    Notes: Background: Standard severity criteria for benign prostatic hyperplasia (BPH) are needed to facilitate clinical and epidemiologic investigations in the common terminology.Patients and Methods: A total of 692 BPH patients from 8 institutions were judged by specialized physicians for their overall disease severity and severity in 4 domains: symptom, function, anatomy, and quality of life (QOL). The items measuring severity in these 4 domains were the International Prostate Symptom Score (I-PSS) questionnaire, uroflowmetry, transrectal ultrasonography (TRUS), and questionnaires on bother and I-PSS QOL index, respectively. By Spearman's rank correlation and multiple regression analysis using physicians' judgement as the criterion variable (gold standard) the items and their cut-off levels were selected so that high predictivity, practicality and clinical significance were maintained, while the number of items in use was minimized.Results: The item(s) and their ranges of values for each severity grade were summarized as follows; the evaluation domain (the selected item(s): range of mild, moderate, severe): symptom (I-PSS: 0–7,8–19, 20–35), function (maximum flow rate and residual urine volume: ≥ 1 5 mL/s and 〈 50 mL, ≥ 5 mL/s and 〈 1 00 mL, 〈 5 mL/s or ≥ 100 mL), anatomy (prostate volume on TRUS: 〈 20 mL, 〈 50 mL, ≥ 50 mL), QOL (QOL index: 0 and 1, 2 to 4, 5 and 6). The overall severity was efficiently predicted when defined according to the combinations of severity grades of the 4 domains. These criteria predicted physicians' judgement with accuracy of about 80% for overall severity and severity of individual domains.Conclusion: The proposed criteria are valid, practical, and would be useful for the common scale of the BPH severity in clinical and epidemiologic studies.

    Notes: Background: Various treatment modalities for benign prostatic hyperplasia (BPH) have emerged and are now in use or await evaluation of clinical usefulness. It is difficult, however, to compare their efficacies on a single scale, because standardized criteria for therapeutic efficacy of BPH treatments have not been established.Patients and Methods: A total of 692 BPH patients from 8 institutions in Japan received various treatments, and were judged by specialized physicians for overall efficacy, and for efficacy in 4 domains: symptom, function, anatomy, and quality of life (QOL). Efficacy of treatment was graded as excellent, good, fair or poor, and assessed using items based on conventional clinical measurements. These items included 1) the difference (post-pretreatment value), 2) relative ratio (post/pre) and, 3) the individual values of pre or posttreatment measurements. The cut off levels for each grade were heuristically selected by Spearman's rank correlation and multiple regression analysis so that the results accurately predicted physicians' judgement, while the feasibility was maintained.Results: The results for each efficacy grade (range of excellent, range of good, range of, fair, range of poor) were summarized as follows:Symptom: (post/pre treatment ratio of I-PSS) ≤ 0.25, ≤ 0.5, ≤ 0.75, 〉 0.75.Function: (post-pre of Qmax) ≥ 10 mL/s, ≥ 5 mL/s, ≥ 0.25 mL/s, 〈 0.25 mL/s.Anatomy: (post/pre ratio of prostate volume) ≤ 0.5, ≤ 0.75, ≤ 0.9, 〉 0.9.QOL: (pre-post of QOL index) ≥ 4, 3, 2 and 1, ≤ 0.The overall efficacy grade was defined as the median of efficacy grades of 3 domains: symptom, function and QOL. The agreement rates between the criteria and physicians' judgement on the dichotomous efficacy (either excellent plus good, or fair plus poor) were approximately 80% in individual domains and overall estimate, and consistent among various treatments.Conclusion: The proposed criteria are fairly accurate, simple, and practical, and thus may be useful as a standard method for assessing the clinical efficacy of BPH treatments.

    Notes: Summary The clinical results obtained in 81 cases of bladder tumor are used as a basis for discussion of factors influencing the effect of bladder instillation therapy. The following conclusions are derived: 1) If bladder instillation therapy aims only at complete remission (CR), tumors over 1 cm in diameter should be excluded, because this is the theoretical size limit beyond which instillation therapy cannot lead to CR. 2) At least 40 ml of drugs may be needed for instillation therapy for tumors located in area B, the limited part posterior to the trigonum.

    Notes: Summary In the present series of trials, Adriamycin (ADM) intravesical instillation therapy was found to be effective against tumors of papillary morphology measuring 〈10 mm in diameter that were of low pathological stage and low histological grade. The rate of complete disappearance increased in proportion to the concentration of ADM and the duration of retention of the drug, although the efficacy rates were almost the same in each trial. On the other hand, side effects on the bladder were reduced when the instilled solution was lower in concentration and the retention time was short.

