    Notes: Summary The difference in the hearing threshold before and after treatment with cis-diaminnedichloroplatinum (DDP) is analysed in 69 patients. Hearing loss due to DDP treatment is mainly limited to 8,000 Hz and the incidence is about 40%. The effect of DDP is dose-related, although even at the lowest dose 20% of the patients are affected. The age of the patients is not an important factor. Loss of hearing due to DDP treatment occurs independent of any pre-existent hearing loss although those patients with great pre-existent hearing loss do not show a further loss. Of the patients with hearing loss, 39% show a difference of 20 dB or more between the left and the right ear. Hearing loss due to DDP is of minor importance compared with many of the other side-effects of DDP.

    Description / Table of Contents: Summary An unusual blood vessel in the cochlea of a patient is reported. The blood vessel derives from the osseous lamina spiralis and crosses straight through the scala tympani toward the lateral wall of the perilymphatic duct. In its course a branch derives from this vessel toward the other spiral vessel. In the region of this aberrant vessel a complete hair-cell loss is present. A high tone perceptive loss with a relative dip and tinnitus was found in the same ear during life. The possible cause and effects of this aberrant vessel are discussed.
    Notes: Zusammenfassung Bericht über einen ungewöhnlichen Verlauf eines Blutgefäßes in der Cochlea eines Patienten. Das Gefäß zog von der Lamina spiralis ossea unter Durchquerung der Scala tympani bis zur lateralen Wand des Ductus perilymphaticus. In der Region dieses abnorm verlaufenden Gefäßes fand sich ein völliger Haarzellverlust. Beim Patienten bestand während des Lebens ein Hörverlust vor allem bei 2000 Hz mit Ohrensausen.

    Description / Table of Contents: Summary Gentamicin-induced cochlear degeneration in the guinea pig was studied by complete hair-cell counting (cytocochleograms) and phase-contrast and interference microscopical examination of the stria vascularis and Reissner's membrane. Gentamicin (100 mg/kg/day) was administered over a period of 7–17 days. The first loss of hair cells (OHC) occurred in a region 6–8 mm from the round window. From this ‘degeneration point’, the loss of haircells progressed towards the round window (fast) and the apex (slowly). The stria vascularis showed no signs of degeneration. Reissner's membrane, on the other hand, showed intracellular vacuolization of the endolymphatic cells over the complete length of the cochlea after 12 or more days' intoxication. Hearing loss was measured by electrocochleography with skin electrodes. The histologic findings were compared with the objective audiograms.
    Notes: Zusammenfassung Die durch Gentamyzin verursachte Degeneration in der Kochlea des Meerschweinchens wurde mit dem Zytokochleogramm sowie mit Phasenkontrast- und Interferrenzmikroskopie der Stria vascularis und der Reissnerschen Membran dargestellt. Der erste Haarzellenverlust wurde 6–8 mm vom runden Fenster entfernt festgestellt. Von diesem „Degenerationspunkt“ aus schreitet der Haarzellverlust rasch zum runden Fenster und langsam spitzenwärts fort. In der Stria vascularis war keine Degeneration zu erkennen, aber die Reissnersche Membran zeigte eine intrazelluläre Vakuolisierung der endolymphatischen Zellen. Der Hörverlust wurde mit der Elektrokochleographie bestimmt und mit den histologischen Befunden verglichen.

    Notes: Summary Vancomycin has been successfully used clinically for many years. Although early reports found ototoxicity to be a side effect of this antibiotic, later studies could not confirm this. For this reason we have started an experimental study in an animal model on the ototoxic effects of vancomycin. We now report the results of this study. Fourteen healthy Mongolian gerbils were treated with intraperitoneal injections of vancomycin (80 mg/kg per day) for a 2-week period. Before and after treatment each animal's hearing was evaluated by evoked response audiometry. Post mortem the cochleae were investigated by scanning electron microscopy in Amsterdam and by microdissection with surface preparations (hair cell counting) in Stockholm. The results of this study show that there is no clear evidence for the existence of ototoxicity due to vancomycin in this dosage.

    Notes: Abstract Use of an ionomer bone cement is described for repairing the ossicular middle ear system in revision stapes surgery. The cement was used to enlarge an eroded inadequate incus to allow placement of a new stapes prosthesis. Reconstruction of the incus with this new bone cement was readily performed and hearing results after 2 years of follow-up have been promising.

    Notes: Summary The microscopical and ultramicroscopical changes in the stria vascularis due to cis-diamminedichloroplatinum II (DDP) were studied. Sixteen healthy adult guinea pigs were used for the experiment. A standard dosage DDP (1.5 mg/kg/d) was administered over a period of 5–20 days. A clear degeneration pattern was found (ranging from no changes to cystic degeneration with protrusion of marginal cells followed by loss of marginal cells). DDP seems to be especially toxic for marginal cells in stria vascularis in the guinea pig.

    Notes: Summary A scanning electron microscope (SEM) study of corrosion cast preparations of the vessels of the spiral lamina and the spiral limbus was carried out in adult rats. This method confirms present knowledge of the modiolar vascularization and also shows its distribution in a three-dimensional model. As a result of this technique we have been able to demonstrate the modiolar vascular supply of the organ of Corti at different levels.

    Notes: Summary We report the case history of a 54-year-old man who developed a fatal neurological disorder 4 years after a succesful tympanoplasty with homograft pericardium. The final diagnosis of this case was Creutzfeldt-Jakob disease. This infectious spongiform encephalopathy is probably caused by a slow virus that can be transmitted by transplantation materials. The possible accidental transmission of Creutzfeldt-Jacob disease by the use of homograft materials in otologic surgery is discussed.

    Notes: Summary A transcutaneous bone-conduction hearing aid was implanted in 11 patients who were not suitable for transcranial sound amplification. Audiological and surgical selection criteria were followed strictly. One device had to be explanted and minor revision surgery was needed in two cases for skin irritation and scarring. In general the aids were well tolerated but the amplification power of the external device proved to be insufficient in some patients, in whom bone conduction levels were on the borderline of the selection limits.

    Notes: Abstract This study reports the evaluation of hearing results after implantation of a Teflon piston of a different diameter in cases of otosclerosis requiring stapedotomy. By random selection, a Teflon piston with a shaft diameter of 0.3 mm was inserted in 34 cases and a piston with a shaft diameter of 0.4 mm in 26 cases. A retrospective analysis of the pre- and postsurgery audiological results of these two patient groups was carried out by microcomputer. A repeated statistical measures analysis of variance was used to test and estimate the air-conduction frequency-specific differences between the two prostheses with respect to changes in pre- and postoperative healing. The results of this comparative study of the two Teflon pistons with different diameters indicate statistically a greater hearing gain for the 0.4-mm prosthesis, especially in the lower frequencies.
