    Notes: Abstract We report a case of decompression illness in which the patient developed paraparesis during scuba diving after rapid ascent. MRI of the spine revealed a focal intramedullary lesion consistent with the symptoms. The pathophysiological and radiological aspects of spinal decompression illness are discussed.

    Notes: Abstract We assessed high-resolution MRI of the lacrimal system with conjunctival contrast enhancement in patients with suspected stenosis. We studied 18 patients with epiphora affecting 22 eyes, using a surface coil, before and after conjunctival and intravenous Gd-DTPA. Stenosis or obstruction of the ducts was found in 18 of 22 cases: at canalicular level in 3 cases, at the sac in 7 cases, and in the nasolacrimal duct in 8 cases. Periductal pathology following surgery to the maxillary sinus or dacryocystorhinostomy was seen in three cases. In conjunction with the findings on syringing and/or conventional dacryocystography, the narrowing was classified as stenosis or obstruction.

    Description / Table of Contents: Summary Aim: For dynamic contrast medium (CM) studies, parameter images exploit specific features of the time/intensity curve (TIC) of each pixel and represent these values in a new image. Exisiting concepts of two-dimensional CM analysis are extended for three-dimensional applications using adequate computer graphic visualization. Methods: Imaging: intravenous CM injection; first-pass analysis using fast CT acquisition of a single slice; CM accumulation by 40 MR slices (6 sequences every 90 s=240 images). Software: TIC of arbitrary 2D/3D regions; parameter images: (1) temporal maximum intensity projection (TMIP) of all voxels over time, (2) temporal CM gradients (TG) based on TIC amplitude, (3) TIC slope, (4) time of TIC peak, (5) correlation coefficient to reference TIC. The calculated 3D data of CM accumulation was visualized using multiplanar reformation, MIP, surface reconstruction, volume rendering, texture mapping and animation. Results: In first-pass analyses, TMIP and TG allowed the simultaneous or separated presentation of different temporal phases of the CM bolus. Correlation images emphasized regions with similarities to given TIC patterns. Three-dimensional computer graphic techniques enabled (1) anatomical/functional mapping of original image and CM accumulation and (2) fused display of both spatial CM enhancement and color-encoded time of TIC peak in one common image. Conclusions: The quantification of presence, magnitude, and time-of-peak of CM accumulation in local image regions supports the assessment of vascularization and of ischemic or necrotic areas.
    Notes: Zusammenfassung Ziel: Bei dynamischen Kontrastmittel- (KM) Studien können spezifische Merkmale der Zeit/Intensitäts-Kurve (ZIK) eines jeden Bildpunktes verwendet und in Form von Parameterbildern dargestellt werden. Aufbauend auf bestehenden Konzepten der zweidimensionalen Analyse der KM-Dynamik soll eine dreidimensionale Erweiterung mit adäquater computergraphischer Visualisierung realisiert werden. Methode: Bildgebung: Intravenöse KM-Injektion; First-pass-Analyse mit CT bei schneller Akquisition einer einzigen Schicht; Untersuchung der längerfristigen KM-Anreicherung mit 40 MR-Schichten (6 Sequenzen alle 90 s=240 Bilder). Softwareentwicklungen: ZIK beliebiger ROIs/VOIs; Parameterbilder: 1. Maximum-Intensitäts-Projektion (TMIP) aller Pixel als Funktion über die Zeit; 2. zeitliche KM-Gradienten (TG) anhand der ZIK-Amplitude; 3. ZIK-Steigung/Gefälle; 4. Zeitpunkte der ZIK-Maxima; 5. Korrelationskoeffizienten zu Vergleichs-ZIK. Die berechneten 3D-Daten der KM-Anreicherung wurden mittels multiplanarer Reformatierung, MIP, Oberflächenrekonstruktion, Volume Rendering und Textur-Mapping visualisiert und animiert. Ergebnisse: TMIP und TG erlaubten bei First-pass-Analysen die simultane oder getrennte Präsentation unterschiedlicher Zeiträume des KM-Bolus. Die Korrelationsbilder gestatteten die Hervorhebung von Regionen, die Ähnlichkeiten zu vorgegebenen ZIK-Mustern aufwiesen. Computergraphische Techniken gestatteten 1. ein anatomisch/funktionales Mapping von Originalbild und KM-Anreicherungsverhalten sowie 2. die fusionierte Darstellung von KM-Anreicherungsbetrag und farbkodierter Zeit des KM-Maximums in einem gemeinsamen Bild. Schlußfolgerungen: Die exakte Quantifizierung von Präsenz, Grad und Ankunftszeit des Kontrastmittels in lokalen Bildregionen unterstützt die Beurteilung der Vaskularisation sowie von ischämischen bzw. nekrotischen Arealen.

    Notes: Abstract Background:Gemcitabine (Gemzar®) and 5-fluorouracil (5-FU)plus folinic acid (FA) both have proven activity in the treatment of patientswith advanced pancreatic cancer. The present study was initiated toinvestigate the efficacy of gemcitabine in combination with 5-FU–FA. Patients and methods:Thirty-eight patients, median age 60 years(range 34–70) with inoperable, stage IV, pancreatic cancer were enrolledinto the study and treated on an outpatient basis. All except one patientreceived at least one cycle of treatment with gemcitabine (1000mg/m2), followed by FA (200 mg/m2) and 5-FU (750mg/m2) administered as a 24-hour continuous infusion on days 1, 8,15 and 22 of a 42-day schedule. No patient had received prior chemotherapy orradiotherapy. All 38 patients were assessed for efficacy, toxicity and timeto progressive disease. Results:Two patients (5%), achieved a partial response andthirty-four patients (89%) achieved stable disease. There were twoearly deaths (≤4 weeks). The median time to progression was 7.1 months(range 0.4–18.1+; 95% confidence interval (95% CI):5.3–7.9 months). Three patients had a progression-free interval ofgreater than 12 months and 12 of 38 patients (32%) survived longer than12 months. The median overall survival was 9.3 months (range 0.5–26.5;95% CI: 7.3–13.0 months). The incidence of grade 3 and 4toxicities was low. Conclusions:The combination of gemcitabine and 5-FU–FA isactive and well tolerated and seems to offer an improvement inprogression-free interval over both gemcitabine monotherapy and 5-FU–FAtherapy.
