visuospatial skills in the early phase of the disease. Among visual object perception tests, AD patients were impaired only in the task where they had to name the stimulus (Silhouettes task). The score in this task significantly correlated with the Boston Naming Test score (r= .73; p 〈 0.0001). The performance on the Object decision task (which does not require naming), figure-ground analysis, and identification of degraded stimuli was unimpaired. Similarly there were no significant differences for any of the spatial perception tests. The results showing that the performances on most measures of visual and spatial perception is unimpaired, indicate that the “early” perceptual processes, as well as the access to structural knowledge, are not defective in early AD. On the other hand, AD patients are impaired at the level of semantic access and name retrieval. The finding of an highly significant correlation among tests requiring visual access to structural and semantic knowledge suggests that a rigid separation between perceptual and semantic mechanisms may be unwarranted.

    Description / Table of Contents: Sommario L'interesse nei confronti delle funzioni frontali è progressivamente aumentato negli ultimi anni. Nonostante ciò, solo per pochi test “frontali” sono disponibili dati normativi italiani. Lo scopo di questo studio era ottenere valori normativi per due test “frontali” utilizzando un campione di soggetti normali selezionati in maniera casuale. Il primo test, ossia il Test di Classificazione e Rievocazione di Figure, risulta selettivamente compromesso in pazienti sottoposti a escissione frontale unilaterale; il secondo test, ossia il Test della Percora Nera, deriva da compiti utilizzati per rilevare la compromissione frontale negli animali ed è stato usato per individuare una disfunzione di tipo frontale nei parkinsoniani. Sono stati testati 100 soggetti normali, di età compresa fra i 22 e i 79 anni, stratificati per scolarità secondo il modello scolastico italiano. Vengono presentate le griglie di correzione per entrambi i test. La loro utilità consiste non solo nella loro sensibilità al danno frontale, ma anche nella possibilità di dissociare aspetti cognitivi differenti della disfunzione dei lobi frontali.
    Notes: Abstract Interest in frontal functions has progressively increased over recent years; however, despite this, there are only a few “frontal” tasks for which Italian normative data are available. The objective of this study was to obtain reference values for two frontal tests from a random sample of normal adults: the first (the Test of Classification and Recall of Pictures) has been shown to reveal selective impairment in patients with unilateral frontal excision; the second (the Odd-Man-Out Test) derives from tasks which have been used to evaluate frontal functions in animals and detect frontal impairment in Parkinson patients. We tested 100 normal subjects, aged 22 to 79 years, stratified by education according to the Italian school system. The correction grids for both tests are also presented. The usefulness of these tests is represented not only by their sensitivity to frontal impairment, but also by the possibility of dissociating the various cognitive aspects of frontal lobe dysfunction.

    Notes: Abstract We are herewith reporting new data which modifies, explains and extends our earlier work on this subject. Previous observations of low CH4 conversion and high selectivity to C2 hydrocarbons were erroneous due to the formation of bulk CaCO3 in the catalyst. The carbonate was detected by powder X-ray diffraction and was shown to accumulate during the reaction and decompose during regeneration. Catalytic runs which incorporated an internal standard revealed a deficit in the C balance consistent with carbonate formation. Actual CH4 conversions were ≈ 20% with ≈ 15% selectivity to hydrocarbons. The effect of steam in promoting coupling over combustion was affirmed.

    Notes: Abstract Model Ziegler-Natta catalysts are prepared by gas phase deposition of ultra-thin TiCl4/ MgCl2 films in UHV conditions. A monolayer of TiCl4 chemisorbed on a solid solution of titanium and magnesium chloride is formed in this way. The reduction and alkylation of TiCl4 by its reaction with a liquid layer of AlEt3 condensed on the halide film is monitored by XPS. Most of the TiCl4 is reduced by AlEt3 and is incorporated in the mixed titanium /magnesium chloride. The model catalyst is active in the polymerization of ethylene and propylene at 300 K, both in the absence and in the presence of AlEt3 in the reaction cell. The polymers that form over the catalyst film have been characterized by Raman spectroscopy. The weak signals from methyl end groups and unsaturations suggest high molecular weight for both polymers. The polypropylene film has a high degree of isotacticity even without the use of any electron donor. For the propylene polymerization reaction the overall turnover frequency is in the range between 0.1 and 1 molecule/(site s).
