Summary Report of 18 cases, in which septicemia developed due to bacterial colonization of Spitz-Holter or Pudenz valves. These are 9% of all patients having undergone ventriculoatriostomy in the neurosurgical hospital of the University of Freiburg from 1961 to 1965 (average occurrence according to the literature: 14%). Local complications in the scalp wound, such as wound necrosis, fistula, or subcutaneous suppuration, occurred in 6 of these 18 cases. They were caused by coagulase-positive staphylococcus aureus in 5 cases and by coagulase-negative staphylococcus albus in one case (wound necrosis without suppuration). In the other 12 cases there was no visible contamination of the operation site. Symptoms of septicemia occurred in 8 of the cases within two weeks after ventil insertion. Coagulase-negative staphylococcus albus was the causative organism in 6 of these cases. In one case achromobacter was the infecting agent. In another case it was coagulase-positive staphylococcus aureus. The short interval between ventil insertion and onset of septicemia as well as the preponderance of bacteria belonging to the normal skin flora suggest that the bacteremia in these 8 cases has originated in the introduction of saprophytic organisms at the time of surgery. 4 cases of bacteremia occurred several months after surgery. In two of these staphylococcus albus was the causative agent, in the other two staphylococcus aureus. Altogether the early cases, which occurred within two weeks after surgery, are prevailing in our material (13 of 18 cases). Both valve systems participate in valve sepsis according to the frequency of their implantation. Myelomeningoceles show no higher frequency in valve sepsis than other causes of hydrocephalus. The clinical course was fulminant and sometimes complicated by osteomyelitis, staphylococcal pneumonia or meningitis in those cases with inflammatory reactions around the valve due to pathogenic staphylococci. The majority of our cases had no local complications and apart from the septic temperatures showed only slight symptoms of sepsis, such as anemia and hepatosplenomegaly. The general condition of the patients was well. Four patients died, two by staphylococcal sepsis and two in connection with the increased liquor pressure after removal of the shunting apparatus. Intensive and specific antibiotic treatment was successful only in one patient, another case possibly recovered spontaneously. The shunt was removed in 12 cases; reimplantation of a shunting system was not necessary in 7 cases.

Zusammenfassung Es wird über 18 Fälle mit Ventilsepsis berichtet. Die Komplikation ist in 9% der in der Neurochirurgischen Universitätsklinik Freiburg i.Br. operierten Hydrocephaluskinder eingetreten (durchschnittliche Häufigkeit in der Literatur: 14%). In 6 Fällen traten lokale Komplikationen im Operationsbereich (Wundnekrose, Fistelung, subcutane Eiterbildung) ein, die zur Sepsis führten. Fünf Fälle mit Absceßbildung waren mit Staphylococcus aureus infiziert, ein Fall (Wundnekrose) ohne Eiterung mit Staphylococcus albus. Bei den anderen 12 Fällen lag keine lokale Schädigung oder andere erkennbare Ursache für die Ausbildung der Sepsis vor. In 8 von diesen Fällen begannen die Sepsiszeichen unmittelbar postoperativ, so daß ein direkter Zusammenhang mit dem operativen Eingriff naheliegt, zumal als Erreger bei 7 von den 8 Fällen ubiquitäre Hautkeime (sechsmal Staphylococcus albus, einmal Achromobacter) isoliert wurden. Die restlichen 4 Sepsisfälle traten erst Monate nach der letzten Operation auf und waren zweimal durch Staphylococcus albus und zweimal durch Staphylococcus aureus bedingt. Insgesamt überwiegen die innerhalb von 2 Wochen postoperativ aufgetretenen Frühsepsisfälle (13 von 18 Fällen). Die beiden verwendeten Ventilsysteme (Spitz-Holter, Pudenz) sind an den Sepsisfällen entsprechend ihrer Verwendungshäufigkeit beteiligt. Bei Patienten mit Myelomeningocele ist keine Häufung von Ventilsepsisfällen zu beobachten. Der klinische Verlauf war bei Vorliegen einer Wundinfektion mit pathogenen Staphylokokken foudroyant und in einigen Fällen kompliziert durch Osteomyelitis, abscedierende Pneumonie oder Meningitis. Die anderen, zahlenmäßig überwiegenden Fälle verliefen trotz septischer Temperaturen symptomenarm. Die sich häufig ausbildende Anämie machte nur selten eine Bluttransfusion erforderlich. 4 Patienten mit Ventilsepsis starben, davon 2 durch Staphylokokkensepsis und 2 in Zusammenhang mit dem nach der notwendigen Ventilentfernung aufgetretenen Liquordruck. Ausführliche und gezielte antibiotische Behandlung führte nur in einem Falle zur Ausheilung der Sepsis, ein weiterer Fall ist möglicherweise spontan ausgeheilt. Das infizierte Ventilsystem wurde in 12 Fällen entfernt, siebenmal erübrigte sich eine erneute Ventileinlage.
