    Description / Table of Contents: Summary We report on dermatomyositis-like adverse cutaneous reactions following long-term maintenance therapy with hydroxyurea in two patients suffering from chronic myelogenous leukaemia (CML). In addition to non-specific side effects, such as xerosis, pruritus and hyperpigmentation, both patients presented with more specific skin changes, i.e. erythematous lesions, scaling, and partially atrophic areas distributed in a linear fashion on the dorsal aspects of the hands and fingers. In addition, teleangiectatic erythema of the face was present in both patients, and this was associated with oedema of the eyelids in one patient. Despite these dermatomyositis-like features there were no clinical signs of muscular involvement, and muscle-specific enzymes were within normal ranges. Skin biopsy specimens revealed an interface dermatitis characterized by a lichenoid cell infiltrate, vacuolar alteration of basal cells, necrotic keratinocytes within the spinous zone, focal hypergranulosis, orthohyperkeratosis and telangiectases in the upper part of the dermis. Analogous histopathological findings have been documented in lichen planus-like skin changes on the hands following hydroxyurea therapy. It seems doubtful whether there are actually any major differences between those skin changes described as dermatomyositis-like and those interpreted as lichen planus-like in patients receiving long-term hydroxyurea therapy.
    Notes: Zusammenfassung Zwei Patienten mit chronischer myeloischer Leukämie (CML) entwickelten während einer Hydroxyurea (Litalir ® )-Langzeittherapie dermatomyositisartige Veränderungen an den Händen. Beide Patienten zeigten neben unspezifischen Hautveränderungen, wie sie auch nach anderen Zytostatika vorkommen, so Hauttrockenheit, Pruritus, Hyperpigmentierungen etc., für Hydroxyurea spezifischere Befunde über Handrücken und Fingerstreckseiten, nämlich bandförmig ausgerichtete, unscharf begrenzte, schuppende Erytheme und hyperpigmentierte atrophische Areale mit Betonung über den Fingergelenken. Beide Patienten wiesen zudem teleangiektatische Erytheme im Gesicht auf, bei einem Patienten bestand ein periorbitales Ödem. Trotz dieses dermatomyositisartigen Aspektes lagen die Kreatinkinase-Werte im Normbereich, auch waren keine Muskelbeschwerden aufgetreten. Histologisch lag eine „Interface Dermatitis“ zugrunde mit lichenoidem Infiltrat, vakuolärer Alteration der Basalzellen, nekrotischen Keratinozyten in suprabasaler Lokalisation, Hypergranulose, Orthohyperkeratose und Teleangiektasien in der oberen Dermishälfte.

    Description / Table of Contents: Summary In this review we focus on the diagnostic importance of antinuclear antibodies (ANA), the biological function of the relevant autoantigens and on some methodological questions regarding the detection of ANA. The qualitative and quantitative evaluation of ANA has improved significantly in recent years with the introduction of several new test kits. A precondition for the rational use of those assays is knowledge of the diagnostic validity of the detected ANA with regard to the method used. ANA are autoantibodies that react with nucleic acids, protein-nucleic acid complexes and proteins of nuclei. The reasons for their in vivo production are unknown. ANA characterize several of the so-called connective tissue autoimmune diseases and their subtypes, either alone or in typical combinations. They differ significantly with regard to their prevalence and thus in their diagnostic validity. Specificity and prevalence do not correlate. ANA are not markers of disease activity except for antibodies against dsDNA. ANA levels can be interpreted correctly only in connection with clinical symptoms and other laboratory findings.
    Notes: Zusammenfassung Die Arbeit soll einen aktuellen Überblick über die diagnostische Bedeutung von antinukleären Antikörpern (ANA), die biologische Funktion der jeweiligen Autoantigene sowie über einige methodische Fragen des ANA-Nachweises geben. Mit der Einführung zahlreicher neuer Testbestecke haben sich die Möglichkeiten zur qualitativen und quantitativen Bestimmung von ANA in den letzten Jahren erheblich erweitert. Eine Voraussetzung für einen rationalen Einsatz dieser Assays besteht in der Kenntnis der diagnostischen Validität des nachzuweisenden ANA unter Berücksichtigung der jeweiligen Methode. Die entsprechende Literatur zeigt jedoch viele Widersprüche. ANA sind Autoantikörper, die mit Proteinen, Nukleinsäuren und Protein-Nukleinsäure-Komplexen der Zellkerne reagieren. Die Ursachen für ihre Synthese in vivo sind unbekannt. Die ANA charakterisieren verschiedene der sog. bindegewebigen Autoimmunkrankheiten oder Kollagenosen und deren Subtypen, entweder einzeln oder in typischen Kombinationen. Sie unterscheiden sich erheblich im Hinblick auf ihre Spezifität und Prävalenz und damit auch in ihrer diagnostischen Validität. Spezifität und Prävalenz korrelieren nicht miteinander. ANA sind keine Aktivitätsmarker, ausgenommen die Antikörper gegen dsDNA. Die Ergebnisse von ANA-Bestimmungen können nur im Zusammenhang mit anderen paraklinischen Befunden und den klinischen Symptomen richtig bewertet werden.

