    Description / Table of Contents: Summary Electronmicroscopical, histochemical and biochemical investigations were carried out in a case of socalled Reticulosarcomatosis of the skin (Gottron). The electronmicroscopical results show a typical morphological substrate of the tumor tissue. It is composed out of multinuclear cell complexes containing individual mononuclear cells. There is only one single contured membrane of low optical density between the mononuclear individual cells and the multinuclear giant cell formations. It is suggested that the individual cells loose their plasmolemms and murge into the environmental giant cell formations. The almost complete absence of nuclear membranes, the high content of free ribosomes in the cytoplasma, the diminution and the morphological alteration of the mitochondria points towards a high metabolism of these tumor cells mainly directed to the production of cell-own proteins. Histochemically the tumor cells show a high activity of lactate dehydrogenase as a glycolytic enzyme. Histochemical reactions of enzymes of the citric acid cycle and the cytochrome-system were relatively weak. Quantitative biochemical analysis revealed relatively high activities of enzymes of glycolysis and pentose-phosphate-shunt and comparatively low activities of enzymes which are bound to mitochondria like NAD-linked glutamate dehydrogenase or restricted for special metabolic pathways. The results of the experimental investigations show a good correlation between the morphological and biochemical organization of tumor cells in socalled Reticulosarcomatosis (Gottron).
    Notes: Zusammenfassung Es wird über elektronenmikroskopische, histochemische und biochemische Untersuchungen bei sog. Reticulosarkomatose der Haut (Gottron) berichtet. Die elektronenmikroskopischen Befunde zeigen, daß das Tumorgewebe ein charakteristisches feingewebliches Substrat besitzt. Es setzt sich zusammen aus vielkernigen Zellkomplexen und in diesen gelegenen einkernigen Individualzellen. Die einfach ausgebildete und schwach konturierte Membran zwischen einkernigen Einzelzellen und mehrkernigen riesenzellartigen Zellformationen spricht dafür, daß die Einzelzellen unter Verlust ihres Plasmalemms in die sie umgebenden riesenzellartigen Zellformationen aufgehen. Das weitgehende Fehlen von Kernmembranen, die hohe Zahl von freien Ribosomen und die Verminderung sowie morphologische Alteration der Mitochondrien sprechen für einen hohen Stoffwechsel der Tumorzellen, der vorwiegend auf die Produktion zelleigenen Proteins ausgerichtet ist. Histochemisch waren die Tumorzellen charakterisiert durch einen starken Reaktionsausfall auf das glykolytische Enzym Lactatdehydrogenase. Der histochemische Reaktionsausfall auf Enzyme des Citronensäurecyclus und der Atmungskette war vermindert. Die quantitativ-biochemischen Untersuchungsergebnisse zeigten relativ hohe Aktivitäten von Hauptkettenenzymen und des Pentosephosphat-Shunts bei vergleichsweise niedrigen Aktivitäten von Enzymen, die mitochondrial lokalisiert sind (NAD-spezifische Glutamatdehydrogenase) oder speziellen Stoffwechselwegen zuzuordnen sind.

    Notes: Summary There is a wealth of information about monoclonal antibody (MAb) specificity and function on fixed tissues, yet little is known about formation and release of antigen-antibody complexes and their functional behavior in vivo. We analyzed the pathway of radiolabeled MAbs directed against melanoma-associated antigens by radioimmunoelectron microscopy (RIEM) on metabolically active cells of the melanoma cell lines SK-MEL-28, MeWo and Colo 38 at different time intervals. In parallel, binding and release of MAbs were investigated by the radioantibody binding assay (RBA). Both procedures gave essentially concordant results. Preferentially stable binding of immune complexes (ICs) to the cell surface after 30 and 120 min was shown for the MAb L10. Internalization was demonstrated for the MAb M.2.9.4. At the ultrastructural level, direct evidence of this phenomenon was obtained by visualization of radioactivity within the cytoplasm after 120 min. In the RBA this process was indicated by resistance of bound MAbs to acid buffer desorption. RIEM pointed to different transport mechanisms: constitutive internalization by endocytotic vesicles, or receptor-mediated endocytosis by coated vesicles. Shedding was indicated for the MAb R24 by release of the ICs from the cell membrane. It was demonstrated that stable fixation of ICs on the cell surface or modulation by internalization led to high accumulation rates, while shedding of antigen-antibody complexes resulted in a low accumulation of the MAb in tumor cells. Assuming that the potential of MAbs for clinical application is determined by the biological behavior of antigen-antibody complexes, these methods are suitable for demonstration of antigenic modulation by MAbs and eventually enable us to predict the localization, penetration and distribution pattern of individual MAbs in the melanoma patient.

