    Notes: The benign vascular tumours known as angioleiomyomas, which originate from smooth muscle cells of arterial or venous walls are rare, particularly in the region of the hand. Frequently, the diagnosis is only made after surgical extirpation and histological assessment. The occurrence of an angioleiomyoma in a haemophiliac has not yet been reported, to our knowledge. A 62-year-old patient with a haemophilia B complained of increasing pain in the vicinity of the soft tissue covering the carpo-metacarpophalangeal joint of his left palm. In the T1-weighted magnetic resonance images a hypointense well-demarcated mass was found, showing a homogenous enhancement after intravenous application of contrast medium. T2-weighted images showed a hyperintense signal. Based on clinical and radiographic findings, the tumour was initially thought to be a haemophilic pseudotumour arising from a prior local haematoma. Intraoperatively, the mass was found to be solid, and histological assessment diagnosed an angioleiomyoma, without signs of malignant transformation. Descriptions of the clinical symptoms of angioleiomyoma in an extremity vary considerably in the literature. Alhough rare, the vascular leiomyoma should therefore be contemplated in the differential diagnosis in patients with a solid nodular lesion of unclear aetiology in arms or legs. Especially in a haemophiliac patient, the growth can easily be mistaken for a pseudotumour because of its clinical and radiological similarities.

    Notes: Summary Voltage clamp experiments were done on myelinated nerve fibres of Xenopus laevis to study the effect of calcium on the ionic currents of the nodal membrane treated with scorpion venom. Increasing the calcium concentration of the medium from 1.8 to 7.2 mM produced the following changes of the sodium and potassium permeabilities,P Na andP K: a) The incomplete sodium inactivation, which is typical for nodes of Ranvier treated with scorpion venom, is abolished. The curves relatingm ∞ and τ m to membrane potential are shifted by 8–10 mV towards larger depolarizations.P Na and the time constants of the fast and slow components of the sodium inactivation remain practically unchanged. b) The maximum potassium permeability,P K, which is reduced by scorpion venom to 45% of the value observed in the normal node, increases to 68% by addition of calcium ions; τ n is unchanged. These effects of calcium ions on the ionic permeabilities explain the shortening of the extremely prolonged action potential and the inhibition of spontaneous activity observed in nodes of Ranvier poisoned with scorpion venom.

    Description / Table of Contents: Zusammenfassung Die klinische und radiologische Diagnostik von Labrumverletzungen ist schwierig. Erfahrungen mit der i. v.-Injektion gadoliniumhaltiger Kontrastmittel (KM) und der damit verbundenen Signalintensitätssteigerung im Kniegelenk erleichtern Aussagen bezüglich Knorpel- und Meniskusläsionen. In dieser Studie wurde erstmals die Technik der indirekten MR-Arthrographie am Schultergelenk in der Diagnostik von Labrumläsionen evaluiert. Methode: 28 Patienten mit klinischem Verdacht auf eine Verletzung des Labrum glenoidale und/oder der Rotatorenmanschette wurden prospektiv mit einer nativen MR-Standarduntersuchung und einer indirekten MR-Arthrographie (0,1 mmol/kg KG Gadolinium-DTPA i. v.) untersucht. Die Ergebnissicherung erfolgte mittels Arthroskopie und/oder Arthrotomie. Bei 20 Patienten wurden klinische Tests (Apprehension-, Schubladen-, Grinding-, Sulkus-, Yergason-Test) in Hinblick auf ihre Aussagekraft bei Labrumverletzungen überprüft. Ergebnisse: Die indirekte MR-Arthrographie führte zu einer signifikant besseren Abgrenzbarkeit von Labrum und glenoidalem Knorpel (p 〈 0.05). Die indirekte MR-Arthrographie zeigte im Nachweis von Labrumrissen eine Sensitivität von 90 % sowie eine Spezifität von 89 %, verglichen mit 79 und 67 % der MR-Standarduntersuchung. Die klinischen Tests (Treffsicherheit zwischen 50 und 70 %) waren alle nicht zuverlässig genug, eine Labrumläsion vorherzusagen.
    Notes: Purpose: Clinical and radiological evaluation of labral tears remains challenging. It has been shown that intravenous administration of contrast agents produces an MR arthrographic effect without the need for intraarticular injection. This is the first study evaluating this new technique of indirect MR arthrography in diagnosis of glenoid labrum tears. Methods: 28 patients with clinically suspected labral injuries were prospectively investigated (1.5 Tesla, flexible surface coil). A native MR exam of the shoulder (transverse and oblique-coronar orientation, T1-weighted spin-, proton density- and T2*-weighted gradient echo sequences) and indirect MR arthrography (transverse and oblique-coronar orientation, fat-suppressed T1-weighted spin-echo sequences, intravenous injection of gadopentetate dimeglumine [0.1 mmol/kg], followed by 10–15 min of joint movement) were performed. Results were confirmed by arthroscopy and/or open surgery. Additionally sensitivity evaluation of clinical tests for investigation of labral tears were performed. Results: Indirect MR-Arthrography improved delineation of the glenoid labrum and hyaline cartilage significantly (p 〈 0.05). Sensitivity and specificity of indirect MR arthrography in diagnosis of labral injuries were 90 % and 89 % respectively, compared to 79 % and 67 % of the native MR exam. The reliability of the checked clinical tests is not sufficient enough to determine labral lesions (predictive value between 50 and 70 %). Conclusion: Indirect MR-arthrography is a promising non-invasive technique in the evaluation of the glenoid labrum.

