Investigations of the tensile properties of the anterior and posterior cruciate ligament demonstrated that pre-mortal immobilisation played a considerably more significant role than the donor’s age. The tensile properties of the patellar tendon as affected by age and pre-mortal immobilisation have not yet been measured. Since the central third of the patellar tendon is the most common substitute in anterior and posterior cruciate ligament reconstruction, these investigations are necessary to define the tensile properties. We performed tensile stress experiments on 26 human cadaver knee joints using a computer-controlled material testing machine. The age of the specimen and the duration of pre-mortal immobilsation before harvesting (as soon as possible after death) was noted. The cause of death was also recorded. None of the tested knee joints had a ligamentous instability, severe degenerative changes or fresh osseous lesions. Amputated knee joints of patients with vascular or tumour diseases were excluded from the study. Tensile testing was performed in a special frame at an angle between tibia and tibia of 30∞. The rupture speed was 500 mm/s. The length, width and depth of the patellar tendon was measured, and the central third prepared and remeasured. Maximum rupture force, rupture force at brake and elongation were tested. Tensile stress, tensile stress at break, elongation and modulus were calculated. The mean age of the donors was 60 years. The mean area of the central third of the patellar tendon was 33.8 mm2, and the mean length 52.0 mm. The pre-mortal duration of immobilisation ranged from 0 to 9 weeks (mean, 22.85 days). We found a linear correlation. The average tensile stress was 44.0 N/mm2 (range, 9.2–151.6 N/mm2), the tensile stress at break 41.52 N/mm2 (range, 9.2–151.6 N/mm2) and the modulus 278.8 N/mm (range, 96.6–1080 N/mm). The pre-mortal immobilisation period was found to be significantly more important than the donor’s age.

Durch Zugfestigkeitsuntersuchungen am vorderen und hinteren Kreuzband unter Berücksichtigung von Alter und Immobilisationsdauer wurde ein deutlich höherer Einfluß der prämortalen Immobilisationsdauer auf die biomechanischen Eigenschaften nachgewiesen. Entsprechende Untersuchungen für das mittlere Patellarsehnendrittel liegen nicht vor. Dieses Sehnentransplantat gilt als meist implantiertes der Standardtransplantate und wird sehr häufig für den Einsatz als allogenes Transplantat gewonnen. Daher wurden entsprechende Zugfestigkeitsuntersuchungen durchgeführt. Bei 26 menschlichen Kniegelenken wurden die Reißfestigkeit, die Reißkraft bei Riß, die Zugspannung, die Zugfestigkeit bei Riß, die Dehnung und das Elastizitätsmodul bestimmt. Es kamen nur Präparate ohne Vorschäden zum Einsatz. Die prämortale Immobilisationsdauer, die Todesursache sowie Patientendaten wie Alter und Geschlecht wurden anonymisiert übermittelt. Bis zum Zeitpunkt der Versuchsdurchführung wurden die Präparate tiefgefroren gelagert und vor der Versuchsdurchführung auf Raumtemperatur aufgetaut. Die Messungen wurden in einer Zugprüfmaschine mittels einer speziellen Aufhängevorrichtung bei einer Geschwindigkeit von 500 mm/min vorgenommen. Das Durchschnittsalter der Spender belief sich auf 60 Jahre. Die durchschnittliche Aufbewahrungsdauer war 6,8 und die Immobilisationsdauer 22,85 Tage. Die Querschnittsfläche des mittleren Patellarsehnendrittels betrug im Durchschnitt 33,8 mm 2 und die Länge 52,0 mm. Die maximale Zugfestigkeit betrug 44,0 N/mm 2 (9,2–151,6), die Zugfestigkeit bei Riß 41,52 N/mm 2 (9,2–151,6) und das Elastizitätsmodul 278,8 N/mm (96,6–1080). Die Messungen zeigten einen signifikant höheren Einfluß auf die Transplantateigenschaften der prämortalen Immobilisation als das Alter. Durch diese Zugfestigkeitsuntersuchungen werden die guten biomechanischen Eigenschaften des mittleren Patellarsehnendrittels für den Kreuzbandersatz belegt. Allogene Transplantate sollten nur von Spendern gewonnen werden, bei denen die prämortale Immobilisationsdauer bekannt ist. Ähnliche Zugversuche sind für alle Biomaterialien zu fordern, die als Ersatztransplantate in der rekonstruktiven Chirurgie zum Einsatz kommen.
