    Notes: Summary Humoral immunity to bacterial antigens was tested in 49 tissue typed patients with juvenile onset diabetes mellitus (JOD) and in 50 healthy controls. The number of patients with agglutinins to E. coli and staphyloccoci was significantly lower compared to controls (p〈0.001, p〈0.01 respectively). Missing antibody formation to pertussis and diphtheria toxoid could also be detected in a higher percentage of JOD patients than of controls (p 〈 0.05; p ≃ 0.05, respectively). By contrast heteroagglutinins to sheep and rabbit erythrocytes were found in similiar proportion in both groups and the values of immunoglobulin serum concentrations showed no difference between patients and controls. In addition no correlation between antibody formation and genes of the HLA complex was found. It is suggested that the severely reduced agglutinin formation to bacterial antigens might be partly responsible for susceptibility to bacterial infections in juvenile diabetics.

    Description / Table of Contents: Abstract The safety of blood and blood components is an important aspect of transfusion medicine. Even if absolute safety of blood products cannot be achieved, this goal has come closer due to significant improvements. These include: (1) exact donor registration; (2) component preparation according to legal requirements and GMP guidelines; (3) determination of all relevant diagnostic parameters according to GMP/GLP guidelines; (4) adequate storage of products; and (5) an often underestimated parameter ensuring safety, strict guidelines for the use of blood products in the treatment of patients. Some of the results of the first Graz Consensus Conference on Transfusion Medicine are presented.
    Notes: Zusammenfassung Die Sicherheit von Blut- und Blutkomponentenkonserven ist ein wichtiges Anliegen der Transfusionsmedizin. Auch wenn es bei der Anwendung transfusionsmedizinischer Präparationen keine absolute Sicherheit für den Therapieempfänger geben kann, so wird durch die Einbeziehung wesentlicher Sicherheitsverbesserungen große Annäherung an das nicht erreichbare Idealbild möglich gemacht. Zur Sicherheitsverbesserung zählen 1.) eine genau geführte Spendererfassung, 2.) die Herstellung der Blutkomponenten nach den Prinzipien des Arzneimittelgesetzes bzw. der GMP (Gute Herstellungsordnung)-Richtlinien, 3.) die diagnostische Aufarbeitung transfusionsmedizinisch relevanter Parameter und dabei die Beachtung von GMP-/GLP-Richtlinien, 4.) die adäquate Produktlagerung und -versorgung und – als leider zu wenig beachteter Sicherheitsgarant – 5.) die Beachtung strenger Indikationsrichtlinien für diesen Therapiezweig. In der vorliegenden Arbeit werden die Teilergebnisse der 1. Grazer Konsensustagung „Transfusionsmedizin”, welche die Indikationsrichtlinien betreffen, mitgeteilt.

    Description / Table of Contents: Zusammenfassung Die celluläre (mittels Migrations-inhibitionstest, MT) und humorale (mittels Immunfluorescenztechnik) Immunreaktion gegen Nebennieren-Antigene wurde bei 19 Patienten mit idiopathischer Nebennierenrindeninsuffizienz (INI) und 19 Kontrollpersonen untersucht. Bei 12 Patienten konnte eine ausgeprägte Hemmung der Migration mononuclearer Zellen (MM) in Gegenwart von NN-Mikrosomen beobachtet werden; dagegen lag bei 18 der 19 Kontrollpersonen die MM im Normbereich. Zusätzlich konnten bei 11 (53%) der Patienten Nebennierenrinden-Antikörper (NNR-AK) festgestellt werden. In einer weiteren Untersuchungsreihe wurden NN-Mikrosomen mit NNR-AK vorinkubiert, im MT eingesetzt und die erhaltenen Migrationsindices mit denen durch NN-Mikrosomen allein erzielten verglichen. Bei 6 der 10 NNR-AK positiven Patienten konnte eine Hemmung der MM mit NN-Mikrosomen durch spezifische AK blockiert werden. Bei den NNR-AK negativen Patienten war kein einheitlicher Trend zu einer negativen oder positiven Beeinflussung der MM nachweisbar. Eine Blockierung der cellulären Immunreaktion durch spezifische Antikörper in diesem System erscheint wahrscheinlich. Mögliche Mechanismen, die dazu führen könnten, sowie die Bedeutung der NNR-AK werden diskutiert.
    Notes: Summary In 19 patients with idiopathic adrenal insufficiency and in 19 controls, cellular (as measured by the migration inhibition test) and humoral (by means of immunofluorescence) immune reaction to adrenal antigens were investigated. Significant inhibition of migration was observed in 12 patients with adrenal microsomes; migration of mononuclear cells was however within normal range in 18 out of 19 controls. Adrenal antibodies could be detected in 11 (53%) of the patients. In a further series of investigations adrenal microsomes have been preincubated with specific antibody and used in migration inhibition tests. The migration indices observed with microsomes were compared to those obtained with microsomes preincubated with adrenal antibody. Migration inhibition with microsomes could be blocked by antibody in almost all antibody-positive patients. No uniform trend on the migration of mononuclear cells was found in the antibody-negative patients. A blocking of the cellular immune reaction by specific antibody seems likely. Possible mechanisms which could lead to such phenomena as well as the biologic significance of adrenal antibodies are discussed.

    Description / Table of Contents: Zusammenfassung Die Antikörperbildung gegen die Gammaglobulingruppen Gm (a,x,b,f) und Inv (l) wurde bei 54 Kindern (Frühgeburten, Nephrotikern, Hypound A-γ-Globulinämikern), die mehrfach gammaglobulinhaltige Therapeutica appliziert erhielten, untersucht. Dabei konnten bei 5 unter 28 Gm (a)-negativen von Gm (a)-negativen Müttern stammenden Kindern anti Gma festgestellt werden. Zwei anti Gma-Träger fanden sich unter 9 Gm(a)-negativen Kindern mit Gm (a)-positiven Müttern, die für das Steinberg-Speiser-Phänomen typische Mutter-Kind Konstellation. Die Ursachen für die Seltenheit des Auftretens von Antikörpern gegen Gammaglobulingruppen bei diesen kranken Kindern wird diskutiert, wobei besonders auf die Quantität des applizierten Gammaglobulins und auf die transiente Natur der anti Gm-Körper hingewiesen wird.
    Notes: Abstract The occurrence of antibodies against the gammaglobulin groups Gm (a,x,b,f) and Inv(1) was determined in children who were either been born prematurely or suffered from nephrosis, hypo-or agammaglobulinemia, and who were treated with gammaglobulin. Anti-Gma was produced in five out of 28 Gma negative children from Gma negative mothers. Two anti-Gma carriers were found amongst nine Gma negative children from Gma positive mothers, which is the typical mother-child combination with regard to the Steinberg-Speiser phenomenon. The reasons for the rarity of the formation of antibodies to gammaglobulin groups are discussed with special reference to the amount of gammaglobulin administered, and the transient nature of Gm antibodies.

    Notes: A close correlation between juvenile diabetes mellitus and HLA-B8, HLA-BW15 and HLA-CW3 was found. Association of these antigens with juvenile diabetes mellitus was closely dependent on the ages of onset of the disease. Frequencies of BW15 and CW3 showed a remarkably low incidence in the childhood diabetics (0–15 years old) and were found increased in the patients with later (16 years or older) onset diabetes. HLA-B8 frequencies were found increased in all the groups of diabetics with different ages of onset. These findings point to the importance of HLA-B8 in childhood and HLA-B8, BW15 and CW3 in the later onset juvenile onset diabetes mellitus (JOD).
