    Notes: Abstract. The physiological metabolism of proteins guarantees that different cellular compartments contain the appropriate concentration of proteins to perform their biological functions and, after a variable period of wear and tear, mediates their natural catabolism. The equilibrium between protein synthesis and catabolism ensures an effective turnover, but hereditary or acquired abnormalities of protein structure can provoke a premature loss of biological function, an accelerated catabolism and diseases caused by the loss of an irreplaceable function. In certain proteins, abnormal structure and metabolism are associated with a strong tendency to self-aggregation into a polymeric fibrillar structure, and in these cases the disease is not principally caused by the loss of an irreplaceable function but by the action of this new biological entity. Amyloid fibrils are an apparently inert, insoluble, mainly extracellular protein polymer that kills the cell without tissue necrosis but by activation of the apoptotic mechanism. We analyzed the data reported so far on the structural and functional properties of four prototypic proteins with well-known biological functions (lysozyme, transthyretin, β2-microglobulin and apolipoprotein AI) that are able to create amyloid fibrils under certain conditions, with the perspective of evaluating whether the achievement of biological function favors or inhibits the process of fibril formation. Furthermore, studying the biological functions carried out by amyloid fibrils reveals new types of protein-protein interactions in the transmission of messages to cells and may provide new ideas for effective therapeutic strategies.

    Description / Table of Contents: Résumé Vingt prothèses totales de hanche présentant un descellement fémoral aseptique de stade III ou IV ont été reprises par une tige fémorale sans ciment personnalisée revêtue d'hydroxyapatite. Une greffe fémorale a été associée dans la moitié des cas. A deux ans de recul moyen : - 15 prothèses sont restées parfaitement stables,- 4 ont présenté un enfoncement modéré, inférieur à 0,5 cm ; - 1 cas enfin représente un échec de fixation avec migration évolutive de 1,5 cm. Les greffes osseuses semblent s'être assimilées préférentiellement lorsqu'elles étaient placées en situation endostée. Les résultats cliniques furent favorables puisque le score moyen selon la cotation de Merle d'Aubigné est passé de 8,4 en préopératoire à 14,4 au dernier contrôle. Deux fractures peropératoires du fémur n'ont pas compromis le résultat final. La conception de la tige fémorale personnalisée assistée par ordinateur semble avoir amélioré la stabilité primaire dans des fémurs trés remaniés et le revêtement d'hydroxyapatite a paru induire une ostéo intégration assez rapide. Toutefois, la qualité de ces résultats préliminaires ne saurait préjuger de l'avenir et un recul supérieur est nécessaire pour des conclusions définitives.
    Notes: Abstract Twenty total hip arthroplasties with stage III and IV aseptic femoral loosening were revised with a custom cementless hydroxyapatite coated femoral component. A femoral graft was used in 50% of the cases. At an average of two years follow up: - 15 prosthesis remained perfectly stable; - 4 exibited slight early and non progressive subsidence less than 0.5 cm; - finally one case is a failure with progressive migration of 1.5 cm. The bonegrafts seemed to be preferentially incorporated in the endosteal situation. The clinical results were favourable since the average score according to the Merle d'Aubigné hip scale was improved from 8.4 before operation to 14.4 at the last check-up. Two intra operative femoral fractures have not jeopardized the final result. The conception of the computerdesigned custom-made femoral stem seemed to improve the primary stability in very altered femorae and the hydroxyapatite coating has seemed to induce rather quick bony integration. Nervertheless, the quality of these preliminary results cannot prejudge the future and a longer follow up is required for definitive conclusions.

    Notes: Summary The case of a spontaneous cervical epidural hematoma treated by anterior corporectomy and arthrodesis is reported. An anterior approach is preferable when an epidural hematoma is anterior to the dural sac and when MRI shows an aspect of old clotted blood that can not be easily evacuated by a posterior laminectomy.

    Description / Table of Contents: Résumé Les auteurs rapportent deux cas de fracture de tête fémorale en céramique, l'une survenue à la suite d'un choc traumatique, et l'autre sans cause apparente. Après une revue de la littérature, ils étudient les différents mécanismes pouvant entraîner une fracture de tête en céramique. Il peut s'agir d'un défaut de fabrication, d'un choc traumatique, ou d'une erreur de manipulation, le cône morse représentant le point faible du système. Ils concluent à la nécessité de ne pas employer une tête neuve sur un cône usé, ni de l'impacter fortement, et de vérifier l'absence de toute interposition au sein du cône morse.
    Notes: Summary Fracture of the ceramic head of a replacement arthroplasty of the hip has been encountered in 2 patients. After studying the literature the causation of such fractures has been assessed. Defects in manufacture of the prosthesis, damage during its handling and incorrect use may be responsible, with the tapered neck being the principal instrument of failure. The use of a new head with a used tapered neck is not recommended. Care must be taken in placing the head on the neck and a precise fit is required with no interposition of soft tissue.

They conclude that it is an inflammatory exudate with perhaps some immunological significance. A small amount of metal elements were seen, but there was no evidence of overt wear.

    Notes: Abstract. We have recently described an anti-β 2-microglobulin (β2-m) monoclonal antibody (mAb 14H3) capable of recognizing the epitope 92–99 of the protein in the monomeric native state as well as in the fibrillar polymeric state, but not in the major histocompatibility complex type I (MHCI) anchored to the cell membrane. In the present study, we investigated the molecular basis for the inaccessibility of the C-terminal end of β2-m in the MHCI complex, and demonstrated that mAb 14H3 binds the soluble fraction of the MHCI complex with a Kd of 0.3 μM. An interaction between the complex and the membrane protects β2-m from immunological recognition at the MHCI level. This protection from antibody recognition can be weakened by procedures such as heat shock or γ irradiation that perturb the membrane structure and commit the cell to the apoptotic pathway. mAb 14H3 can recognize MHCI in a transient state that most likely precedes β2-m shedding and may be proposed as a useful tool for dynamic analysis of MHCI conformational modifications.
