    Notes: Summary An anatomically-correct model of a larynx inside a head and neck model was constructed so that the location, direction and amount of force applied to the neck could be measured. Fifty trained staff from three general hospitals were then asked to apply cricoid pressure on the model. None was able to state the force that should be applied (30 N), and only five (10%) actually applied cricoid pressure effectively. After training using the model, 45 (90%) applied cricoid pressure correctly (p 〈 0.001). This study demonstrates that improved training in cricoid pressure is needed and supports earlier researchers who suggested that this can be achieved using simulators.

    Notes: Sensitivities of 16 monoconidial isolates of Venturia inaequalis to the sterol demethylation inhibitor flusilazole were found to be stable, when cultures which had been successively transferred to inhibitor-free nutrient agar and maintained at 20 C were retested 6 months after isolation. In contrast, with storage of these isolates at 2 C for 7 additional months, phenotypic sensitivities were subject to change. A high degree of phenotypic instability was also observed for 77 representative strains which had been subcultured for 10 months. In both cases, the majority of strains became more sensitive to flusilazole. The frequency and magnitude of these changes were most pronounced for strains with moderate levels of resistance. More than 70% of the strains investigated were collected from wild-type populations, suggesting that the reversion of resistant strains was not related to a process of unstable adaptation but to a high degree of plasticity in the genetic control of sensitivities to sterol demethylation inhibitors.

    Notes: The antifolate combination pyrimethamine/sulphadoxine (PYR/SDX; Fansidar) is frequently used to combat chloroquine-resistant malaria. Its success depends upon pronounced synergy between the two components, which target dihydrofolate reductase (DHFR) and dihydropteroate synthetase (DHPS) in the folate pathway. This synergy permits clearance of parasites resistant to either drug alone, but its molecular basis is still unexplained. Plasmodium falciparum can use exogenous folate, which is normally present in vivo, bypassing SDX inhibition of DHPS and, apparently, precluding synergy under these conditions. However, we have measured parasite inhibition by SDX/PYR combinations in assays in which folate levels are strictly controlled. In parasites that use exogenous folate efficiently, SDX inhibition can be restored by levels of PYR significantly lower than those required to inhibit DHFR. Isobolograms show that the degree of synergy between PYR and SDX is highly dependent upon prevailing folate concentrations and are indicative of PYR acting to block folate uptake and/or utilization. No significant synergy was observed at physiological drug levels when PYR/SDX acted on purified DHFR, whether wild type or mutant. We conclude that the primary basis for antifolate synergy in these organisms arises from PYR targeting a site (or sites) in addition to DHFR, which restores DHPS as a relevant target for SDX.

    Notes: This exploratory study tests one of the key assumptions of community policing: that there is a relatively high level of consensus both within and between community groups, or stakeholders, about community problems and potential solutions. Results show that in the target community there is some consensus about social problems and their solutions. However, the study also reveals that the consensus may not be community-wide, but may exist only among a relatively small group of "active" stakeholders who differ significantly about the seriousness of most of the problems and the utility of some solutions. Implications for community policing are discussed.

    Notes: The shikimic acid pathway, ubiquitous in microorganisms and plants, provides precursors for the biosynthesis of primary metabolites such as the aromatic amino acids and folic acid. Several branchpoints from the primary metabolic pathway also provide aromatic and, in some unusual cases, nonaromatic precursors for the biosynthesis of secondary metabolites. We report herein recent progress in the analysis of two unusual branches of the shikimic acid pathway in streptomycetes; the formation of the cyclohexanecarboxylic acid (CHC)-derived moiety of the antifungal agent ansatrienin and the dihydroxycyclohexanecarboxylic acid (DHCHC) starter unit for the biosynthesis of the immunosuppressant ascomycin. A gene for 1-cyclohexenylcarbonyl-CoA reductase, chcA, which plays a role in catalyzing three of the reductive steps leading from shikimic acid to CHC has been characterized from Streptomyces collinus. A cluster of six open reading frames (ORFs) has been identified by sequencing in both directions from chcA and the putative role of these in CHC biosynthesis is discussed. The individual steps involved in the biosynthesis of DHCHC from shikimic acid in Streptomyces hygroscopicus var ascomyceticus has been delineated and shown to be stereochemically and enzymatically distinct from the CHC pathway. A dehydroquinate dehydratase gene (dhq) likely involved in providing shikimic acid for both DHCHC biosynthesis and primary metabolism has been cloned, sequenced and characterized.

    Notes: Summary Limited information is available regarding the physiological responses to prolonged load carriage. This study determined the energy cost of prolonged treadmill walking (fixed distance of 12 km) at speeds of 1.10 m·s−1, 1.35 m·s−1, and 1.60 m·s−1, unloaded (clothing mass 5.2 kg) and with external loads of 31.5 and 49.4 kg. Fifteen male subjects performed nine trials in random order over a 6-week period. Oxygen uptake (VO2) was determined at the end of the first 10 min and every 20 min thereafter. A 10-min rest period was allowed following each 50 min of walking. No changes occurred in VO2 over time in the unloaded condition at any speed. The 31.5 and 49.4 kg loads, however, produced significant increases (ranging from 10 to 18%) at the two fastest and at all three speeds, respectively, even at initial exercise intensities less than 30% VO2max. In addition, the 49.4 kg load elicited a significantly higher (P〈0.05) VO2 than did the 31.5 kg load at all speeds. The measured values of metabolic cost were also compared to those predicted using the formula of Pandolf et al. In trials where VO2 increased significantly over time, predicted values underestimated the actual metabolic cost during the final minute by 10–16%. It is concluded that energy cost during prolonged load carriage is not constant but increases significantly over time even at low relative exercise intensities. It is further concluded that applying the prediction model which estimates energy expenditure from short-term load carriage efforts to prolonged load carriage can result in significant underestimations of the actual energy cost.

    Notes: Abstract An in vitro system was developed capable of testing the accuracy and reproducibility of pulse oximeter readings. The pulse oximeter probe receives signals through a pulsating blood cuvette. The development of the design of the cuvette is described. Using the final design (or ‘model finger’), a comparison is made between readings from a Datex Satlite pulse oximeter (SpO2) and saturation values obtained by use of a multiwavelength bench oximeter (SaO2). Linear regression analysis of the data gives SpO2=0·88 SaO2+11·2 (r=0·979, p〈0·001).
