    Notes: Summary To investigate the effects of fluid expansion on endogenous atrial natriuretic peptide (ANP) and cyclic 3′,5′-guanosine monophosphate (cGMP), four male volunteers were studied before, during and after intravasal volume loading. Volume expansion was performed by intravenous infusion of 2,000 ml isotonic saline solution within 30 min. Mean plasma ANP levels increased 2.5-fold from 31.2 pg/ml to 81.7 pg/ml 40 min after the start of infusion. Plasma cGMP levels paralleled the rise in ANP, shwoing a mean cGMP increment from 2.7 pmol/ml to a maximum of 8.2 pmol/ml. Both ANP and cGMP levels were back to basal levels 120 min after termination of the infusion. Stimulation of endogenous ANP release by volume loading suggests that ANP is involved in the regulation of fluid homeostasis in man. The parallel rise in plasma cGMP levels supports the idea that cGMP is a mediator for the effects of ANP.

    Description / Table of Contents: Abstract Introduction. Transfusion-associated infections with hepatitis C virus (HCV) are seen in 1:28,000 to 1:288,000 cases per transfused blood unit in Germany today, infections with hepatitis B virus (HBV) and human immunodeficiency virus (HIV) in about 1:147,000 to 1:31,000 and 1:2 Mio to 1:200,000 cases, respectively. Under their immunosuppressive therapy oncologic patients acquire a HCV- or HBV-infection easily and infection takes a chronic course. Methods. In a retrospective investigation children with hematological and oncological diseases attending Kinderpoliklinik München were tested serologically for infections with HCV (n=165), HBV (n=167), and HIV (n=171). In total, they had received 20,850 blood products there (including 3648 cellular preparations or fresh frozen plasma). Results. Infections with HBV or HIV were not seen. All 6 HCV-infected patients had received transfusions abroad. Conclusions. The rates of infection were much lower than described in the literature. Nevertheless, the transfusion-associated risk of infection must not be neglected and in administration of blood products advantages and risks have to be considered carefully.

Transfusionsbedingte Hepatitis-C-Virus(HCV)-Infektionen werden in Deutschland heute in 1:28.000–1:288.000 Fällen pro Bluteinheit beobachtet, Infektionen mit Hepatitis-B-Virus (HBV) oder Humanen Immundefizienzviren (HIV) mit 1:147.000–1:31.000 bzw. 1:2 Mio.–1:200.000 wesentlich seltener. Unter immunsuppressiver Therapie erwerben Patienten mit onkologischen Erkrankungen häufig eine HCV- oder HBV-Infektion mit chronischem Verlauf. Methode. An der Kinderpoliklinik München wurden retrospektiv Kinder mit hämatologisch-onkologischen Erkrankungen serologisch auf Infektionen mit HCV (n=165), HBV (n=167) bzw. HIV (n=171) untersucht. Sie hatten dort insgesamt 20.850 Blutprodukte erhalten (davon 3648 zelluläre Präparationen oder gefrorenes Frischplasma). Ergebnisse. Infektionen mit HBV oder HIV wurden nicht beobachtet. Alle 6 mit HCV infizierten Kinder waren zuvor im Ausland transfundiert worden. Diskussion. Die Infektionsraten waren wesentlich niedriger als vielfach in der Literatur beschrieben. Dennoch muss das transfusionsbedingte Risiko berücksichtigt werden. Bei der Verabreichung von Blutprodukten sind Nutzen und Risiken streng gegeneinander abzuwägen.
