    Notes: Abstract Magnetic resonance imaging of the eye usually includes T2-weighted images both for screening purposes and for characterization of melanoma. Conventional T2-weighted spin-echo (SE) imaging suffers both from long acquisition times and incomplete recovery of the vitreous' signal. A fast SE sequence was therefore compared prospectively with conventional sequences in 29 consecutive patients with lesions of the eye. Fast SE images delineated melanoma and other lesions of the eye from vitreous better than conventional T2-weighted images. Image quality and lesion conspicuity were improved on the fast sequence. Whereas melanoma appeared hypointense to vitreous on both types of images, subretinal effusion was hypointense on fast images and hyperintense on conventional T2-weighted images. Ghosting of the globe, which, however, did not decrease diagnostic value, was more pronounced on fast images. Conventional T2-weighted images may be replaced by fast SE images in MR studies of the eye with a gain in lesion conspicuity and significant time saving.

    Notes: Summary Serotonin (5-HT) and 5-hydroxyindolacetic acid (5-HIAA) were determined in seven brain regions of semistarved and control male rats. After semistarvation on a high carbohydrate diet serotonin turnover, as indicated by 5-HIAA/5-HT ratio, was increased in the total brain and several regions both three and 24 hours after the last meal. In contrast, after semistarvation on a high-protein diet serotonin turnover was decreased three hours after ingestion of the final meal, but increased 24 hours thereafter. Compulsary running wheel activity for one hour did not influence diet inducet changes in serotonin turnover. Alterations in plasma corticosterone during semistarvation were not related to changes in central serotonin turnover. Data suggest that the interaction of caloric restriction and diet composition determines serotonin turnover during semistarvation.

    Notes: Summary Influence of diet composition on mood during weight-reducing diets was studied in healthy young women of normal weight. A broad range of macronutrient intake was achieved by means of divergent dietary instructions for the composition of a 1,000 kcal per day diet adhered to for six weeks. Global mood during the last three weeks of the diet was significantly better in the “vegetarian” than in the “mixed” diet group. During this time a significant correlation was observed between relative carbohydrate intake and global mood (r=−0.74; p〈0.01) and between the ratio of plasma tryptophan to other large neutral amino acids (a predictor of tryptophan flow into brain) and global mood (r=− 0.52; p〈0.05). Results suggest that group differences are related to differences in carbohydrate intake. It is hypothesized that impairment of central serotonergic function due to reduced tryptophan availability can prompt mood deterioration in situations of relatively low carbohydrate intake.

    Description / Table of Contents: Abstract The brain is an important target organ for gonadal hormones. Estrogens have protective and trophic effects on neural systems and modulate the activity of several neurotransmitter systems. Initial studies support the hypothesis that estrogen therapy in menopause may have protective effects toward the development of dementia, particularly Alzheimer's disease. Furthermore, estrogens may improve verbal fluency, memory, and mood in elderly women. Still, the available evidence is not sufficient to support general treatment recommendations with respect to estrogens and the therapeutic targets dementia and depression.
    Notes: Zusammenfassung Das Gehirn ist ein wichtiges Zielorgan für Gonadenhormone. Östrogene haben neuroprotektive und neurotrophe Effekte und beeinflussen die Aktivität verschiedener Neurotransmittersysteme. Erste Studien unterstützen teilweise die Hypothese, dass Östrogene in der Postmenopause eine Prophylaxe dementieller Erkrankungen insbesondere der Alzheimer Erkrankung, darstellen können. Östrogene verbessern unter bestimmten Bedingungen verbale Fähigkeiten und Gedächtnis im Alter und heben Stimmung und Befinden an. Für allgemeine Therapieempfehlungen hinsichtlich der Indikationen Demenzprophylaxe, Demenztherapie und Depression sind die vorliegenden Evidenzen aber noch nicht ausreichend.

    Description / Table of Contents: Abstract. Purpose: Evaluation of quality of life data and patient satisfaction to estimate the outcome of laparoscopic antireflux surgery (LARS) is nowadays an important issue, the long-term outcome of this has not yet received much attention. Methods: In the present study we evaluated the outcome of quality of life data of 70 patients who underwent “floppy” Nissen fundoplication at our institute 3 years after surgery. Quality of life was evaluated with the Gastrointestinal Quality of Life Index (GIQLI). Additionally the subjectivity and objectivity of the quality of the procedure and possible side effects were evaluated with a questionnaire. Results: Three years after laparoscopic Nissen fundoplication, patients gave their quality of life (GIQLI) in an overall score of 123.9 points. This is comparable to 122.6 points in the normal population. There was no difference detectable in the subdimensions of GIQLI. Ninety-eight percent of the patients estimated their satisfaction with the procedure as excellent or good and would undergo surgery again if necessary. Four patients suffered from minimal side effects from the procedure, but had no decrease in their quality of life. None of the patients needed antireflux medications postoperatively. Laparoscopic redo-fundoplication was performed in two patients 3 months after initial surgery because of persisting dysphagia. Conclusion: The efficacy and long-term outcome of treatment of gastroesophageal reflux disease with laparoscopic “floppy” Nissen fundoplication can be evaluated by objective testing, but also by subjective judgment of the patient and with an evaluation of quality of life.
    Notes: Zusammenfassung. Ziel: Der Evaluierung von Lebensqualitätsdaten sowie der subjektiven Patientenzufriedenheit zur Beurteilung der laparoskopischen Antirefluxchirurgie wird in den letzten Jahren zunehmend an Bedeutung beigemessen. Gefordert werden in der Literatur vor allem Langzeitergebnisse prospektiv durchgeführter Studien. Methode: Die vorliegende Arbeit präsentiert die Ergebnisse nach laparoskopischer „floppy Nissen“-Fundoplicatio bei 70 Patienten anhand von Lebensqualitätsdaten 3 Jahre nach der Operation. Die Erhebung der Lebensqualität erfolgte mit dem Gastrointestinalen Lebensqualitätsindex (GLQI). Zusätzlich wurden anhand eines standardisierten Fragebogens subjektive und objektive Qualitätsmerkmale des Eingriffs sowie potentielle Nebeneffekte der Operation evaluiert. Ergebnisse: Drei Jahre nach laparoskopischer Nissen-Fundoplicatio wurde ein Gesamtscore des GLQI von 123,9 Punkten erhoben, welcher vergleichbar zu einer entsprechend gesunden Referenzpopulation (122,6 Punkte) ist. Die einzelnen Subdimensionen des GLQI verzeichnen entsprechende Punktescores. Über 98 % aller Patienten beurteilen den Erfolg der Operation mit sehr gut oder gut und würden sich gegebenenfalls einer neuerlichen Antirefluxoperation unterziehen. Anhaltende, jedoch die Lebensqualität nicht beeinträchtigende Nebeneffekte der Operation wurden von 4 Patienten angegeben. Kein Patient benötigte eine erneute medikamentöse Antirefluxtherapie. Eine laparoskopische Refundoplicatio war aufgrund einer persistierenden Dysphagie in 2 Fällen innerhalb der ersten 3 postoperativen Monate notwendig. Schlußfolgerung: Die Effektivität der chirurgischen Therapie der gastro-oesophagealen Refluxkrankheit mittels laparoskopischer „floppy-Nissen“-Fundoplicatio läßt sich nicht nur durch objektive Befunde, sondern auch anhand der subjektiven Einschätzung des Patienten und durch die Messung von Lebensqualitätsdaten dokumentieren und längerfristig belegen.
