Résumé Les résultats publiés au sujet de l'ostéotomie intertrochantérienne du fémur dans les cas de dysplasie de la hanche, concernent en général un jugement global des forme et fonction définitives de la hanche d'après des schémas complexes. Ces criterès ne permettent pas cependant d'établir une statistique valable sur le comportement postopératoire des composants morpholiques de la dysplasie. Dans le but d'une telle statistique nous avons procédé à un examen radiologique exact de 100 cas de dysplasie de la hanche après osteotomie intertrochantérienne et nous avons classé nos résultats suivant différents critéres. 1. L'antéversion du col du femur récidive, après correction prêsque totale, dans environ un quart des cas; elle comporte jusqu'à 30 degrés. Dans 14% l'antéversion est supérieure à 40 degrés. 2. Aucune correlation n'a pu être établie entre récidive de l'antéversion et de la coxa valga. 3. Conformeiment au délai d'observation postopératoire nous avons constaté qu' une récidive d'antéversion peut survenir assez rapidement, que sa fréquence est majeure au cours de la 3 ème année postopératoire pour diminner par la suite. 4. Au sujét de l'âge nous avons constaté que les opérations précoces jusqu'à l'âge de 4 ans n'ont qu'une tendance faible à la rechute. 5. Contrairement à d'autres affirmations, nous avons pu démontrer que la médialisation du fragment distal du fémur ne peut pallier à une récidive d'antéversion. 6. L'angle d'antéversion obtenu ne dépend pas en général, de la formation du toit du cotyle et du centrage de l'articulation. De mauvaises conditions réalisées au niveau du toit du cotyle et un centrage inexact coincident cependant de façon évident avec la réapparition de l'antéversion. 7. La plus importante conclusion résultante de notre étude est de procéder au centrage primaire profond de la tête fémorale et à la réalisation de bonnes conditions au niveau du toit du cotyle.
Abstract:
Zusammenfassung Die in der Literatur niedergelegten Ergebnisse der intertrochanteren Femurosteotomie bei der Hüftdysplasie stellen meistens Gesamtbeurteilungen der resultierenden Hüftform und -funktion nach komplexen Bewegungsschemata dar, die über das postoperative Verhalten der morphologischen Teilkomponenten der Hüftdysplasie keine spezielle statistische Aussage zulassen. Zur speziellen Ermittlung der postoperativen Entwicklung der Torsionsstellung des Schenkelhalses wurden 100 unausgewählte Dysplasiehüften nach vorausgegangener intertrochanterer Osteotomie einer exakten röntgenologischen Nachuntersuchung unterzogen und die Untersuchungsergebnisse in mehrfacher Hinsicht aufgeschlüsselt. Als Ergebnis kann herausgestellt werden: 1. Die bei der Operation nahezu vollständig ausgeglichene Drehfehlstellung des Schenkelhalses rezidiviert in etwa einem Viertel der Fälle mit Werten über 30°; bei 14% liegen die Werte mit mehr als 40° Antetorsion eindeutig wieder im pathologischen Bereich. 2. Eine Korrelation zwischen Antetorsions- und Valgusrezidiv ließ sich nicht feststellen. 3. In Abhängigkeit von der postoperativen Beobachtungszeit wurde mitgeteilt, daß ein Antetorsionsrezidiv relativ rasch entstehen kann, die Rezidive im 3. postoperativen Jahr am häufigsten und stärksten sind und in der Folgezeit wieder abnehmen. 4. Bezüglich des Operationsalters war festzustellen, daß die Frühoperationen bis zum 4. Lebensjahr nur eine geringe Rezidivneigung zeigen. 5. Entgegen anderen Behauptungen konnte gezeigt werden, daß die Medialisierung des distalen Femurfragmentes bei der Operation ein Antetorsionsrezidiv nicht verhüten kann. 6. Eine Abhängigkeit des resultierenden Antetorsionswinkels von der Pfannendachbildung und Gelenkzentrierung besteht im allgemeinen nicht. Schlechte Pfannendachverhältnisse und Gelenkzentrierungen sind jedoch in auffälliger Weise mit Antetorsionsrezidiven korreliert. 7. Als wichtigste klinische Folgerung wird zur Verhütung eines Antetorsionsrezidivs eine primäre tiefe Hüftkopfeinstellung und Schaffung guter Pfannendachverhältnisse gefordert.

Summary A review in the literature on results of the intertrochanteric osteotomy of femur with hip dysplasia represents in most parts an appraisal on the whole of resulting hip form and function according to complex patterns of motion, where the statistical analysis is not revealing in detail about the postoperative morphological behavior of the single components of hip dysplasia. Therefore 100 unselected dysplastic hips with preceeding intertrochanteric osteotomy underwent follow-up examination with exact x-ray films, and the results were decoded in multiple aspects. The results can be cited as follows: 1. The surgically almost completely corrected antetorsion of the neck of femur recurs in approximately one fourth of the cases with angles more than 30 degrees; in 14% of the cases were more than 40 degrees antetorsion. 2. A correlation between recurrence of antetorsion and recurrence of the valgus could not be found. 3. In dependance to the postoperative follow-up time, it was noted that recurrence of the antetorsion can develop relatively soon; the recurrence rat ist most frequent in the 3rd postoperative year and decrease in the following time. 4. Concerning the age of operation, it was noted that early surgery up to the 4th year of life antetorsion had only a little tendency for recurrence. 5. Contrary to other statements, we were able to demonstrate that medial displacement of the distal femur fragment can not prevent recurrence of antetorsion. 6. A dependance of the resulting angle of antetorsion to the formation of the acetabular roof and centering of the joint does in general not exist. A bad shelf and centering of joint are particularly correlated to recurrence of antetorsion. 7. The most useful clinical conclusion for preventing an antetorsion recurrence is to aim at a primary deep fitting of the femoral head in the acetabulum and creating an optimal form of the acetabular roof.
