    Notes: Summary Cytostatic-treated persons and cases of severe hepatic failure under corticoid therapy are predisposed to disseminating Aspergillus infections. Constant exposure to Aspergillus spores may result in a fatal Aspergillus infection. The triad of hepatic failure, corticoid therapy and constant exposure to Aspergillus spores is described in a 70-year-old female patient. A painless icterus was clinically diagnosed as non-A non-B hepatitis, with a protracted cholestatic course. She had been treated with an oral corticoid preparation. After leaving the hospital at her own insistance when still in the icteric stage, severe pneumonia due to Aspergillus developed within 14 days; this was confirmed radiologically. The autopsy results showed unexpected infarction, similar to pneumonic foci, in all lobes and dissemination in the myocardium, stomach, kidneys and brain. The liver showed subacute dystrophy. Constant exposure to the conidia of A fumigatus came about as a result of the soil of potted ornamental plants in the patient's living room. The fungus could only be successfully cultured by putting infected tissue particles on Sabouraud dextrose agar; it was not possible by the common method of fractionated streaking.

    Notes: Summary Kidney failure and pneumonia byAspergillus flavus andA. fumigatus were found in a 56-year-old woman who had received antibiotic and corticoid treatment to control high fever. Her bloody tracheal secretion was a suspension of granule-like spore-free colonies of bothAspergillus species. Hemorrhages in mucous membranes and skin suggested a hematogenous dissemination of the fungi. Aspergillus spores in the soil of ornamental plants were assumed to be responsible for the inhalatory infection. The kidney function normalized rapidly under treatment by amphotericin B plus flucytosine and hemodialysis performed eight times. After 29 days of antimycotic treatment (amphotericin B 463 mg, flucytosine 150 g), besides normalization of the kidney function, healing of the penumonia and bleeding from skin and mucal membranes took place. One and a half years later kidney function and blood parameters were found to be normal. In cases ofAspergillus pneumonia and kidney failure, a combined treatment by hemodialysis and amphotericin B plus flucytosine is recommended. In addition, there is discussion of the general importance of uremia and its influence on the mycotic infection.

    Notes: Summary The risk for AIDS patients from Cryptococcus neoformans is outlined on the basis of a case report on a 28-year old male patient whose disease was complicated by cryptococcosis. Beside the description of the diagnosis of cryptococcosis (demonstration of the agent and its antigen), epidemiological associations (habitat of Cr. neoformans in fecal matter of birds) and the clinically, mostly not recognized, route of infection via the lungs is stressed. The effective therapy with the combination of amphotericin B and 5-flucytosine, which also in this case has been successful is described from the clinical and microbiological angles. Finally, proposals for the prevention of Cr. neoformans infections in AIDS patients and for a special mycological surveillance directed at this fungus are made.

    Notes: Summary A 40-year old homosexual AIDS patient recovering from a Pneumocystis carinii pneumonia developed a Cryptococcus neoformans infection with involvement of the central nervous system (CNS) which could be treated successfully with amphotericin B and flucytosine. After a symptomfree interval of 4 1/2 months, a new acute fatal disease of the CNS did not reveal a cryptococcosis relapse but a necrotizing Toxoplasma encephalitis, a cytomegalovirus infection and striking cultural findings of Staphylococcus aureus in all organs examined. Neither by culture nor by histology Cr. neoformans could be detected in the CNS or in the other organs examined. The temporal course of the Cr. neoformans infection and its specific diagnosis are commented. It is demonstrated that (during or after successful therapy of Pneumocystis carinii pneumonia) a specific cultural examination of specimens from the respiratory tract for Cr. neoformans is needed, in order to recognize a Cr. neoformans infection in its primary stage, i. e. before hematogenous dissemination of Cr. neoformans leading to the secondary stage of the infection.

Bei der sogenannten N-Assimilationsprüfung nach dem Benhamschen Platten-Ausstrichverfahren unter Verwendung des Wickerhamschen Grundsubstrates (modifiziert nach H.Seeliger) kommt es bei Zusatz von Kanarienvogel (Serinus canaria)-Kotpulver (1%), als einziger N-Quelle, zu einer intensiven Braunfärbung der sich hierauf Üppig entwickelnden Cr. neoformans-Kolonien. 2. Dieser Farbeffekt konnte ebenfalls bei Verwendung von Kotpulver von Dompfaff (Pyrrhula pyrrhula) und Stieglitz (Carduelis carduelis) beobachtet werden, nicht dagegen bei Verwendung von Kotpulver von Taube (Columba livia), Wellensittich (Melopsittacus undulatus) und Kormoran (Phalacrocorax carbo). 3. Es wurde festgestellt, daß es sich bei der Futterzusammenstellung für Kanarienvogel, Stieglitz und Dompfaff um dieselben Pflanzensamen handelt und zwar: Hanfsamen (Cannabis sativa), Leinsamen (Linum sativum), Rübsen (Brassica rapa), Kanariensaat (Phalaris canariensis), Mohn (Papaver somniferum), geschälter Hafer (Avena sativa) und Negersaat (Guizotia abyssinica). 4. Zusatz der pulverisierten bzw. gequetschten oben aufgeführten Pflanzensamen zum N-Assimilations-Substrat als einziger N-Quelle ergab nur bei Negersaat-Zusatz eine intensive Braunfärbung der sich hierauf entwickelnden Cr. neoformans-Kolonien. 5a. Auf N-Assimilations-Grundsubstrat, mit Zusatz von zerkleinerter Negersaat oder wäßrigem Negersaat-Filtrat als einziger N-Quelle, kam es zurBraunfärbung von insgesamt 23 Cr. neoformans-Stämmen verschiedenster Herkunft. 5b. Keine Braunfärbung der Kolonien wurde beobachtet bei insgesamt 26 Stämmen der Übrigen Cryptococcus-Arten wie Cr.albidus, Cr.diffluens Cr. laurentii, Cr. luteolus, Cr. gastricus und Cr. terreus. 5c. Ebenfalls konnte keine Braunfärbung festgestellt werden bei insgesamt elf Stämmen (bzw. Arten) der Gattungen Candida, Torulopsis, Rhodotorula, Trichosporon und Lipomyces. 6. Der nach bisherigen Untersuchungen nur mit Cr. neoformans farbreagierende Stoff von Negersaat ist wasserlöslich und unempfindlich gegenüber Temperaturen von 110

