    Notes: Abstract A 47,XXY karyotype was found in a 6-year-old girl. The patient had female external genitalia, clitoromegaly, remnants of the ductus mesonephricus, uterus, and gonads in the labia majora which were determined to be testes by histology. Cytogenetic and DNA analyses suggest that the Y chromosome had a normal structure and that both X chromosomes were of maternal origin. The unusual clinical findings in the patient are discussed.

    Description / Table of Contents: Zusammenfassung Über die verschiedenen Stadien der Portioentwicklung in der Kindheit und Jugend und den Verlauf der konnatalen Ektropionierung des zervikalen Drüsenfeldes auf die Portiooberfläche bei der Lebenden besitzen wir bisher nahezu keine Kenntnisse. Klinisch vaginoskopische Untersuchungen an 1850 Mädchen aller Altersstufen, die wegen gynäkologischer Beschwerden in unsere Sprechstunde für Kinder und Jugendliche überwiesen wurden, zeigen, daß es eine Persistenz des physiologischen Ektropiums von der Neugeborenen- bis zur Adoleszentenzeit nicht gibt. Die praktische Bedeutung der Beobachtung dieses hormonabhängigen Verhaltens wird deutlich, wenn man pathologische Veränderungen richtig einschätzen will. Dies ist nur möglich, wenn die physiologischen Stadien der Portioentwicklung bekannt sind. Anhand der hier vorgenommenen Stadieneinteilung ist die Diagnostik einer gestörten sexuellen Entwicklung und einer Neubildung leichter möglich. Das Ektropium der Cervix uteri ist in der postpartalen und pubertalen Phase ein physiologischer, in der Ruheperiode ein pathologischer Befund, der dringender Abklärung bedarf. Eine sichere Diagnose kann nur mit Hilfe der Vaginoskopie gestellt werden. Dieses Verfahren hat sich als medizinisch, technisch und psychologisch problemlos erwiesen. Es ist unentbehrlicher Bestandteil der kindergynäkologischen Untersuchung.
    Notes: Summary To date we have almost no information regarding the various stages of cervical development in childhood and youth, nor about the development of a congenital ectropion of the cervical glandular field in girls. We conducted clinical vaginoscopic examinations on 1,850 girls of all ages, who had been directed to our clinic for children and adolescents because of gynecological complaints. These examinations showed that physiological ectropions do not persist in girls between the ages of newborn to adolescent. The practical meaning of the observations of this hormone-dependent behavior becomes clear when we attempt to interpret pathological changes correctly. This is only possible in cases where the physiological stage of development of the cervix is known. Based on the different stages that we have identified, diagnosis of a disturbed sexual development is simplified. The ectropion of a uterine cervix is a physiological phenomenon when it occurs postnatally and during puberty, whereas its appearance during the rest period is considered to be pathological and calls for immediate corrective measures. A reliable diagnosis can only be made with the aid of vaginoscopy. This procedure has proved to be unproblematic in its medical, technical, and psychological aspects. It is an indispensable part of a pediatric gynecological examination.

    Description / Table of Contents: Summary Contrary to current opinion, the fetal vagina is not a solid plate. Already at the 14th week of gestation, the urethra, vagina, uterus, and Fallopian tubes have a lumen which can be identified by means of the intravaginal injection of rapidly setting silicon liquid. However, after the 26th week of gestation the injected substance fills the vagina but does not enter the cervical canal. This is probably due to functional plugging of the endocervical canal by the secretory activity of its epithelium. Silicon plastic moulds of the vaginal vault consistently show a circular groove separating the middle from the upper third of the vagina. This groove could be the line of fusion between the mesodermal Mullerian ducts and the urogenital sinus.
    Notes: Zusammenfassung Entgegen der bisherigen Vorstellung ist die fetale Vagina keine solide Platte. Nach Ausguß mit einer Silikonmasse stellen sich schon in der 14. Fetalwoche Urethra, Rektum, Vagina, Uterus und beide Tuben als Hohlräume dar (Viszeroplastik). Jedoch bereits vor der 26. Fetalwoche füllt sich die Vagina vorwiegend infolge der „abdichtenden“ sekretorischen Aktivität des unteren Zervixdrüsenfeldes nur noch bis zur Portio. Die Viszeroplastiken zeigen im Übergang vom mittleren zum oberen Vaginaldrittel immer eine Furche, die der Fusionsstelle zwischen Müllerschem Mesoderm und Sinus-Endoderm entsprechen dürfte.

    Notes: Summary Twenty-eight hours after CO2 insufflation and curettage in a 26 year old, haemolysis, jaundice and anuria developed. As an ectopic pregnancy was also suspected, a diagnostic laparoscopy was done and was followed by a hysterectomy and left salpingo-oophorectomy before Clostridium perfringens was grown on culture. The survival of the patient despite clostridial infection is probably due to immediate surgical treatment and intensive post-operative care.