    Notes: Tufted hair folliculitis (THF) is a rare disease which is characterized by the emergence of multiple hairs from widely dilated follicular orifices surrounded by an inflammatory infiltrate resulting in scarring alopecia. The pathogenesis is not yet fully understood. Although colonization with Staphylococcus aureus could not always be detected and systemic treatment with antibiotics alone is not sufficient, this microorganism is considered to play an important role. Around 30 patients with THF have been reported since the first publication. We present a patient with pemphigus vulgaris who developed THF. To our knowledge, this is the fourth case with an association of these two entities. Amongst other causing mechanisms, the autoimmune reaction may play an important role for the development of THF.

    Notes: : A double-blind, 24-week, placebo-controlled study was conducted in Germany to evaluate the safety and efficacy of topically applied 2% minoxidil solution in the treatment of male pattern baldness. One hundred sixty-three patients were evaluable, and applied 1 ml of either the placebo or 2% minoxidil solution twice daily throughout the study. The 2% minoxidil solution was significantly superior to placebo in all primary efficacy parameters (nonvellus hair count, and investigator and patient subjective evaluations of hair regrowth). One patient in each group discontinued treatment after developing scalp dermatitis: both cases were considered to be related to the treatment. No significant changes were observed in vital signs or laboratory parameters. Topical minoxidil 2%solution is safe, and is a more effective treatment than placebo for early male pattern baldness.

    Notes: Abstract Protein I has become the basis of serotyping of N. gonorrhoeae. A panel of 12 anti-protein I monoclonal antibodies is supplied by Syva Company, Palo Alto, CA, through distributors for research purposes only. These distributors are responsible for storing the monoclonal antibodies, supplying research groups and training new users, particularly in the interpretation of coagglutination reactions. A European Workshop was held in Heidelberg, Federal Republic of Germany in July 1988 to discuss serotyping with these antibodies. The current uses of serotyping of N. gonorrhoeae include epidemiological studies, clinical purposes and surveillance of antibiotic resistance and plasmid carriage. Predominant serovars may be subtyped either by additional antibodies or with the use of another technique, such as auxotyping, determination of antibiotic sensitivities, plasmid analysis or genetic fingerprinting. Conversely, there is growing evidence that it may be appropriate to group certain serovars together. Data collected during prevalence studies could be combined with clinical information and an international data bank set up. This supposes a close future collaboration of all groups involved in the epidemiology of N. gonorrhoeae.

