    Description / Table of Contents: Zusammenfassung. Einleitung: Resektionen ausgedehnter Tumore, Tumorrezidive und Radioderme am Hals führen zu komplexen Defekten, die unterschiedliche Deckungsvariationen benötigen. Patienten und Methoden: Bei 35 von insgesamt 57 interdisziplinär mit der Klinik und Poliklinik für Hals-Nasen-Ohrenkranke behandelten Patienten wurden freie Jejunumtransplantate zur Rekonstruktion eingesetzt. Ein zirkulärer Pharynxdefekt war bei 30 Patienten zu überbrücken, 5 Patienten erhielten eine freie Jejunumpatchplastik. Bei 4 Patienten wurde eine Neoglottis nach Ehrenberger [7] angelegt. Zur Hautweichteildeckung erhielten 2 Patienten freie, mikrovasculäre Scapulalappen, 8 Patienten ein abgeteiltes, antimesenterial eröffnetes und mucosektomiertes Jejunumsegment mit Spalthauttransplantat. Ergebnisse und Schlussfolgerungen: 34 Transplantate waren vital. Eine Transplantatnekrose konnte durch eine zweite Jejunumtransplantation erfolgreich behandelt werden. Eine Fistel an der Pharyngojejunostomie wurde mit dem zunächst als Monitorsegment ausgelagerten Jejunumanteil gedeckt, 2 weitere Fisteln mit Deltopectorallappen saniert. Eine Teilung eines freien, mikrovasculären Jejunumtransplantats ermöglicht eine einzeitige Rekonstruktion von Schluckweg, Sprache und Hautweichteilmantel mit einer arteriellen und einer venösen Mikroanastomose.
    Notes: Abstract. Introduction: Resections of extensive tumours, recurrences and radiodermatitides at the neck cause complex defects. Free bowel transplantation enables one-stage reconstruction. Patients and Methods: In 35 out of 57 interdisciplinarily treated patients, small-bowel transplantation was performed for reconstruction. Thirty patients received cylinder grafts, five jejunal patch plastics. In four patients neoglottis formations according to Ehrenberger were performed. To cover soft tissue defects, two patients received scapular free flaps, eight patients a jejunal segment, incised at the anti-mesenteric side, the mucosa stripped and covered by a split skin graft. Results and Conclusions: Thirty-four grafts were viable. One patient received a successful re-transplantation. One postoperative fistula at the hypo-pharyngojejunostomy was closed by a seperated jejunal segment which was primarily used as monitor, two others by pectoral flaps. Separation of a jejunal graft into two or three segments enables restoration of swallowing, voice and covering soft tissue in a one-stage procedure. The whole reconstruction necessitates only one arterial and one venous microanastomosis at the neck.

    Notes: Summary In a prospective study paired sera of 14 patients suffering from Bell's palsy were examined for antibodies against Varicella-zoster Virus (VZV), Herpes-simplex Virus (HSV), Cytomegalovirus (CMV), and Epstein-Barr Virus (EBV). For determination of antibodies against VZV, HSV, and CMV an enzyme-linked immuno sorbent assay was carried out. Indirect immunfluorescence was carried out to determine antibodies against EBV. Serum samples were taken within 5 days of Bell's palsy having been diagnosed and were compared with further serum samples that were taken 4 weeks later. No evidence could be furnished of significant differences between the antibody titers of the paired sera. The viral etiology of Bell's palsy due to an exogenous infection or by activation of a latent infection seems to be unlikely.

    Notes: Abstract Hemodilution is considered to be a useful therapy for sudden hearing loss by improving cochlear blood flow (COBF) as a result of decreasing viscosity of the whole blood. The purpose of this investigation was to evaluate whether hemoconcentration actually leads to a diminished COBF and impaired function of the cochlea to thus play a role in the pathogenesis of sudden hearing loss. Ten New Zealand White rabbits were anesthetized and ventilated. Cochlear function was evaluated by measuring distortion products of otoacoustic emissions (DPOAEs) at the beginning of each experiment and after 120 min. In the interim, each test animal’s hematocrit was raised by an infusion of packed red cells. Control animals were not infused, so hematocrits were left unchanged. Reproducibility of DPOAE measurements were found to be dependent upon the stimulus level. Correlation coefficients were 0.83 for 65 dB SPL and 0.78 for 45 dB SPL. Although no changes in the absolute level of DPOAEs were observed after raising the hematocrit, correlation coefficients were diminshed to 0.68 at 65 dB SPL and 0.58 at 45 dB SPL. Hemoconcentration caused no apparent changes in the cochlear function in our animals. Although these findings may reflect species differences, hemoconcentration might still be a factor causing sudden hearing loss in older, atherosclerotic patients.