    Description / Table of Contents: Summary We describe a patient with plexiform neurofibroma and contralateral circumscribed hyperpigmentation along the lines of Blaschko. Such findings represent a form of segmental neurofibromatosis. The origin and classification of segmental neurofibromatosis and its relationship to NFI (von Recklinghausen disease) are discussed.

    Description / Table of Contents: Summary In recent years the investigation of cytokines has been a focus of scientific interest in order to understand the pathogenesis of a variety of diseases. In additions cytokines are increasingly being used for therapeutic purposes, including dermatologic disorders. IL-10 is a recently described cytokine with anti- inflammatory and immunosuppressive qualities which plays an important role in immunoregulation. The overexpression of this mediator has been proven in some inflammatory dermatoses as well as in various skin tumors. These observations help to understand down regulatory events in hyperinflammatory conditions such as allergic or toxic dermatitis on the one hand and the suppression of an adequate anti-tumor response and thereby the progression of malignant tumors on the other hand. Recent investigations indicate a relative IL-10 deficiency in psoriasis. Initial therapeutic applications of IL-10 in psoriasis underline the pathophysiological importance of this cytokine.
    Notes: Zusammenfassung Seit einiger Zeit steht der Nachweis von Zytokinen auch in der Dermatologie im Mittelpunkt des wissenschaftlichen Interesses, insbesondere da diese ein besseres Verständnis der Pathogenese verschiedenster Erkrankungen erlauben. Darüber hinaus finden Zytokine zunehmend auch in der Dermatologie therapeutische Anwendung. Interleukin-(IL-) 10 ist ein erst seit kurzem bekanntes Zytokin, welches in der Immunregulation insbesondere durch seine antiinflammatorischen und immunsuppressiven Eigenschaften eine herausragende Rolle besitzt. Zwischenzeitlich konnte die verstärkte Expression dieses Mediators bei einigen entzündlichen Dermatosen ebenso wie bei verschiedenen Malignomen der Haut nachgewiesen werden. Diese Beobachtungen sind von Bedeutung, da sie einerseits die gewünschte Limitierung von hyperinflammatorischen Prozessen wie bei Ekzemen und Erythemen, andererseits aber auch die Unterdrückung einer adäquaten anti-Tumorantwort und somit Progredienz von Malignomen erklären könnten. Neuere Untersuchungen zeigen, daß bei der Psoriasis eine relative IL-10-Defizienz besteht. Erste therapeutische Applikationen von IL-10, die eine antipsoriatische Potenz vermuten lassen, sprechen auch hier für die pathophysiologische Bedeutung dieses Zytokins.

