    Notes: Evans tenodesis is one of the techniques used much for correcting the chronic lateral instability of the ankle. Long-term results (average follow-up 8 years) are reported for 113 operations on 109 patients. It is concluded that the method used gives a good functional result when the major complaint is recurrent sprains of the ankle. Functional stability does not necessarily correlate with mechanical stability when tested clinically or by radiographs.

    Notes: Abstract The effects of 30 min running with stepwise increasing intensity (exhaustive, energy demand approx. 50 → 100% ofVO2max), 60 s supramaximal running (anaerobic, ≥125% ofVO2max) and 40–60 min low-intensity running (acrobic, 40–60% ofVO2max) on serum concentration of muscle-derived proteins were studied in 5 male and 5 female elite orienteerers. S-Carbonic anhydrase III (S-CA III) was used as a marker of protein leakage from type I (slow oxidative) muscle fibres and S-myoglobin (S-Mb) as a non-selective (type I+II) muscular marker. The fractional increase in S-CA III (ΔS-Ca III) was 0.37±0.09 (mean±SEM,p〈0.001), 0.10±0.05 (N. S.) and 0.46±0.09 (p〈0.001) 1 h after exhaustive, anaerobic and aerobic exercise, respectively. The corresponding values for ΔS-Mb were 1.45±0.36 (p〈0.001), 0.39±0.13 (p〈0.01) and 0.67±0.18 (p〈0.001). The value for the ΔS-CA III/ΔS-Mb ratio was 0.68±0.03 after the acrobic exercise, but only 0.25–0.26 (p vs. aerobic exercise 〈0.001) after the two high-intensity forms of exercise. Since type I fibres of skeletal muscle are known to be responsible for power production during low-intensity exercise, whereas fibres of both type I and type II are active at higher intensities, the ΔS-CA III/ΔS-Mb ratio may depend on the recruitment profile of type I vs. type I+II fibres.

    Description / Table of Contents: Zusammenfassung In einem Zeitraum von 11 Jahren wurden 15 Fälle von Ermüdungsfrakturen des Sesambeines am I. MTP-Gelenk diagnostiziert und behandelt. Das Durchschnittsalter betrug 22,3 Jahre, die Geschlechtsverteilung: 6 Frauen und 9 Männer. Alle Patienten waren Sportler, bei denen die Beschwerden während des Trainings auftraten, ohne direktes Trauma. In 8 Fällen befand sich die Fraktur im medialen, in 6 im lateralen Sesamoid und in einem Fall waren beide Sesambeine betroffen. Zur genauen Diagnose kam man durch die Anamnese, wie auch durch die Röntgen- und Isotopenuntersuchung. 10 Patienten wurden konservativ behandelt, Vermeidung excessiver Belastung und das Tragen von besseren Trainingsschuhen wurden vorgeschrieben. In 5 Fällen mußte chirurgisch vorgegangen werden, das fragmentierte, Schmerz verursachende Sesambein wurde entfernt. Die Nachbehandlung verlief komplikationslos und die Sportler konnten ihr Training nach 6 bis 8 Wochen post Op wieder aufnehmen. Die histologische Untersuchung der extirpierten Sesambeine ergab: fibrotisch veränderte Frakturflächen, was die Diagnose einer Stressfraktur bestätigte. 3 der konservativ behandelten Sportler haben immer noch leichte Beschwerden bei intensivem Training, die anderen sind beschwerdefrei.
    Notes: Summary Over a period of 11 years 15 cases of stress fractures of the sesamoid bones of the first metatarsophalangeal (MTP) joint were treated in athletes. The mean age of the patients was 22.3 years, and there were 9 males and 6 females in the series. All patients were athletes, who began to suffer from the symptoms during training without any trauma. Eight fractures were located in the medial, six in the lateral sesamoid bone, and in one case both sesamoids were affected. The diagnosis was performed on the basis of the history, symptoms, clinical examination, and radiological, or isotope scanning findings. Ten of the patients were treated conservatively by prescribing an avoidance of excessive physical activity and better training shoes. In five cases surgical excision of the fragmented painful sesamoid bone was performed. There were no complications in the series and the athletes could start gradually training 6–8 weeks after the operation. The histology showed fibrotic non-union at the fracture site and supported the diagnosis of stress fracture. Three of the conservatively treated athletes had mild symptoms in intensive training, others had a good end result.

    Description / Table of Contents: Résumé Entre 1985 et 1989 furent réalisées 102 reprises d'arthroplasties de hanche sur 92 patients en utilisant un tige fémorale isoélastique. Dans 45% des cas la métaphyse fémorale était détériorée. 11 des patients sont décédés pendant le suivi moyen de 5,7 années. Le composant fémoral a été-révisé pour prise de jeu dans 13 cas, pour infection dans 5 cas et pour dislocation dans 3 cas. Un examen radiologique de 70 hanches révéla une migration de 5 millimètres ou plus dans 27 hanches et un début de migration fut détecté dans 20 hanches. Neuf tiges subirent une migration de plus 8 mm et furent jugées comme descellées de facon certaine. Toutes les fractures (11 préopératoires et 15 preopératoires) guérirent sans problème. Dans la plupart des cas une légère hypertrophie corticale ń excédant pas 2 mm était observable radiographiquement. 3 patients furent exclus de l'évaluation clinique par suite d'une maladie générale sévère. Parmi les 66 hanches restantes la fonction fut excellente dans 18% des cas, bonne dans 50%, passable dans 26% et mauvaise dans 6% des cas selon l'échelle des résultats de D'Aubigne-Postel. Le problème de la tige isoélastique est la déficience de la fixation primaire, comme l'indique le grand nombre de problèmes précoces. Les resultats montrant une proportion d'échec globale de 33% sont non-satisfaisants et la tige isoélatique utilisée dans cette étude ne peut pas être recommandé pour une révision primaire de hanche.
    Notes: Summary We carried out 102 hip revision arthroplasties using an uncemented isoelastic femoral stem on 92 patients between 1985 and 1989. The proximal femoral bone stock had deteriorated in 45%. Eleven patients died during the mean follow up of 5.7 years. The femoral component has been revised again for loosening in 13, for infection in 5 and for dislocation in 3. Radiographs of 70 hips showed incipient migration at 3 months in 20, and at the time of review 27 hips had migrated 5 mm or more. Nine stems had migrated more than 8 mm and were judged to be loose. There were 11 fractures before operation and 15 during operation; they all healed. Slight cortical hypertrophy of not more than 2 mm was present in most cases. Three patients (4 hips) were excluded because of severe systemic illness. Of the remaining 66 hips, the clinical outcome was excellent in 18%, good in 50%, fair in 26% and poor in 6%. The isoelastic stem is associated with poor primary fixation which is indicated by early subsidence. The results, with a total failure rate of 33%, are unsatisfactory and the isoelastic femoral stem used in this series cannot be recommended for revision operations.