    Notes: Summary The effect has been studied of intravenously administered glycerol on paracellular and cellular transport of macromolecular HRP tracer in the lateral cochlear wall. The findings indicate osmotically induced diffusion phenomena by the opening of shunt pathways through junctional complexes in the stria vascularis. Vesicular transport is accelerated, direction of transport however is not affected by the changed osmolality.

    Description / Table of Contents: Summary Transport of macromolecules in the spiral prominence of the guinea pig was studied after perilymphatic and intravenous horseradish peroxidase (HRP) injection. HRP was taken up by the spiral prominence epithelium from the perilymphatic space and in a much higher degree from the endolymphatic space. A highly endocytic ativity was shown by the stroma cells surrounding the spiral prominence vessel. HRP particles were stored in the cytoplasm within vesicles and vacuoles of different sizes and different contrasts. The mode of tracer storage in the spiral prominence epithelium and in the stroma cells from the morphological point of view suggests lysosomal digestion. HRP was transported from the perilymphatic space into the spiral prominence vessel lumen by means of vesicular transport through the vessel wall, while transport of the tracer from the vessel lumen into the perilymphatic space after intravenous injection was not observed.
    Notes: Zusammenfassung Nach perilymphatischer und intravenöser Applikation von Meerrettichperoxidase wurde der Transport des Makromoleküls in der Prominentia spiralis des Meerschweinchens verfolgt. Die Tracer-Partikelchen wurden aus dem perilymphatischen Raum und vornehmlich jedoch aus dem Ductus cochlearis in das Oberflächenepithel der Prominentia spiralis aufgenommen. Die das Vas prominens umgebenden Stromazellen nahmen relativ viel Meerrettichperoxidase auf, die im Zytoplasma in unterschiedlich großen und unterschiedlich kontrastreichen Vesikeln und Vakuolen abgelagert wurde. Die Art der zytoplasmatischen Ablagerung des Tracers in den Epithelzellen und den Stromazellen spricht vom morphologischen Aspekt her für einen lysosomalen Abbau. Meerrettichperoxidase wurde von der Perilymphe in das Lumen des Vas prominens transportiert. Dieser Transport erfolgte vesikulär durch das Gefäßendothel. Ein Transport des Tracers in umgekehrter Richtung wurde nach intravenöser Applikation nicht beobachtet.

    Description / Table of Contents: Zusammenfassung Nach einmaliger intravenöser Ethacrynsäure-Applikation (40 mg/kg Körpergewicht) wurden die ultastrukturellen Veränderungen in der Prominentia spiralis der Meerschweinchenschnecke nach Einwirkungszeiten zwischen 5 und 90 min untersucht. Nach einer initialen Schwellung der den Endolymphraum begrenzenden Epithelzellen entwickelte sich ein interzelluläres Ödem und eine ausgeprägte Schrumpfung der das Vas prominens umgebenden Stromazellen mit nachfolgender Erweiterung des Perivascularraumes. Während die Veränderungen in der Stria vascularis und der Prominentia spiralis im zeitlichen Verlauf sich weitgehend identisch entwickelten, setzte die Rückbildung zur normalen Ultrastruktur in der Prominentia spiralis früher ein.
    Notes: Summary Ultrastructural changes in the guinea pig spiral prominence were studied at various times after a single intravenous injection of ethacrynic acid (40 mg/kg body-weight). Initial swelling of endolymph-facing epithelial cells was followed by dilatation of the intercellular spaces and marked shrinkage of the stroma cells surrounding the spiral prominence vessel. While the changes in the stria vascularis and the spiral prominence progressed at about the same pace, the regression to normal ultrastructure set in earlier in the spiral prominence.

    Notes: Summary The cytochemical localization of adenylcyclase in the lateral cochlear wall was studied using a modification of the Reik-Howell method. All the cell membranes of the stria vascularis and spiral prominence cells, except the vascular endothelium, showed a low enzyme activity. In the marginal cell and spiral prominence epithelium facing the endolymph, this activity was limited to the perilymphatic membrane sections. The results are discussed.
