    Description / Table of Contents: Zusammenfassung. Jeder rekonstruktive Eingriff an der Bauchdecke hat die Wiederherstellung der funktionellen Stabilität und adäquate Weichteilbedeckung zum Ziel. Es werden Anatomie und Hebung des gestielten myofascialen M.-tensor-fasciae-latae-Lappens und des freien mikrovasculär angeschlossenen musculocutanen M.-latissimus-dorsi-Lappens beschrieben und klinisch vorgestellt. Die gestielte M.-tensor-fasciae-latae-Einheit gewährleistet bei ausgedehntesten Fasciendefekten bei Mehrfachrezidivhernien ohne anatomisch faßbare Bruchpforte eine starke und zugleich dynamische funktionelle Wiederherstellung der Fascienkontinuität zur Prävention eines neuerlichen Bruchs. Der innervierte mikrochirurgisch anastomosierte M.-latissimus-dorsi-Lappen stellt die optimale funktionelle und ästhetische Rekonstruktion ausgedehnter Bauchwanddefekte, die alle anatomischen Schichten betreffen, dar.
    Notes: Summary. The aim of any abdominal wall reconstruction is maximal functional stability and adequate soft tissue coverage. The anatomy, elevation and clinical application of the myofascial tensor fasciae latae transposition flap and of the microvascular musculocutaneous latissimus dorsi free flap are presented. Repairing extensive fascial defects and recurrent hernias with the tensor fasciae latae transposition flap provides strong, dynamic, and functional reconstruction of fascial continuity to prevent a further recurrence. Adequate functional and aesthetic repair of a full-thickness abdominal wall defect can be optimally managed by the innervated microsurgical latissimus dorsi free flap.

    Description / Table of Contents: Zusammenfassung. Zwischen 1980 und 1995 wurden an unserer Abteilung 207 Patienten mit 219 ausgedehnten Decubitalulcera im Beckenbereich operativ behandelt. Davon wurden 80 Patienten, welche im Zeitraum von 1992–1995 operiert wurden, nachuntersucht. Die postoperative Komplikationsrate lag bei 22 %, die Rezidivrate bei 27 %. Zur Qualitätssteigerung wurde das prä- und postoperative Management standardisiert. In dieser Arbeit wird der besondere Stellenwert dieser präoperativen Vorbereitung, die Kriterien der Lappenwahl, das chirurgische Vorgehen und die postoperative Nachbetreuung diskutiert.
    Notes: Summary. Between 1980 and 1995, 207 patients with 219 extensive pelvic pressure sores underwent reconstructive surgery at our department. Eighty patients operated upon between 1992 and 1995 were followed up. The postoperative complication rate was 22 % and the relapse rate 27 %. Pre- and postoperative management has been standardized for the purposes of quality control. The value of preoperative management, choice of flap, operative procedure and postoperative care are discussed in this paper.

    Description / Table of Contents: Zusammenfassung. Die notfallmäßige Versorgung schwerer Extremitätenverletzungen mittels freier Lappenplastik stellt ein modernes, aber nicht oft praktiziertes Behandlungskonzept dar. Die positiven Erfahrungen mit diesem Konzept und die Vorteile gegenüber dem konventionellen Vorgehen werden erläutert. Das initiale radikale Débridement ist der entscheidende Teil der Operation, gefolgt von der Osteosynthese. Der freie Gewebetransfer wird entsprechend den Erfordernissen des Extremitätendefekts ausgewählt und mikrochirurgisch angeschlossen. Durch dieses moderne Behandlungskonzept des freien Gewebetransfers im Akutstadium konnte eine frühe Rehabilitation der Extremitätenfunktion erzielt werden. Der Nachbeobachtungszeitraum reicht von 3 Monaten bis zu 8 Jahren. Es wurde weder ein Lappenverlust beobachtet noch traten tiefe Wund- oder Knochenentzündungen auf. Durch das vorgestellte Behandlungskonzept konnte eine Reduktion der Lappenverlustrate, eine verringerte assoziierte Morbidität und funktionell verbesserte Wiederherstellung verglichen mit den Literaturangaben bei konventionellem Vorgehen erzielt werden.
    Notes: Summary. The concept of emergency free tissue transfer for severe extremity injuries represents a cutting-edge technology. We discuss our very positive results with this technique. The conceptual reasons for these favorable results, compared with conventional approaches, are also discussed. An initial, radical debridement is the most important part of the operation. This is then followed by osteosynthesis. The correct type of free tissue transfer is chosen according to the requirements of the soft tissue defect. Qualitatively better results with earlier definitive rehabilitation were achieved with free tissue transfer performed during the acute stage of limb wound treatment. The follow-up period ranged from 3 months to 8 years. We experienced neither flap loss, osteomyelitis, nor severe wound infection. Using the modern concept of emergency free tissue transfer, we reduced the rate of flap loss and associated morbidity while achieving a functionally improved reconstruction.