    Notes: Abstract Expression of managanese superoxide dismutase (Mn-SOD) mRNA during the pregnant mare serum gonadotrophin (PMSG)/human chorionic gonadotrophin (HCG)-induced ovulatory process, and during the estrous cycle was examined in rat female reproductive organs. Mn-SOD mRNA levels in theca interna cells markedly increased in PMSG-primed rats and high levels of the transcripts were maintained after HCG injection. The PMSG-enhanced expression of Mn-SOD mRNA in follicular epithelial cells increased concomitantly with luteinization of these cells. The levels of Mn-SOD mRNA remained high and became equivalent in both granulosa and theca lutein cells 24 h after HCG injection. Neither luteinization nor the expression of Mn-SOD mRNA was observed in the epithelial cells of unovulated follicles. Luteal bodies had formed 3 days after HCG injection, and the same level of Mn-SOD mRNA expression continued in lutein cells, but not in stromal cells. During the estrous cycle, Mn-SOD mRNA was localized to theca interna cells on proestrus, to the epithelial cells of luteinizing follicles on estrus, and to newly formed luteal bodies on diestrus. The epithelial cells in the oviduct did not express Mn-SOD mRNA throughout the ovulatory process or the estrous cycle. Expression of Mn-SOD mRNA in the luminal epithelial cells of the uterus increased after PMSG injection, reaching a maximum after 24 h, and became relatively negative 3 days after HCG injection when corpora lutea had formed in the ovary. During the estrous cycle, uterine epithelial cells and leukocytes showed marked increases in Mn-SOD mRNA expression on estrus and on proestrus, respectively. Expression in the vaginal epithelium became apparent 3 days after HCG injection and continued for at least 12 days after HCG injection. The expression was localized to the superficial layer of the epithelium. During the estrous cycle, expression occurs in the basal layer on proestrus and estrus, transferring to the superficial layer on diestrus day 1, and expression stops on diestrus day 2. The relationship between the expression of Mn-SOD mRNA and hormone-induced metabolic changes, including steroidogenesis, is discussed.

    Notes: Abstract. Early morning urine osmolality in 1,453 nonenuretic children and 144 enuretic children was measured in a population study. Early morning urine osmolality was lower in nonenuretic children aged 3 – 4 years than in those aged over 5 years. A similar pattern was observed in enuretic children. No significant difference in early morning urine osmolality was observed between nonenuretic children and enuretic children at any age. These data suggest that nocturnal urine concentration may be immature in nonenuretic children under 4 years. We did not find any evidence for increased nocturnal urine production as a cause of enuresis.

    Notes: Background: There have been few detailed transmission electron microscopic studies of the cytoskeleton of osteoclasts to date because of the high electron density of their cytoplasmic matrix, and there is a controversy concerning the presence of stress fibers in these cells.Methods: The cytoskeleton and nuclear matrix of osteoclasts in the rat cranium were studied by transmission electron microscopy after perfusing the organs with a mixture of Triton X-100 and a low concentration of glutaraldehyde to reduce the electron density of the intracellular matrix and by immunoelectron microscopy using anti-actin and anti-vimentin antibodies.Results: In osteoclasts, abundant microfilaments, 5 - 6 nm in diameter, extended across the cell organelles. These filaments were also observed just under the cell membrane facing the bone, but thick bundles of microfilaments were not noted. In the cytoplasm, there were fibers 10-12 nm in diameter, which were considered to be intermediate filaments distributed in the same pattern as microfilaments. Filamentous structures consisting both of the microfilaments and intermediate filaments connected the nuclei. Intermediate filaments linked nuclei at nuclear pores (near the euchromatin), and microfilaments connected nuclei at areas of the nuclear membrane containing nucleolus-associated heterochromatin. In the nuclear matrix after detergent treatment, thin filaments and intermediate -type filaments were found connecting the heterochromatins.Conclusions: Euchromatin is considered to be an active site of DNA and RNA synthesis. Our structural evidence has lead to the proposal that intermediate filaments in the cytoplasm may act as tracks for transmission of information between nuclei and guides for the transit of molecules through the pores. © 1995 Wiley-Liss, Inc.

    Notes: Summary Ultrastructural changes in hippocampal granule cells, mossy fibers and mossy fiber boutons were examined following the administration of picrotoxin in adult rats. Generalized seizures occurred within 5–10 min after the intraperitoneal injection of picrotoxin. The electron-microscopic examination of hippocampal tissues from rats that had been perfused with fixative during the seizure revealed that the large dense-core vesicles increased in number and accumulated on the presynaptic membranes of mossy fiber boutons; some of these vesicles appeared to be fused with the membranes, and omega-shaped exocytotic profiles were frequently seen. Furthermore, greatly increased numbers of coated vesicles (60–90 nm in diameter) were observed on the maturing faces of Golgi fields of granule cells. Thus, our study not only indicates an increased incidence of exocytosis of large dense-core vesicles during picrotoxin-induced seizures, but also suggests that these vesicles are replaced in excess from the perikaryon of the granule cell.
