    Notes: Abstract Aneurysms of the pancreaticoduodenal artery are rare and may be associated with celiac artery stenosis or occlusion. Twenty-eight patients are reported in the literature. The diagnostic findings and therapeutic alternatives of four additional patients form the basis of this report. One patient with ruptured pancreaticoduodenal aneurysm was successfully treated by transcatheter embolization, and one patient was treated surgically; both patients had an uneventful recovery. In the remaining two patients, the aneurysms were left untreated. One patient died 1 year later of an unrelated cause, and the other patient is symptomfree after 2 years.

    Notes: Abstract. The aim of this study was to assess the interobserver variations in diagnosis of pulmonary embolism (PE) with cine technique and to compare the diagnostic accuracy of pulmonary arteriograms to final-outcome diagnosis. One hundred and seventy patients with clinical suspicion of acute PE were examined with ECG, laboratory tests, chest X-ray, pulmonary scintigraphy and selective pulmonary cine arteriography. The follow-up time was 6 months. Fifty-one arteriograms were interpreted as positive for PE. Two pulmonary emboli were missed when compared with the diagnosis as stated by the final-outcome committee. No arteriograms were considered as not of diagnostic quality. Mean interobserver agreement in lobar vessels was 100 %, in segmental vessels 93 % and in subsegmental vessels 63 %. The mean interobserver agreement was 89 %. Pulmonary cine arteriography produces high diagnostic accuracy and few inconclusive results in patients with suspected PE.

    Notes: Collagen-induced arthritis (CIA) is an experimental rat model sharing a number of features with human rheumatoid arthritis (RA). The model is associated with a proinflammatory (TH1) type of immune response and treatments with cytokines associated with TH2 immune responses are beneficial. Since agents with TH1-inducing properties, such as Freund's incomplete adjuvant (FIA), are necessary for disease induction, it is of interest to investigate whether an adjuvant with TH2-inducing properties affects CIA in a different way than does FIA. The authors studied arthritis development in DA rats after immunization with the TH2 stimulatory adjuvant alum adsorbed to rat collagen type II (CII) or collagen II fragments. Such treatments suppressed disease development both prophylactically and therapeutically. This beneficial effect of alum–CII immunization was associated with an increase in the IgG1 anti-CII antibody response as compared to untreated rats or rats pretreated with alum alone. Treatment with alum without the addition of collagen did not have any clinical effect. In addition, alum–CII treated rats had a significantly higher expression of IL-4 mRNA than untreated rats in the lymph nodes, 7 days after CIA induction. The authors suggest that alum–CII induces a TH2 immune response against rat CII which counteracts the development of CIA.

    Notes: DA rats develop transient arthritis after subcutaneous immunization with adjuvant-oil, while chronic arthritis and collagen autoreactivity ensues when collagen is added to the oil. We show here that DA rats can be protected from oil-induced arthritis (OIA) and rat collagen-induced arthritis (rCIA) by addition of antigen to these arthritogenic inocula.We have investigated this remarkable phenomenon and demonstrate that both foreign and self antigens can be protective, apparently provided they are immunogenic; hence HSP-65kDa, ovalbumin, rat myelin basic protein, rat IgG and bovine albumin are effective while rat albumin is not. This protection is long-lasting and disease-specific because rats protected from rCIA resist a later attempt to induce arthritis, but not experimental autoimmune encephalomyelitis (EAE). Protection from rCIA depends neither on the blocking of humoral autoreactivity to collagen nor on a change in the isotype profile of anti-collagen antibodies. We demonstrate that immunogens can also be protective when injected intraperitoneally only a few days before onset of arthritis. Our results indicate that protection is mediated through bystander immune reactions towards the co-immunized antigen and that the arthritogenicity of a given provocation, be it adjuvants, microbes or autoantigens, may be a complex net result of arthritogenic and contra-arthritogenic immune reactions.

    Notes: Summary Correct diagnosis of malignant lymphoma of the brain and differentiation from malignant glioma, metastases, meningeoma and infection is often difficult. With the aim of finding characteristics pointing to the correct diagnosis all CT examinations from 16 patients with primary or secondary lymphoma of the brain were analysed. In 3 of 10 patients with primary lymphoma and 4 of 6 with secondary lymphoma the tumors were multiple. No differences between the CT appearance of primary and secondary lymphoma were found except that secondary lymphomas were generally smaller and more often multiple. The lymphomas were most often well demarcated, had a density equal to or slightly higher than normal brain tissue, were surrounded by no or slight edema and showed a moderate to marked contrast enhancement. The tumors were situated in the basal ganglia, corpus callosum or cerebellum in high frequency and were always in contact with either the ependyma of the ventricles or the superficial subarachnoid space. A tumor with widespread infiltration of the surroundings of the ventricles seen in 6 patients in the material is highly characteristic of lymphoma.

    Description / Table of Contents: Zusammenfassung Durch Registrierung der Dichteveränderungen des Nierentransplantats auf schnell wiederholten CT-scans (Sequenz-CT) nach i. v. Kontrastmittel-Bolusinjektion lassen sich Parameter registrieren, die Hinweise auf die regionale Organperfusion geben. Da die Abstoßungsreaktion des Transplantats im Gegensatz zur Normalfunktion und ATN mit einer corticalen Minderperfusion einhergeht, ist eine Differenzierung beider Funktionsstörungen möglich. Bei 300 Untersuchungen von 100 Patienten wurden die wichtigsten Parameter und deren diagnostische Sicherheit ermittelt. Neben morphologischen Veränderungen wie Nierenvergrößerung und Ausscheidungsverzögerung erwiesen sich Einzelheiten der Enhancementkurven wie Steilheit des Anstiegs oder die Dauer des Rinden-Mark-Dichtegradienten als wesentlich. Die diagnostische Sicherheit übertrifft die anderer Röntgenverfahren.
    Notes: Summary Density changes of the renal transplant are registered in rapidly repeated scans by computed tomography (sequential CT) after IV bolus injection of the contrast medium, which provides information about regional organ perfusion. Since there is marked reduction of cortical perfusion in transplant rejection in comparison to normal function or ATN, there is a possibility of differentiating functional lesions. One hundred patients underwent 300 examinations, the results of which were evaluated. Besides morphological changes like increasing size and delayed excretion, detail of enhancement curves like initial slope and duration of the cortex-medulla density gradient proved to be the most important diagnostic parameters. Diagnostic accuracy is superior to other X-ray modalities.

    Notes: Summary Human sera containing the autoimmune specificities anti-RNP, anti-Sm or anti-La were used to screen a ν gt 11 human cDNA library. Positive clones were isolated and further characterized. To verify the specificity of the antibodies reacting with the cloned antigen, immobilized fusion proteins were used to select epitope specific antibodies. These were then used in immunofluorescence and immunoblotting experiments. The cDNA inserts from nositive clones were cloned into plasmid vectors to facilitate sequencing and mapping of the regions coding for the different autoantigenic epitopes.