    Description / Table of Contents: Grundlagen: Die Versorgung von ausgedehnten oder komplexen Handverletzungen stellt besondere Ansprüche im Hinblick auf das funktionelle und ästhetische Ergebnis. Bei dieser Art der Verletzung ist die primäre Weichteildeckung von besonderer Bedeutung, da sich dadurch die Ausgangssituationen für eine Wiederherstellung der Funktion deutlich verbessern lassen. Methodik: Im Zeitraum von Oktober 1986 bis Mai 1996 wurden 43 Patienten mit 49 freien Lappenplastiken bei komplexen und kombinierten Handverletzungen primär behandelt. Der mittlere Nachbeobachtungszeitraum lag bei 39 (6–92) Monaten, der mittlere Krankenhausaufenthalt bei 19 (8–40) Tagen. In Abhängigkeit vom Zeitpunkt der Rekonstruktion mit einem freien Lappen sowie basierend auf der klinikeigenen Klassifikation erfolgte die Einteilung der Patienten in 3 Gruppen: Primärrekonstruktion (innerhalb von 24 h), primär aufgeschobene Rekonstruktion (zwischen 2 und 7 Tagen) und Spätrekonstruktion (nach 7 Tagen bis 3 Wochen posttraumatisch). Ergebnisse: In der Gruppe der Primärrekonstruktion ergaben sich die besten Ergebnisse, da durch ein radikales Débridement und sofortige Defektdeckung mit gesundem Gewebe (freier Lappen) eine physiologische Wundheilung erreichbar ist. Durch diese Art der Versorgung verkürzten sich Krankenhausaufenthalt und damit die Kosten, da Lappenverluste, Infektionen und multiple Mehreingriffe vermieden wurden. Ebenso wurden dadurch frühe Rehabilitation und damit Reintegration ins Arbeitsleben ermöglicht. Schlußfolgerungen: Aufgrund der erzielten Ergebnisse erscheint die frühzeitige Behandlung von komplexen und ausgedehnten Handverletzungen durch radikales Débridement und primäre Rekonstruktion mit freien Lappenplastiken zielführend und sollte bei der Erstbehandlung immer bedacht werden.
    Notes: Background: The treatment of large and complex hand injuries is particularly challenching concerning the functional and cosmetic outcome. In this kind of injuries the primary defect cover is of paramount importance and so the initial situation for secondary reconstructive options may be evidently improved. Material & methods: Between October 1986 and Mai 1996 43 patients with large complex and combined hand injuries were primarily treated with 49 free flaps. The mean follow-up was 39 (6 to 92) months, the mean hospital stay was 19 (8 to 40) days. Depending on the time of the reconstruction and based on a classification established in our clinic, the patients were distributed among three groups: primarily (within 24 hours), delayed (between 2 and 7 days), and late (after 7 days till 3 weeks posttraumatically) reconstruction. Results: Within the group of primary reconstruction the best results could be achieved, since radical debridement and primary defect cover by healthy tissue (free flap) led to physiological wound healing. That caused reduced hospital stay thus decreased costs, since flap failure, infection and multiple secondary surgical interventions could be diminished. Also earlier rehabilitation and reintegration into social life and profession was achievable. Conclusion: Due to the obtained results a primary treatment of large and complex hand injuries through radical debridement and reconstruction with free flaps seems to be effective and should already be considered in the emergency room.

