Summary 4 to 6 years ago, 7 patients with pronounced maxillary protrusion were treated by a combination of surgery and orthodontics. On 5 of them it was possible to do a late followup study. For the broadening of the apical base, autogenous bone (either rib or cancellous bone) had been used. By means of radiographic studies the incorporation of the bone has been shown to occur. During the retrusion of the upper anterior teeth the apices grew either into the transplanted bone or they were covered by transplanted bone. Autogenous rib and cancellous bone from the iliac crest are usable for transplantation into the maxillary base, but cancellous bone is preferable. No adverse changes were found in the periodontium or the vitality of the teeth. Nor was there any recurrence of maxillary protrusion.
Abstract:
Résumé 7 patients présentant une très forte protrusion du maxillaire supérieur subirent il y a 4 à 6 ans un traitement combiné chirurgical et orthopédique. 5 patients furent être contrôlés ultérieurement. L'élargissement de la base apicale du maxillaire fut obtenue par des autogreffes de côtes ou de spongieuse. Chez tous les patients contrôlés, des tomographies prouvèrent le succès des greffes. Dans le traitement conservateur par rétraction du bloc incisif, les extrèmités radiculaires migrent vers l'os transplanté ou s'y incorporent même. Des transplants autologues de côtes ou de spongieuse sont particulièrement indiqués. Nous donnons notre préférence aux greffes d'os spongieux. Des complications au niveau de l'appareil fixateur des dents ou de leur vitalité n'ont pas été observées. On ne déplore pas non plus de récidive.

Zusammenfassung 7 Patienten mit extremer Oberkieferprotrusion wurden vor 4 bis 6 Jahren kombiniert chirurgisch-orthopädisch behandelt, wobei 5 Patienten nachuntersucht werden konnten. Zur Verbreiterung der apikalen Basis wurden autologe Rippen- bzw. Spongiosatransplantate verwendet. Bei allen untersuchten Patienten konnte die Inkorporierung des Knochens anhand von Tomogrammen nachgewiesen werden. Bei der konservativen Retrusion der Oberkieferfront wandern die Wurzelspitzen entweder in den transplantierten Knochen ein oder werden von diesem bedeckt. Autologe Rippenspäne sind für die Vorpflanzung gut geeignet, wobei wir die Verwendung von bone chips vorziehen. Nachteilige Folgen für den Zahnhalteapparat oder die Vitalität der Zähne sind nicht beobachtet worden. Ebenfalls fanden sich keine Rezidive.
