    Description / Table of Contents: Summary Eight female patients with drug-induced agranulocytosis (five patients with definite seropositive rheumatoid arthritis (RA), three patients with upper respiratory infections) were studied for the presence of HLA-B27 and antinuclear antibodies (ANA). Five of eight patients were found to be HLA-B27 positive and all RA patients had ANA in their serum. The frequency of HLA-B27 and ANA was found to be significantly different from control groups. It is concluded that the occurrence of HLA-B27 in female patients with seropositive RA (especially in those with ANA) and of HLA-B27 alone in other individuals could reflect an increased risk for drug-induced agranulocytosis.
    Notes: Zusammenfassung Acht weibliche Patienten mit medikamentös ausgelöster Agranulozytose (fünf Patienten mit definitiver seropositiver rheumatoider Arthritis (RA), drei Patienten mit Infekten der oberen Luftwege) wurden auf das Vorliegen des HLA-Antigens B27 und von antinukleären Antikörpern (ANA) untersucht. Fünf von acht Patienten waren B27-positiv; alle Patienten mit RA hatten ANA in ihrem Serum. Die Frequenz von HLA-B27 und ANA war signifikant gegenüber Kontrollgruppen erhöht. Wir vermuten, daß das Vorkommen von HLA-B27 bei Frauen mit seropositiver RA (besonders bei jenen mit ANA) und von HLA-B27 allein bei anderen Personen Ausdruck eines erhöhten Risikos für eine medikamentös ausgelöste Agranulozytose sein könnte.

    Description / Table of Contents: Summary A forty year old healthy adult man was admitted to our out-patient department because of a highly elevanted rheumatoid factor. Since the person was planning an extended stay in Africa, we performed an extensive history, clinical and laboratory examination to rule out oligosymptomatic inflammatory diseases. We further analyzed if the person presented genetic risk factors to develop rheumatic diseases in the future or if the presence of the rheumatoid factor could be a reaction to multiple immunizations. Possible explanations for an elevated rheumatoid factor including the genetic risk factors are discussed.
    Notes: Zusammenfassung Ein gesunder 40jähriger Mann wurde wegen eines hohen Rheumafaktors in unserer Ambulanz vorgestellt. Da der Betroffene einen mehrmonatigen Auslandsaufenthalt in Afrika plante, wurden eine ausführliche Anamnese, klinische und laborchemische Untersuchungen durchgeführt. Die Vorstellung erfolgte zum Ausschluss einer oligosymptomatischen rheumatischen Erkrankung, zur Frage nach genetischen Risikofaktoren eine solche zu entwickeln, und zur Frage, ob Rheumafaktoren als Impfreaktion auftreten können; diese Aspekte werden diskutiert.

    Notes: Abstract Thirty-two female patients with confirmed ankylosing spondylitis (AS) and 33 women of similar age with pure ileitis terminalis Crohn were examined for genitourinary infection. Urethral syndrome was found in 15 out of 32 patients with AS: 11 of them had urethritis and 4 urethritis associated with vaginitis. Five women of the control group suffered from urethritis. In all cases with genitourinary infection, Chlamydia trachomatis was isolated. By comparing the AS-patients (urogenital infection group and the non-infected group) with regard to other present clinical parameters, it was found, as expected, that the erythrocyte sedimentation rate in the 1st hour was significantly higher in the infected group. In addition, the infected patients had a significantly higher incidence of enthesopathy, involvement of the spinal column, and higher C-reactive protein values (CRP ≥ 5 mg/l). A family history of AS was equally present. Other clinical parameters, such as inflammatory involvement of the joints and HLA-B27 correlation, did not differ significantly between infected and non-infected patients.