    Notes: Summary Strains of Debaryomyces hansenii, Deb. kloeckeri, Deb. subglobosus, Deb. nicotianae and their imperfect forms Torulopsis famata and T. candida were found to be able to use creatine and creatinine as a sole source of nitrogen. Most strains could assimilate creatine when tested by the auxanographic method, while creatinine was only assimilated in liquid medium. In several instances the reaction with creatine in the auxanogram was positive, and in the liquid medium negative. We have considered the taxonomic value of the utilization of creatine and creatine in the first place for distinction between the four Debaryomyces species, and in the second place for the differentiation of the four species as a group from other species.

    Notes: Zusammenfassung 1. Ein aus Kanarienvogelkot gezüchteter Cr. neoformans-Stamm (W 71) blieb in trockenem Flußsand und Gartenerde nach einer Aufbewahrungszeit von 480 Tagen (bei Zimmertemperatur) noch wachstumsfähig. Auch in trockenem Kanarienvogelkot (Aufbewahrungszeit420Tage) und an trockenem Vogelfutter (Aufbewahrungszeit 240 Tage) blieb derselbe Stamm wachstumsfähig. 2. Zu Vergleichszwecken wurden Stämme folgender Sproßpilzarten auf ihre Widerstandsfähigkeit gegen Austrocknung geprüft: Candida albicans, C. tropicalis, C. krusei, Trichosporon cutaneum, Rhodotorula mucilaginosa, Cryptococcus albidus, Cr. diffluens, Cr. gastricus und Cr. terreus. Nach 244tägiger Aufbewahrungszeit in trockenem Flußsand waren noch wachstumsfähig: Rhodotorula mucilaginosa, Cryptococcus albidus, Cr. diffluens und Cr. terreus. 3. Aufschwemmungen von trockenem Flußsand, der vor 1 Jahr mit Cr. neoformans infiziert wurde, führten nach i.m. Verabreichung bei der weißen Maus zu generalisierten tödlichen Crytokokkosen und zu Muskelzerstörungen, wie sie an anderer Stelle beschrieben wurden (Staib 1962b). 4. In feuchtem Flußsand war Cr. neoformans bei einer Temperatur von 50°C nach 24 Std abgetötet. Im trockenen Sand dagegen (der vor 1/12 Jahren mit Cr. neoformans infiziert wurde) fanden sich nach 24stündiger Erhitzung auf 80°C am Großteil der Sandkörner noch koloniebildungsfähige Cr. neoformans-Zellen. Die Hitzetoleranz der an Trockenheit gewöhnten Cryptokokken lag so hoch, daß nach dreistündiger Erhitzung auf 100°C und nach einstündiger Erhitzung auf 120°C noch vereinzelte koloniebildungsfähige Cryptokokken nachweisbar waren. Nach zweistündiger Erhitzung auf 120°C war kein Wachstum mehr festzustellen. Junge, nicht an Trockenheit gewöhnte Kulturen desselben Stammes verfügen nicht über diese hohe Hitzetoleranz.

    Notes: Abstract A favourable outcome was observed in a 12 year-old boy who developed invasive pulmonary and cerebral aspergillosis during antineoplastic treatment for central nervous system relapse of acute lymphoblastic leukemia. Combination therapy with amphotericin B and 5-Fluorocytosine led to complete regression of pulmonary infiltrates. Despite enlargement of the cerebral lesion monitored by computerized tomography, no viable fungi were found in the completely resected abscess after a 4 weeks' course of antifungal treatment preceeding neurosurgery. Histological examination confirmed the diagnosis of an aspergillotic abscess. The initially severe neurological symptoms disappeared after successful surgery. Aspergillus fumigatus was detected in the soil of a potted ornamental plant in the mother's living room, suggesting that this might have been the source of the infectious agent.
