    Notes: Summary Ten of 159 patients showed arthritic symptoms during the course of BCG immunotherapy for advanced cancer. The arthritic symptoms occurring after BCG injections had the following characteristics: (1) The incidence of arthritis was closely correlated with the host immunologic responsiveness to BCG; (2) These symptoms usually occurred 1–5 months after the first BCG injection (7/10); (3) The arthritic symptoms usually started with morning stiffness (9/10), which was followed by acute inflammatory signs in the affected joints. They gradually subsided in response to treatment with nonsteroid antiinflammatory drugs, but were not completely cured while the effectiveness of BCG continued; (4) The symptoms were aggravated by additional BCG injections (8/10). (5) This form of arthritis could be differentiated from rheumatoid arthritis, tuberculous, or purulent arthritis by its clinical course and by roentgenograms of the affected joints. It is thought to be induced by the adjuvant effect of BCG, and is a new side effect of BCG immunotherapy.

    Description / Table of Contents: Zusammenfassung Es wird über 22 Fälle von “InterSterno-Costo-Clavikulärer Ossifikation” berichtet. Die klinischen und pathologisch-anatomischen Befunde zeigen, daß die abnorme Ossifikation durch tine nichteitrige, chronische Entzündung im Bereiche der Sterno-Costo-Clavikular-Region und des costo-clavikulären Ligamentes hervorgerufen wird. Wir klassifizieren die Röntgenbefunde in 3 Stadien, entsprechend dem Grad der Verknöcherung: lokalisiert, generalisiert und hyperostotisch. In dieser Reihenfolge schreitet auch die Erkrankung fort. Eine betrdchtliche Zahl der Patienten zeigten auch abnorme Röntgenbefunde an der Wirbelsäule oder den Sacroiliacalgelenken. Eine häufige Begleiterscheinung der Erkrankung war auch eine Pustolosis palmaris et plantaris. Die meisten Fälle wurden erfolgreich mit entzündungshemmenden Medikamenten behandelt; bei einigen Patienten wurde eine Besserung der Beschwerden erst nach chirurgischer Entfernung der verknöcherten Masse, einschließlich Clavikula oder 1. Rippe, erzielt.
    Notes: Summary We present 22 cases with inter-sterno-costoclavicular ossification. Clinical and pathological findings show that abnormal ossification observed in this situation is due to non-suppurative chronic inflammation of the soft tissues around the sterno-costo-clavicular region such as the costo-clavicular ligament. We have classified X-ray findings into three stages according to the extent of the ossification; localized, generalized, and hyperostotic, and show that the disease progressed in this sequence. A considerable number of the patients showed abnormal X-ray findings in the spine or the sacro-iliac joint. Frequent association of pustulosis palmaris et plantaris was noted in this disease. Most of the cases were treated effectively with anti-inflammatory drugs, but a few cases required surgical resection of the ossified mass with the clavicle or the first rib in order to relieve the severe pain.

    Description / Table of Contents: Zusammenfassung 6 jugendliche Patienten mit ankylosierender Spondylitis werden untersucht. Die klinischen Beschwerden bestanden hauptsächlich in einer Polyarthralgie bzw. Coxalgie, aber ohne Schmerzen und Einsteifung im Bereich der Lendenwirbelsäule. Diese klinischen Bilder waren atypisch für ankylosierende Spondylitis und wiesen eher auf eine periphere Arthropathie im Sinne einer juvenilen rheumatoiden Arthritis hin. Nach langfristiger Beobachtung ergaben sich jedoch bei all diesen Patienten eindeutige Hinweise auf ankylosierende Spondylitis. Die HLA-B27 Antigene waren in 4 von 6 Fällen positiv. Diese Häufigkeit entspricht in etwa der bei den Erwachsenen ankylosierenden Spondylitis. HLA-Bestimmung erscheint daher für die Frühdiagnose sehr wichtig.
    Notes: Summary We have experienced six patients of juvenile ankylosing spondylitis. Their chief complaints were polyarthralgia or coxalgia, however, any pain or stiffness in lumbar regions was absent. These clinical features were atypical for those of ankylosing spondylitis, and much more like those of a peripheral arthropathy such as juvenile rheumatoid arthritis. After a long-term follow-up, all the patients showed definite clinical features of ankylosing spondylitis. HLA-B27 was found in 4 out of 6 cases. This frequency is almost equal to that of adult ankylosing spondylitis patients. HLA typing was very important in the early diagnosis.

    Description / Table of Contents: Zusammenfassung Wir berichten über die klinischen und röntgenologischen Krankheitsbilder der ankylosierenden Hyperostose der Wirbelsäule bei 70 Fällen. Rückenschmerzen in verschiedenen Bereichen wurden bei 49 Fällen beobachtet. HLA-Bestimmungen zeigten keine signifikanten Abweichungen. Röntgenologisch chrakteristische Befunde waren Osteophytenbzw. Brückenbildungen der Brustwirbelsäule, die auf dem rechten und vorderen Ted des Wirbelkörpers, besonders im kaudalen Anteil auffällig zu erkennen waren. Verknöcherung des hinteren Längsbandes wurde bei 21 Fällen beobachtet.
    Notes: Summary The clinical and radiological features of 70 patients with ankylosing spinal hyperostosis are presented. Pain in some region of the back was found in 49 patients. No significant deviations were found in HLA typing. Radiologically, the most characteristic features were bony spurs or bridges in the dorsal spine, where they predominated on the right and anterior aspects of the spine, especially in the lower dorsal region. Ossification of the posterior longitudinal ligament was found in 21 patients.