    Description / Table of Contents: Abstract Background and Objective. In the therapy of malignant melanoma sentinel lymph node(SLN) excision has assumed increased importance. The localisation of the sentinel node is possible by lymphoscintigraphy and gamma probe guidance. Aim of the study was to prove whether SLN can be identified sonographically. Patients/Methods. 23 patients (16 women, 7 men; average age 42.7 years) with malignant melanoma required SLN excision. Before the patient underwent lymphoscintigraphy, sonography of the regional lymph nodes was performed. The position of lymph nodes (LN) with asymmetrical extension of the cortical substance was marked cutaneously according to the probe position in two axis (M1). During lymphoscintigraphy the gamma probe position orthograd to the skin with the highest count rate was marked (M2). Then a second sonography of the region was performed. Results. In all patients M1 and M2 marked the same point. During the operation the sonographicalliy documented position of the SLN could be confirmed in all cases. The second sonography after the lymphoscintigraphy showed a more blurred distinction between the cortical substance and the center of the lymph nodes. Conclusions. Our results show that sonography allows an identification of SLN. We think that the preoperative sonography of the SLN is an important supplementary method in addition to lymphoscintigraphy.
    Notes: Zusammenfassung Hintergrund und Fragestellung. Die Exstirpation des mittels Lymphabstromszintigraphie dargestellten Sentinel-Lymphknotens gewinnt in der Therapie des malignen Melanoms an Bedeutung. Die Frage, ob der Sentinel auch sonographisch diagnostiziert werden kann, war Gegenstand der Untersuchung. Patienten/Methodik. Bei 23 Patienten mit malignem Melanom (16 w, 7 m, Durchschnittsalter: 42,7 Jahre) wurde die Indikation zur Exstirpation des Sentinel-Lymphknotens gestellt. Vor der präoperativen Lymphabstromszintigraphie wurde die Sonographie der regionären Lymphknoten durchgeführt. Die Lokalisation von Lymphknoten mit umschrieben kappenartig verbreiterter Rinde wurde kutan entsprechend der Insonationsachsen markiert (M1). Während der Tc99m-Lymphabstromszintigraphie wurde der Sondenauflagepunkt mit der höchsten Countrate senkrecht zur Hautoberfläche markiert (M2). Anschließend erfolgte eine 2. Sonographie der Region. Ergebnisse. Bei allen Patienten bestand eine direkte Übereinstimmung von M1 und M2. Intraoperativ konnte die sonographisch dokumentierte Lage zu den Nachbarstrukturen des Sentinel in allen Fällen bestätigt werden. Die Sonographie nach der Szintigraphie zeigte eine unschärfere Abgrenzung zwischen Rinde und Mark. Schlussfolgerungen. Unsere Ergebnisse lassen darauf schließen, dass der Sentinel-Lymphknoten sonographisch identifiziert werden kann. Die präoperative Sonographie stellt eine wichtige Ergänzung zur Lymphabstromszintigraphie dar.

    Notes: Summary A young female patient, expressing the symptom triad of Netherton's syndrome, i.e., ichthyosis linearis circumflexa Comèl, trichorrhexis invaginata and other hair shaft defects, and atopic diathesis, has been treated successfully with the new retinoid preparation Etretin. Our electron microscopical study especially focused on the ultrastructural effect on the characteristic, active part of the skin lesions, which is only found within a narrow borderline just preceding the lesion's margin. In untreated skin, this part is characterized by dermal inflammation, immigrating inflammatory cells, and specific keratinization disturbances: synthesis of keratinization proteins is suppressed, serum exudates invade the epidermis, either filling the intercellular spaces of the upper spinous and the granular layer as finely granular, amorphous material, or they are partly phagocytosed and lie within intracellular, round-oval inclusions. The portions of the lesions lying towards the center are unspecific and represent recovery stages, ultrastructurally resembling stages of normal wound repair. Oral therapy with Etretin did not heal the basic defect, but drastically reduced exoserosis and the deposition of intra- and extracellular material. Keratinization seemed to normalize. The condition of the hair was also improved.

    Description / Table of Contents: Summary Several comparative histochemical and biochemical methods for the determination of dehydrogenases of the energy supplying system are compared and the difficulties encountered in the interpretation of quantitative enzyme histochemical reactions are discussed. According to the results it is even with modern histochemical methods only possible to get an approximation of the absolute value of the enzyme activity in question. The strongest point of enzyme histochemical methods lies still in the enzyme histotopical field. Quantitative assessments will therefore give only approximate values and usually need complementary biochemical determinations.
    Notes: Zusammenfassung Am Beispiel einiger vergleichender histochemischer und biochemischer Aktivitätsbestimmungen von Dehydrogenasen des energieliefernden Stoffwechsels werden die Schwierigkeiten einer quantitativen Auswertung enzymhistochemischer Reaktionen aufgezeigt und diskutiert. Nach unseren Ergebnissen erlaubt der jetzige Stand der histochemischen Methodik nur grobe Rückschlüsse auf die absolute Höhe der vorliegenden Enzymaktivitäten. Die Stärke enzymhistochemischer Methoden liegt auch weiterhin auf dem Gebiet der Enzymhistotopie. Quantitative Fragestellungen bedürfen in der Regel ergänzend der biochemischen Methodik.
