Summary The introduction points to the necessity of outlining the anatomical and neuro-physiological relations especially for the temporal and the rhinencephalic structures in order to make the forms of epilepsy to be discussed better understandable. The position of the psychomotor seizures in relation to the other focal-cortical and to the idiopathic forms of epilepsy is discussed and a hypothesis about the reasons of their special character is given. In the next chapters general remarks are made about the problem of the epileptic focus and its pathological anatomy, and then follows a representation of problems of diagnosis, indication, prognosis, technic and results of operations for focal epilepsy and a special chapter about hemispherectomy. Indications and technic of the latter operation are explained and the results showing improvement are emphasized. The experiences of the authors in the operative treatment of focal epilepsy are encouraging, but the authors warn of too great enthusiasm and point out, that the operative results depend to a high degree on a very strong indication for operation. Also the results which are encouraging in the first years after operation may later prove not so encouraging.

Résumé Dans l'introduction on souligne les difficultés rencontrées à rester fidèle à la délimitation du sujet tel qu'il a été posé lors de la fixation du thème de ce rapport. Les deux premiers chapitres sont consacrés aux données anatomiques et neuro-physiologiques actuelles des régions temporales et des structures rhinencéphaliques permettant une meilleure compréhension des formes d'épilepsie dont il est question. La position particulière prise par les crises temporales et psychomotrices en regard des crises focalisées d'autres régions du cerveau et des crises idiopathiques est commentée et une hypothèse étiologique est proposée pour expliquer les particularités de leur développement et de leurs manifestations cliniques. Il s'y ajoute quelques remarques plus générales concernant le foyer épileptogène et en particulier sur l'anatomie pathologique de lépilepsie focale. Les chapitres suivants traitent des problèmes de diagnostic, de l'indication opérative, du pronostic, de la technique et des résultats de l'intervention. Un chapitre à lui seul est consacré à l'hémisphérectomie, dont l'indication, la technique et le pronostic favorable sont soulignés. Les résultats personnels des auteurs en matière de traitement chirurgical de l'épilepsie focale ont été encourageants, mais il faut se garder d'un enthousiasme exagéré. Il est souligné que le succès de l'intervention dépend pour une très grande part d'une indication excessivement stricte et que des résultats opératoires du début peuvent se gâter dans les années qui suivent.

Zusammenfassung Einleitend wird auf die Schwierigkeiten der Abgrenzung des Themas in der vorliegenden Form hingewiesen und die Notwendigkeit unterstrichen, zum besseren Verständnis der zu behandelnden Epilepsieformen etwas genauer auf die anatomischen und neurophysiologischen Verhältnisse, insbesondere des Temporalgebietes und der rhinencephalen Strukturen, einzugehen, die in den beiden folgenden Abschnitten besprochen werden. Die Stellung der temporalen psychomotorischen Anfälle gegenüber den übrigen cortical-herdbedingten und idiopathischen Epilepsieformen wird besprochen und eine Hypothese über die Ursachen ihrer besonderen Verlaufsform entwickelt. Es schließen sich allgemeine Bemerkungen zum Problem des epileptischen Herdes und zur pathologischen Anatomie der Herdepilepsie an. Dann folgt eine Darstellung ihrer diagnostischen Probleme, der Indikation, Prognose, Technik und Ergebnisse des Eingriffes, sowie ein besonderer Abschnitt, der der Hemisphärektomie gewidmet ist. Indikation und Technik werden dargelegt und die günstigen Erfolgsaussichten dieser Operation unterstrichen. Die eigenen Erfahrungen der Autoren auf dem Gebiete der Behandlung der Fokalepilepsie waren ermutigend, doch wird vor einem übersteigerten Enthusiasmus gewarnt und darauf hingewiesen, daß der Erfolg des Eingriffes in hohem Maße von einer strengen Indikationsstellung abhängt, und daß sich anfänglich gute Operationsresultate in der Folgezeit wieder verschlechtem können.
