It is tailored to power-law graphs and scales to trillions of edges.     Notes: Background: Nerve-sparing techniques are commonly used in retroperitoneal lymph node dissection (RPLND) in patients with early stage testicular germ cell tumors to preserve postoperative ejaculation. The indications for nerve-sparing procedures have been extended to patients who have residual retroperitoneal tumor postchemotherapy with an increase in the incidence of local recurrence. Here, we report on 26 Japanese men with advanced testicular cancer who underwent nerve-sparing RPLND after partially successful chemotherapy.Methods: Between January 1995 and December 2000, 26 patients with metastatic or recurrent testicular cancer underwent nerve-sparing RPLND after chemotherapy. Eight patients had seminoma and 18 had non-seminoma. Three patients received high-dose chemotherapy with carboplatin (250 mg/m2 per day × 5 days), etoposide (300 mg/m2 per day × 5 days) and ifosfamide (1.5 g/m2 per day × 5 days) in combination with peripheral blood stem cell transplantation.Results: In all cases, lumbar splanchnic nerves were preserved macroscopically during the operation, at least unilaterally. Twenty-two patients (84.6%) achieved antegrade ejaculation during a mean follow-up at 3.9 months (range: 1–7 months). Three patients have fathered children. Only one patient suffered a retroperitoneal recurrence during a median follow-up at 25.8 months (range: 6–76 months).Conclusion: Nerve-sparing procedures for RPLND are appropriate for patients with metastatic testicular cancer, even after chemotherapy. The procedure preserves ejaculatory function in the majority of the patients without increasing the risk of local recurrence. Nerve-sparing RPLND improves the quality of life in patients who require postchemotherapy RPLND to treat residual tumor.

    Notes: The present study was designed to ascertain retrospectively the validity of ursodeoxycholic acid (UDCA) in the treatment of prostate cancer in terms of prophylactic effects on the occurrence of flutamide-induced hepatopathy in a large number of patients surveyed in a multi-center cooperative study.〈section xml:id="abs1-3"〉〈title type="main"〉Methods:One hundred and eighty-one patients (74.1 ± 4.9 years) with prostate cancer treated with flutamide with (n = 70) or without (n = 111) UDCA were retrospectively evaluated and the occurrence of hepatopathy was compared between these two patient groups.〈section xml:id="abs1-4"〉〈title type="main"〉Results:Between patients treated with UDCA and those without it, no significant differences were noted in age, clinical stage, grade, duration of flutamide administration and serum prostate-specific antigen (PSA) levels before treatment. However, there were significant differences in the presence or absence of previous treatments and treatments used together with flutamide. The incidence of hepatopathy was 11.4% (8/70) in patients with UDCA and 32.4% (36/111) in those without it, showing a statistically significant difference (P 〈 0.05). The hepatopathy-free rate obtained by the Kaplan–Meier method was also significantly higher in patients with UDCA (88.4% 1 year following flutamide administration) than that in those without it (59.6%) (P 〈 0.005).〈section xml:id="abs1-5"〉〈title type="main"〉Conclusion:These results suggest that UDCA has a prophylactic effect against flutamide-induced hepatopathy in patients with prostate cancer.

    Notes: Summary The clinical results obtained in 81 cases of bladder tumor are used as a basis for discussion of factors influencing the effect of bladder instillation therapy. The following conclusions are derived: 1) If bladder instillation therapy aims only at complete remission (CR), tumors over 1 cm in diameter should be excluded, because this is the theoretical size limit beyond which instillation therapy cannot lead to CR. 2) At least 40 ml of drugs may be needed for instillation therapy for tumors located in area B, the limited part posterior to the trigonum.

    Notes: Summary Our AFC regimen was originally developed to treat superficial bladder tumors. It consists of a daily intravesical administration of 20 mg Adriamycin (ADM) and 200 mg Cytosine arabinoside (CA) dissolved in 20 ml of sterilized distilled water, simultaneously with a daily rectal suppository of 750 mg tegafur (Futraful) for a total of 20 applications. Our study demonstrated the following characteristics of the AFC modality: (1) It is simple; (2) it is completed in 3 weeks, which is shorter than conventional bladder instillation therapy; (3) the effective rate is reasonable (70%); (4) no local side-effects were observed because the concentrations of ADM and CA were low; (5) interruption of therapy was unnecessary, because whatever general side-effects were caused, they disappeared within several days of completion of treatment.

    Notes: Abstract We purified an extracellular thermostable β-galactosidase of Saccharopolyspora rectivirgula strain V2-2, a thermophilic actinomycete, to homogeneity and characterized it to be a monomeric enzyme with a relative molecular mass of 145 000 and s°20,w of 7.1 s. In addition to the hydrolytic activity of 1-O-substituted β-d-galactopyranosides such as lactose [a Michaelis constant K m=0.75 mm and molecular activity (k cat)= 63.1 s−1 at pH 7.2 and 55° C] and p-nitrophenyl β-d-galactopyranoside (K m=0.04 mm k cat= 55.8 s−1), the enzyme had a high transgalactosylation activity. The enzyme reacted with 1.75 m lactose at 70°C and pH 7.0 for 22 h to yield oligosaccharides in a maximum yield (other than lactose) of 41% (w/w). A general structure for the major transgalactosylic products could be expressed as (Gal)c-Glc, where n is 1, 2, 3, and 4 with a glucose at a reducing terminal. These oligosaccharides could selectively promote the growth of the genus Bifidobacterium found in human intestines. S. rectivirgula β-galactosidase was stable at pH 7.2 up to 60°C (for 4 h in the presence of 10 μm MnCl2) or 70°C (for 22 h in the presence of 1.75 m lactose and 10 μm MnCl2). Thus the enzyme is applicable to an immobilized enzyme system at high temperatures (60°C 〈) for efficient production of the oligosaccharides from lactose.

    Notes: Both vinyl methacrylate (VMA) and vinyl acrylate (VA) were homopolymerized by n-butyllithium catalyst at -78°C. Both of the anionic polymers were soluble in common organic solvents, and only vinyl group was contained in the polymers. Both of the monomers were homopolymerized by free-radical catalysts under different conditions. The soluble polymers were obtained under low monomer concentrations and at low conversions. It was estimated by NMR and infrared spectroscopy that both the soluble polymers contained mainly a vinyl group, similar to the anionic polymers. The soluble VMA polymers comprised 10-20% cyclic units for monomer concentrations ranging from 1.8 to 0.5 mole/I. The soluble VA polymers comprised 50-60% cyclic units for monomer concentrations ranging from 0.9 to 0.3 mole/l. It was suggested that the cyclic units did not consist of γ-lactone but of larger-membered rings than δ-lactone or of ladder structural units. The difference between the cyclization content of poly-VMA and that of poly-VA might be explained by the copolymerization data of the reference monomers.
