    Notes: Summary The acute haemodynamic effects of the beta-adrenoreceptor agonist, prenalterol, were studied in six patients with chronic end-stage renal failure. Prenalterol 0.8 mg, 1.6 mg, and 3.2 mg was administered i.v. as a bolus, and after the last dose the selective adrenergic beta-1-receptor antagonist metoprolol was administered i.v. in doses of 5 and 10 mg. The haemodynamic effects of the drugs were investigated using impedance cardiography and radionuclide angiocardiography. The main haemodynamic effects were a dose-related chronotropic effect, demonstrated by an increase in heart rate (26%; 〈0.05), and an inotropic effect, shown by an increase in stroke volume index (20%;p〈0.05) and left ventricular ejection time (12%;p〈0.05); the cardiac index was increased by 47% (p〈0.05). Transmural myocardial perfusion (DPTI/SPTI ratio) was decreased by 22% (p〈0.05) after prenalterol. It is concluded that prenalterol has positive inotropic and chronotropic effects in patients with chronic renal failure, that the improvement in left ventricular performance is at the expense of a decreased transmural myocardial perfusion, and that metoprolol is a specific antidote.

    Notes: Summary In fifteen patients with essential hypertension WHO I–II (eight men and seven; mean age 45 years) blood pressure (BP), plasma volume (PV;125I-Albumin space), extracellular volume (ECV;82Brdistribution space) and glomerular filtration rate (GFR;51Cr-EDTA clearance) were measured before and during long-term antihypertensive therapy with metoprolol, alone, and in combination with thiazide diuretics. Metoprolol given alone to all patients for an average of 5 months (mean dose 230 mg) resulted in a reduction in systolic and diastolic pressure by 11% and 8%, respectively. In eight patients the BP reduction was considered inadequate (non-responders). In the group as a whole, ECV increased significantly by 5%. The increase in ECV in the non-responders was five times larger than in the responders (1.025 ml against 205 ml), which may in part account for the inadequate BP control. Addition of a thiazide diuretic for an average of 3.5 months resulted in reduction of ECV to its pretreatment level and a pronounced additional fall in BP, comparable to that in the responders. No systematic change in PV or GFR could be demonstrated. No difference was found between the two groups regarding sex, pretreatment BP, body fluid volumes or renal function. On average the non-responders were 20 years older than the responders. The clinical importance of the apparent age-related expansion of ECV and the mechanism behind fluid retention during antihypertensive treatment with beta-blocking drugs are discussed. As failure of BP control during treatment with beta-blockers may be due to fluid retention, concurrent use of diuretics is indicated.

    Notes: Summary In 73 patients on digitoxin maintenance medication a progressive increase of mean serum digitoxin activity was found with increasing dose. However, there were considerable variations within each group. Change of dose was followed by a new steady state level close to that predicted by zero order absorption-first order elimination kinetics of the drug. In atrial fibrillation the expected fall in ventricular rate was always observed with increasing serum concentration. No significant difference in serum level was found 24 h after identical peroral and intravenous doses had been given to different groups of patients. However, significantly higher serum values were found in normal subjects than in patients with heart disease after identical peroral treatment. This probably indicates poorer absorption by the patients. In 13 subjects, the estimated serum digitoxin half-lives were 3.7 to 11.3 days. These large differences may explain much of the variation in serum values found during steady state therapy. The elimination curves were in accordance with a first order drug kinetics. Only three patients with definite digitalis intoxication were seen, all of whom had high serum values which did not overlap patients without features of intoxication. The results showed that several patients on maintenance therapy were under-digitalized. It is hoped that this or similar methods may make it possible to adjust individual treatment to compensate for differences in absorption, distribution and elimination, and thereby to increase the benefits and decrease the risks of toxicity of digitoxin therapy.

    Notes: Summary In a double blind study, prazosin, a specific adrenergic alpha1-receptor antagonist, or placebo were given to 15 females with primary Raynaud's phenomenon. At a low dose (1 mg twice daily) 5 out of 7 of the prazosin-treated patients reported a reduction of attacks induced by cold (p〈0.05). This was not confirmed by a cold provocation test which showed no improvement at finger temperatures of 15 or 10°C. The highest tolerated dose in the prazosin-treated patients varied from 2–8 mg daily, and the greatest number of side effects was recorded in this group (p〈0.05). None of the patients experienced complete relief from cold-induced attacks. It was concluded either that Raynaud's phenomenon is not only caused by stimulation of alpha1-receptors in digital arteries, or the clinically achievable blockade was insufficient to prevent attacks.

    Notes: Abstract A follow-up study of monkeys who experienced maternal separations as infants 2.5–4.9 years earlier was performed. Matched pairs of previously separated and nonseparated control monkeys were observed individually in four unfamiliar situations, and their behavioral responses and plasma cortisol levels were measured. Results indicated that previously separated animals displayed more disturbance than did their matched controls, and levels of disturbance were significantly correlated with the degree of behavioral and physiological disturbance displayed during the original maternal separations. The results support the idea that the perception of the maternal separation may be a significant factor in whether long-term behavioral deficits exist in social and emotional functioning.

    Notes: Occupational dermatoses were investigated in a factory producing rotor blades for wind turbines by an epoxy-based process. In a blinded study design, 603 workers were first interviewed and thereafter clinically examined. Based on a history of work-related skin disease, clinical findings of dermatitis, or both, 325 (53.9%) of the workers were patch tested with a specially profiled occupational patch-test series and the European standard patch-test series. Calculated on all investigated workers, 17.1% of the workers were diagnosed with occupational dermatoses caused by work. Occupational allergic contact dermatitis was found in 10.9% of the workers. The estimated frequency of irritant contact dermatitis caused by work was 6.1%. Dermatitis on the hands was associated with contact allergy to epoxy resin (P = 0.017). The number of days on leave before the clinical examination was negatively associated with the presence of dermatitis (P = 0.001). Among workers employed 7–12 months, the frequency of occupational contact allergy was higher than that among workers employed for ≤6 months (P = 0.004). Females both washed their hands more often (P 〈 0.001) and used more moisturizers/protection creams at work (P 〈 0.001) than males. No sex differences were found concerning dermatitis on the hands.
