    Notes: Abstract Background: This study was designed to compare conventional laparoscopy with three-dimensional (3-D) laparoscopy. Method: Thirty candidates, 20 inexperienced and 10 experienced in operative laparoscopy, executed standardized exercises on a pelvitrainer. The candidates were randomized to two groups. Group A executed the exercises first with the conventional and then with the three-dimensional system. Group B accomplished the exercises in the reverse sequence. At the end of the exercises, the candidates answered specific questions about the two systems. Results: A total of 21 h 6 min 6 sec of laparoscopic exercises were analyzed—10 h 8 min 1 sec with the conventional and 10 h 58 min 5 sec with the three-dimensional system (p= 0.38). Group A required 12 h 26 min 56 sec to perform all the exercises. There was no statistically significant difference from group B, where the candidates needed 8 h 39 min 10 sec (p= 0.14). Neither were there any differences in the number of failed attempts between the two groups. There were also no statistical difference when the results obtained from the candidates without experience in laparoscopy and the participants experienced in operative laparoscopy were analyzed separately. Both the inexperienced and the experienced candidates became tired earlier, had more headaches, and needed extra time to adapt to the 3-D system. Conclusion: When analyzed in a standardized fashion, 3-D laparoscopy does not have any significant advantages over conventional laparoscopy.

    Notes: Abstract Background: Unsuspected malignancy remains a problem for the laparoscopic surgeon. The aim of this study was to evaluate the risk of ovarian micrometastasis in patients with breast cancer who undergo laparoscopic oophorectomy. Methods: We analyzed 25 premenopausal women with breast cancer who underwent therapeutic laparoscopic oophorectomy. The patients were subdivided into the following two groups according to ovarian pathology: group A with and group B without breast carcinoma micrometastasis. We then reviewed the follow-up data for both groups, with special attention to metastasis of the abdominal wall. Results: Twelve of 44 ovaries removed by laparoscopy showed ovarian breast carcinoma micrometastasis. There were no predictive factors of micrometastasis. After a mean follow-up of 38.1 months (95% CI: 29.2–46.9 months), none of the patients with proven micrometastasis developed metastasis of the abdominal wall, and the 21 puncture sites were inconspicuous. Conclusions: Although 32% of patients may have unexpected ovarian micrometastasis, laparoscopic oophorectomy in patients with breast cancer remains a safe procedure.

    Notes: Zusammenfassung Die Sektiofrequenz ist in den letzten Jahren in den meisten Industrieländern angestiegen und beträgt heute je nach Klinik zwischen 15 und 30 %. Aus kindlichen wie auch aus mütterlichen Gründen hat die Indikation zur Schnittentbindung eine Erweiterung erfahren. Obwohl Mortalität und Morbidität in den letzten Jahren abgenommen haben, ist die Sektio nach wie vor mit erheblichen intra- und postoperativen Komplikationen behaftet. In einer Analyse von 795 Sektioentbindungen fanden wir in 7,05 % schwere, operationsassoziierte Komplikationen. Die wichtigsten intraoperativen Komplikationen betreffen die perioperative Blutung (mit intra- und postoperativ notwendigen Bluttransfusionen) und die akzidentelle Verletzung der Harnblase. Zu den häufigsten postoperativen Komplikationen gehören die Nachblutungen mit der Notwendigkeit von Reoperationen sowie die Wundheilungstörungen. In der postoperativen Phase führen vor allem Infektionen und thromboembolische Erkrankungen zu einer erhöhten Morbidität. Nicht unterschätzt werden dürfen die Langzeitauswirkungen der Sektioentbindung insbesondere auf nachfolgende Schwangerschaften mit einem erhöhten Risiko für Extrauteringravidität, Plazentationsstörungen und Uterusruptur. Zudem kann durch die Sektio auch die Mutter-Kind-Beziehung empfindlich gestört werden.

    Notes: Abstract The purpose of this prospective study was to determine whether laparoscopic adhesiolysis ameliorates chronic abdominal pain in patients with abdominal adhesions. Forty-five patients with chronic abdominal pain lasting for more than 6 months but with no abnormal findings other than adhesions found at laparoscopy underwent laparoscopic adhesiolysis. Thirty-six patients (80%) were available for follow-up after a median time interval of 10 months (range: 6–36 months). Seventeen patients (47.2%) were free from abdominal pain and 13 patients (36.1%) reported significant amelioration of their pain. Six (16.6%) patients had no amelioration. Twenty-nine patients (80.6%) judged the outcome of the operation to be good or beneficial and 35 (97.2%) said that they would undergo the operation a second time if that were necessary. Laparoscopy is an effective tool for the evaluation of patients with chronic abdominal pain, and laparoscopic adhesiolysis cures or ameliorates chronic abdominal pain in more than 80% of patients.

Minimaler Zeitaufwand und Probenverbrauch sind die Vorteile dieser Verfahren, da keine Derivatisierung erforderlich ist. Bei der Pyrolyse/Massenspektrometrie bildet der am C2 gebundene Substituent ein größeres thermisches Spaltprodukt, das den Typus des Anthocyans kennzeichnet. Glykoside und Aglykone sind ebenfalls leicht zu unterscheiden.
    Notes: Summary The use of pyrolysis mass spectrometry and pyrolysis-GC/mass spectrometry is described for the identification of anthocyanins. Minimal time and sample consumption are the advantages of the method, as no derivatization is requested. In pyrolysis mass spectrometry the substituent at carbon two gives rise to a major thermal fragment that is representative for the type of anthocyanin. Glycosides and aglycones are also easily differentiated.

    Description / Table of Contents: Summary A sensitive and specific method for the determination of low levels of tributyltin (TBT) in water and sediments is presented. The procedure is based on the conversion of tributyltin compounds to tributylmethyltin (TBMeT) and analysis using GC with flame photometric detection (GC-FPD) and GC-MS. Detection limits of less than 1 ppt for water and 0.5 ppb for sediment are achieved. River and lake water and lake sediments analysed show tributyltin to be present in the low ppt- and ppb-range, respectively.
    Notes: Zusammenfassung Eine empfindliche und spezifische Methode für den Spurennachweis von Tributylzinnverbindungen (TBT) in Wasser und Sedimenten wird beschrieben. Dabei wird Tributylzinn zu Tributylmethylzinn methyliert und mit Hilfe von Gas-Chromatographie mit flammenphotometrischer Detektion (GC-FPD) sowie GC-MS analysiert. Nachweisgrenzen von weniger als 1 ppt für Wasser und 0,5 ppb für Sediment werden erzielt. TBT wurde in Fluß- und Seewasser sowie Seesedimenten im unteren ppt- bzw. ppb-Bereich nachgewiesen.