    Description / Table of Contents: Zusammenfassung Um die Bedeutung der lokalen Zellproliferation im dermalen Infiltrat bei cutanen malignen Lymphomen zu untersuchen, bestimmten wir den Prozentsatz der 3H-Thymidin-markierten Infiltratzellen (3H-Index. Zwischen dem klinischen Stadium der Mycosis fungoides und der Proliferationsaktivität des dermalen Infiltrats besteht eine lineare Beziehung; im Stadium I ist die Proliferation niedrig, im Stadium III sehr hoch. Nicht erhöht ist der 3H-Index im dermalen Infiltrat beim Sézary-Syndrom, diffusen lymphocytischen Lymphom sowie bei Lymphocytoma benigna cutis. Bei der Parapsoriasis en plaques müssen zwei Formen unterschieden werden: bei der kleinfleckigen Form (“chronic superficial dermatitis”) ist der 3H-Index niedrig, während die großfleckige Form (Präretikulotisches Poikiloderm) eine starke Proliferationsaktivität aufweist. Die Untersuchung des Proliferationsverhaltens gibt neue Einblicke in die Pathogenese des dermalen Infiltrats cutaner Lymphome.
    Notes: Summary To obtain information about the role of local proliferation in the pathogenesis of dermal infiltrate in malignant cutaneous lymphomas, we determined the percentage of 3H-thymidine-labeled infiltrating cells (3H-index). A linear correlation was found between proliferative activity and clinical stage in mycosis fungoides, i.e., the 3H-index is moderately elevated in stage I and high in stage III. The 3H-index is within normal range in dermal infiltrate of Sézary syndrome, diffuse lymphocytic lymphoma, as well as in lymphocytoma benigna cutis. In parapsoriasis en plaques two groups can be distinguished: in the smallplaque variant (chronic superficial dermatitis) the 3H-index is low, whereas the large-plaque variant (prereticulotic poikiloderma) shows strong proliferative activity. Thus, determination of proliferative activity seems to give new insights into the pathogenesis of dermal infiltrate in cutaneous lymphomas.

    Notes: Abstract:  The role of T cells in the pathogenesis of psoriasis is widely acknowledged. However, key aspects of their precise function in the disease as well as the relative pathogenic contribution of T-cell subsets are still unknown. T-cell clones have been isolated from psoriatic plaques but a study of conditions affecting the isolation and expansion of T-cell clones from psoriatic skin has not been reported to date. Here, we observe a correlation of disease activity with the frequency of the CD3+CD8+CD25+ subset. We show that prolonged in vitro culture changes the phenotypic subset distribution of T-cell lines derived from psoriatic skin and that T-cell clones can be isolated by sorting of CD25+ cells emigrated from skin fragments after 7 days. We evaluate various conditions affecting expansion of psoriatic T-cell clones in vitro and show that blocking apoptosis can facilitate proliferation of activated T-cell clones in vitro. Our results indicate a prominent role of the CD8+CD25+ T-cell subset in disease pathogenesis and should be useful in the design of experiments aiming at a systematic analysis of the specificity of clones present in psoriatic plaques.

    Notes: Abstract To investigate the role of T cells in psoriasis, we analyzed the T-cell receptor (TCR) repertoire in peripheral blood lymphocytes (PBLs) and in skin lesions of 9 patients with chronic plaque-stage psoriasis by means of immunohistochemistry. An extended panel of monoclonal antibodies detecting 11 different V(3 families was employed in this study. In the peripheral blood, no predominant TCR Vβ expression was detected and interindividual differences were small. Skin-infiltrating T cells showed only marginal differences in the TCR Vβ usage when compared with PBLs. However, in 3 patients the intraepidermal infiltrate comprised up to 40% of T cells expressing a TCR Vβ region which was virtually absent in the peripheral blood and the dermal infiltrate. Thus, although the TCR Vβ usage of skin-infiltrating T cells showed little difference compared to PBLs, several cases exhibited a significant increase of epider-motrophie T cells using one particular Vβ region.

    Notes: Epidermal dendritic cells showing a diffuse activity of acid nonspecific esterase were observed in three cases of mycosis fungoides stage 1. As these cells by their appearance and localization resemble Langerhans cells, and dermal histiocytes and macrophages react in the same manner, this is another enzyme cytochemical suggestion of a relationship between Langerhans cells and the monocyte-macrophage-system.
