    Description / Table of Contents: Zusammenfassung 5-Fluorouracil führt schon in relativ niedriger Dosierung zu einer Verminderung der Disaccharidase-Aktivitäten in der Dünndarmschleimhaut der Ratte. Bei höheren Dosierungen kommt es neben morphologischen Veränderungen der Darmmucosa zu einem erheblichen Abfall der Disaccharidase-Aktivitäten, die nach einer Einzeldosis auch nach morphologischer Restitution noch über Tage vermindert sein können. Der therapeutisch induzierten Disaccharid-Malabsorption sollte auch bei der zytostatischen Behandlung des Menschen größere Aufmerksamkeit geschenkt werden.
    Notes: Summary Activities of rat intestinal disaccharidases are decreased following treatment even with low doses of 5-fluorouracil. Higher doses of the cytostatic drug cause morphological alterations of intestinal mucosa accompanied by a marked drop in enzyme activities. After a single dose of 5-fluoro-uracil, reduced disaccharidase activities are maintained for a couple of days even after morphological restitution of the mucosa. Malabsorption of disaccharides associates with cytostatic treatment should be paid more attention also in man.

Das Zustandsbild ist dem Kleeblattschädel-Syndrom von Holtermüller und Wiedemann zuzuordnen. Unterleibs-Röntgenaufnahmen der Mutter in der Frühschwangerschaft werden als mögliche Ursache für die Mißbildungen diskutiert. Die ausgesprochene Segmentbezogenheit der Fehlbildungen weist auf eine ursegmentale Störung hin, wobei besonders die Kopfsomiten und die Extremitätenknospen betroffen worden sind.
    Notes: Abstract A case report is presented describing an infant with systemic skeletal malformation including a “cloverleaf skull” (Kleeblattschädel) cleft formation of the facial bones, malformation of the brain, hydrocephalus, and symmetrical notching of hands and feet. This entity has been described as “clover leaf skull syndrome” by Holtermüller and Wiedemann. The etiology of the syndrome is discussed with special reference to lower abdominal X-ray examinations of the mother in early pregnancy as a possible cause of the disorder. The malformations result most likely from a disturbance in the development of the primitive segments involving mainly cranial somites and extremity buds.

    Notes: Abstract In 1972, Wolcott and Rallison described three siblings with a combination of infancy-onset diabetes mellitus and multiple epiphyseal dysplasia. We have observed a brother and sister with the same disorder. The chondro-osseous lesions are those of a spondylo-epiphyseal dysplasia. The diabetes mellitus is relatively mild. Histologic and electron microscopic studies of chondro-osseous tissue show findings similar to those in other epiphyseal and spondylo-epiphyseal dysplasias. In addition, however, atypical collagen-like fibres are found inside and outside chondrocytes. Collagen production seems to be normal in cultured fibroblasts. From the available data it appears that the association of characteristic chondro-osseous and endocrine abnormalities is non-random and that the lesions are independent manifestations of a pleiotropic gene. We propose to call this disorder the Wolcott-Rallison Syndrome.

    Notes: Abstract A male infant with pale-blue sclerae, who died at the age of 6 weeks through the aspiration of food, presented multiple fractures and deformation of the long tubular bones. The clinical and radiological findings and the course indicated osteogenesis imperfecta, type III, according to Sillence's classification. The family history was unremarkable. Light and electron microscopic studies of iliac crest bone obtained postmortem, showed an abrupt interruption of endochondral ossification, with an active periosteal ossification. In the region of the fractures, a mixed desmochondral callus was seen. The endoplasic reticulum of the osteoblasts was markedly dilated, the mitochondria were swollen. The osteoid was reduced in quantity. A postmortem analysis of the collagen types I, II and III obtained from skin, cartilage and bone yielded chromatographically normal collagen constituents. An analysis of the amino acids of the collagen α-chains showed an increased hydroxylysine content. The radiological findings and the clinical course both indicated type III osteogenesis imperfecta: identical biochemical findings have been described only for type II. The morphological and biochemical findings described here may be a manifestation of a variable expressivity of type III osteogenesis imperfecta. On the other hand, heterogeneity of type II osteogenesis imperfecta cannot be ruled out.

    Notes: Summary New methods are presented for the numerical integration of ordinary differential equations of the important family of Hamiltonian dynamical systems. These methods preserve the Poincaré invariants and, therefore, mimic relevant qualitative properties of the exact solutions. The methods are based on a Runge-Kutta-type ansatz for the generating function to realize the integration steps by canonical transformations. A fourth-order method is given and its implementation is discussed. Numerical results are presented for the Hénon-Heiles system, which describes the motion of a star in an axisymmetric galaxy.

    Notes: Summary A class of numerical methods for the treatment of delay differential equations is developed. These methods are based on the wellknown Runge-Kutta-Fehlberg methods. The retarded argument is approximated by an appropriate multipoint Hermite Interpolation. The inherent jump discontinuities in the various derivatives of the solution are considered automatically. Problems with piecewise continuous right-hand side and initial function are treated too. Real-life problems are used for the numerical test and a comparison with other methods published in literature.

    Notes: Summary Hypopharyngeal carcinomas are usually diagnosed in stage 3 and 4 and, even when subjected to combined radiological and surgical therapy, have a very poor prognosis. An improvement in this lamentable therapeutic and diagnostic situation can be expected only from early detection. Therefore, for the detection of precancerous and early stages of hypopharyngeal carcinoma, the proven techniques employed for early detection and grading of precancers and early carcinomas in the larynx were employed. Within the framework of this prospective study, a total of 328 patients have been examined since the beginning of 1978. Following the completion of a computer-compatible questionnaire covering pre-history, symptoms and smoking and drinking habits, a complete endoscopic investigation of the head and neck regions was carried out. For the examination of the pharynx and larynx, the v. Stuckrad zoom endoscope has proved its value, since, with it, even concealed regions of mucosa, as in the hypopharynx, can be reliably inspected. Thus, the visual early detection of very minute and early changes of the mucosa is also possible in this area. Cytological material is obtained from the various topographical regions, stained in accordance with Papanicolaou and evaluated. In the case of suspicious findings (Pap. III) an endoscopic-cytological check is carried out, or tissue biopsy taken for histomorphological work-up. The analysis of the cytological findings showed that, in particular, six cases with Pap. III, manifesting no visible changes in the mucosa of the hypopharynx, were of interest. Since multiple check cytological examinations indicated the moderate dysplasia, these patients were classed as being at particular risk and are now being monitored endoscopically and cytologically, every 3 months. In the examination of etiologically high-risk groups and patients with precancerous conditions of the upper airways and alimentary tract, cytologically suspicious cases indicating dysplasia or early tissue changes in the mucosa, may be more frequently expected in the hypopharynx, too. Only long-term observation will reveal how a cytological finding with dyscaryosis (Pap. III), in the presence of a clinically and endoscopically normal mucosa is to be classified.