    Description / Table of Contents: Summary Fibrous dysplasia, in either monostotic or poliostotic form rarely involves the cervical spine. Most of these lesions remain asymtomatic and are incidental radiographic findings. A symptomatic case of polyostotic fibrous dysplasia involving the first, second and third cervical vertebra with potentially serious consequences is presented. A 53-year old woman who had shown cervical myelopathy for about 3 month was diagnosed as having cervical dysplasia on the basis of plain x-ray and CT scanning. In addition, atlanto-axial instability causing clinical symptoms was detected. A dorsal stabilisation was perfomed using the DCS System.
    Notes: Zusammenfassung Bei der fibrösen Dysplasie finden sich in seltenen Fällen, sowohl bei der mono- als auch polyostotischen Form, eine Beteiligung der Halswirbelsäule (HWS). In der Mehrzahl handelt es sich um Zufallsbefunde im Rahmen der radiologischen Diagnostik. Es wird ein symptomatischer Fall mit atlantoaxialer Instabilität bei Befall des 1., 2. und 3. Halswirbelkörpers vorgestellt. Im Fall einer 53 jährigen Patientin, bei der seit 3 Monaten eine zervikale Myelopathie vorlag, wurde anhand von Nativröntgenaufnahmen und CT-Untersuchung eine zervikale fibröse Dysplasie diagnostiziert. Als Ursache der Symptomatik lag eine atlantoaxiale Instabilität vor. Es erfolgte eine dorsale Stabilisierung mit dem DCS-System.

    Notes: Summary The effect of aconitine (10−5–10−6 g/ml) on membrane potentials and membrane currents of myelinated nerve fibres of Xenopus laevis was investigated. The following observations were made: a) Current clamp conditions: Slow depolarization (10–15 mV), decrease of amplitude and maximum rate of rise of action potential, finally inexcitability. With inward current pulses ‘hyperpolarizing responses’ could be elicited at membrane potentials more negative than the resting potential (E r ). Neither spontaneous activity nor repetitive responses to electrical stimuli were observed. No effects of aconitine were found in Na-free solutions or in the presence of tetrodotoxin. b) Voltage clamp conditions: Development of steady inward current at normal resting potential due to formation of a non-inactivating sodium permeability; heavily poisoned nodes therefore exhibit an N-shaped steady-state current voltage relation with negative slope at membrane potentials more negative thanE r . These non-inactivating sodium channels open more slowly than normal sodium channels, and can only be closed by hyperpolarizing the membrane by about 50 mV. The majority of sodium channels have almost normalτ m; theirm ∞ andh ∞-V relations are shifted by 10–15 mV towardsE r . It is concluded that these changes of the sodium permeability account for the changes of electrical activity observed after treatment with aconitine.

    Description / Table of Contents: Zusammenfassung Anhand der Beobachtung von 2 Fällen mit Desmoid-Fibromen der Gliedmaßen wird einleitend eine Darstellung dieser Tumoren anhand der Literaturberichte gegeben. Die Tumoren sitzen vorwiegend am Stamm; der Gliedmaßenbefall ist selten. Die beiden Beobachtungen betrafen in typischer Weise Jugendliche mit Befall von Unterarm und Oberschenkel bei ähnlicher Verlaufsweise. Im ersten Fall konnte nur durch 7 operative Eingriffe mit Opferung des Nervus medianus und Nervus ulnaris Rezidivfreiheit erreicht werden. Im zweiten Fall handelt es sich um einen solitär und schnell wachsenden großen Tumor mit Ummauerung des Nervus ischiadicus, der ebenfalls deutliche Rezidivneigung zeigte.
    Notes: Summary Two cases of rare desmoid fibromas are being reported, localised in the extremities. After a literature review of these tumors, the clinical characteristics are pointed out and difficulties in therapy discussed. The tumors occasionally appear on the forearm or the thigh of juvenile patients and show a similar development. In the first case, 7 operations with resection of the n. medianus and n. ulnaris were necessary to prevent a recurrence. In the second case, we saw a solitary and rapidly growing tumor, surrounding the sciatic nerve with a great tendency to recur.

    Description / Table of Contents: Zusammenfassung Bei je 75 Kindern und Erwachsenen wurde die alkalische Phosphataseaktivität im Serum nach Bewegungs- und Belastungseinschränkungen der unteren Extremitäten bestimmt. Es zeigte sich bei den Kindern ein deutlicher Abfall der alkalischen Serumphosphataseaktivität in Abhängigkeit vom Grad der Bewegungseinschränkung, im Gegensatz zu den Erwachsenen, bei denen keine Änderung der Enzymaktivität zu beobachten war.
    Notes: Summary Alcaline phosphatase activities in the sera from 75 children of the age of 2–9 years were determined after immobilisation of various degree. A significant decrease of enzymatic activity was to be seen with increasing immobilisation, in contrast to the adult, where the immobilisation had no influence on the alcaline serumphosphatase.

Es wird nach Darstellung der allgemeinen klinischen Symptomatik und deren Besonderheiten dieses Erkrankungsbildes im Erwachsenenalter das klinische Bild sowie die durchgeführte Behandlung dieser ungewöhnlich früh aufgetretenen Erkrankung berichtet.
    Notes: Summary It is reported on a seldom case of psoriasisarthritis in early childhood. The onset of skinlesion firstly occurred at the age of one month. The clinical symptoms of psoriasis advantage are firstly described and compared with the symptoms in our case. It was an unusual early beginning.