    Description / Table of Contents: Zusammenfassung Bei 31 (von 48) Patienten mit 43 (von 63) replantierten oder revaskularisierten Fingern oder Extremitätenanteilen wurde eine Studie der Langzeitergebnisse durchgeführt, wobei das Alter der Patienten zum Unfallzeitpunkt unter 16 Jahren lag. Es wurden die Ergebnisse in Hinblick auf Funktion, Sensibilität, Kälteintoleranz, Wachstumsstörung, Akzeptanz durch den Patienten sowie auf berufliche Konsequenzen erhoben. Die Untersuchungen ergaben exzellente Rückkehr der Sensibilität sowie gute funktionelle Ergebnisse mit einer nur geringen Rate an Kälteintoleranz. Mit der Erfolgsrate von 94,7 % Einheilung erlauben diese Ergebnisse den Schluß, daß im Kindes- und Jugendalter Replantationen oder Revaskularisationen in jedem Fall und auch bei Mehrfachamputationen indiziert sind und versucht werden sollen.
    Notes: Summary In 31 (out of 48) patients with 43 (out of 63) replanted or revascularized digits or parts of the extremities a study of long-term results was performed. Their ages were below 16 years at the time of injury. The results concerning function, sensitivity, cold intolerance, growth disturbance, patient acceptance and occupational changes were evaluated. Early complications consisted of venous stasis in 10.6 % (n = 4), skin necrosis of the wound margin in 5.3 % (n = 2) and in finger-tip necrosis in 5.3 % (n = 2). One lower arm and one finger replanted (5.3 %, n = 2) were lost after several revisions due to venous problems. In no case did arterial problems occur. Tendon adhesions were the most common complication in 28.9 % (n = 11), followed by deviation of the axis in a frontal plane of more than 20 ° in 13.3 % (n = 5) and by joint instability in 5.3 % (n = 2). Only twice (5.3 %) did ankylosis or in another case osteomyelitis of the proximal phalanx result. At amputation level II (between the DIP joint and the nail root), 37.5 % (n = 3) developed a reduction of range of active movement (RAM) with a mean of 31.7 °, and once an arthrodesis was necessary, whereas 50 % (n = 4) of the patients have full range of active motion. At amputation level III (between the MP and DIP joint) 52.7 % (n = 10) developed a reduction of RAM at the PIP joint with a mean of 29.5 °, and twice an arthrodesis was necessary, whereas 26.4 % (n = 5) of the patients have full range of active motion. Of the patients 78.9 % (n = 15) developed a reduction of RAM at the DIP joint with a mean of 35.7 °, and twice an arthrodesis was necessary. Only twice was ankylosis present, but 10.5 % (n = 2) of the patients have full range of active motion. Excellent sensory recovery was observed with a mean dynamic two-point discrimination of 4.8 mm. In addition, only 29 % (n = 9) of the patients report a surprisingly low incidence of cold intolerance, which is in contrast to adults with up to 100 % cold intolerance in replanted digits. In 67.7 % a radiologically measured difference of length with a mean of 3.8 mm and clinically measured a difference in the length of the digits with a mean of 4.9 mm was observed without complaints by the patients. Instead, there was moderate discontent with the aesthetically disturbing finger-tip atrophy, which was up to 30 % in 48.4 % of the patients. Of the patients who are now adult, 16.7 % (n = 4) changed their choice of profession due to the trauma or because of the dissatisfactory result. These results and the success rate of 94.7 % suggest the conclusion that one should not hesitate to perform a replantation or revascularization in children with even multiple amputations as the results are excellent.

    Notes: Abstract A total of 28 human specimens (14 male, 14 female) was used to perform macro- and microscopic studies on the morphologic basis of the urethral continence mechanism. Furthermore, functional studies were performed in six sheep, with the aim of looking at the pudendal and autonomic innervation of the urethra and the rhabdosphincter, as well as the changes of autonomic innervation after selective denervation. Transurethral ultrasound was performed in 34 continent patients, in order to visualize the contractions of the rhabdosphincter. The membranous urethra is innervated by branches of the autonomic pelvic plexus. The rhabdosphincter is an omega-shaped loop of striated muscle fibers that is innervated by the pudendal nerves. These results are supported by the results of animal experiments that show that the autonomic nerves predominantly innervate and regulate the upper part of the urethra, whereas stimulation of the pudendal nerves leads to a contraction of the lower part of the sheep urethra. In electron-microscopy, marked degeneration of the smooth muscle cells could be seen in the sheep with bilateral denervation.

    Notes: Abstract. Objective: A case with posttraumatic tissue loss of the auricle is presented, where successful reconstruction with the aid of a tubed flap is achieved by gradual lysis within three stages. Background: Helical rim losses may be reconstructed with a tubed flap created from postauricular tissue. Method: The vascularity of the tubed flap was increased with the intermittent application of a rubber band tourniquet. Conclusion: The use of a tourniquet before completing the third stage of the helical rim reconstruction may enhance the reliability of this procedure.

    Notes: Abstract The nature of UV-induced pre-recombinational structures was studied using transformation of Saccharomyces cerevisiae cells with non-replicative plasmids. Transformation by double-stranded plasmids irradiated with UV was stimulated up to 50-fold, and both plasmid integration and conversion of the mutated chromosomal selective gene were found to be equally increased. The stimulation observed with such ‘totally’ irradiated plasmids was not found with plasmids bearing lesions in only one strand. This effect is attributed to the formation by excision repair of recombinogenic structures consisting of a pyrimidine dimer opposite a gap. When single-stranded integrative plasmids were irradiated, their transforming potential was decreased but the proportion of transformants that arose by gene conversion, rather than by plasmid integration, was increased from 8% to 49% as a function of the UV dose. Possible reasons why single-strand UV lesions favour gene conversion are discussed.
