    Notes: Summary For metastatic disease of the spine, anterior operations on the vertebral bodies often include methylmethacrylate cementation. The cement curing process may produce high temperatures in the surroundings, as demonstrated in joint replacement surgery, and there is a risk of thermal injury to the spinal nerves. In cadavers, we studied the heat arising during curing of cement on the dural sac, and the temperature of the cement surface was measured when the vertebral body was reconstructed using acrylic cement in the same way as in tumor surgery. The temperature increase on the surface of the dural sac during polymerization was between 4° and 12°C, depending on the amount of protection. Only a moderate temperature elevation was measured on the surface of the dural sac, provided that the posterior cortex of the vertebra was retained together with 0.5 cm of the spongious bone or a silicone membrane.

    Description / Table of Contents: Résumé Cinq cas de fracture par enfoncement du plateau tibial externe ont été traités par relévement du fragment impacté, greffe osseuse et vis à spongieux. Au cours de l'intervention des marqueurs au tantale ont été placés dans le fragment enfoncé et dans la métaphyse tibiale pour analyse stéreophotogramétrique. En post-opératoire un plâtre a été maintenu pendant un mois et l'appui n'a été autorisé qu'à la fin du troisième mois. Une récidive de l'enfoncement du fragment relevé (2.8 mm au maximum) est survenue dans trois cas, durant le premier mois post-opératoire. Les auteurs concluent que la mise en décharge post-opératoire n'est pas indispensable et qu'une mobilistion précoce plus active pourrait être envisagée.
    Notes: Summary Five cases of fracture of the lateral tibial condyle of the split-depression type were treated by elevation of the depressed fragment, bone grafting and cancellous bone screws. During the operation, tantalum markers were inserted into the depressed fragment and the tibial metaphysis for radiographic stereophotogrammetric analysis. Postoperatively a plaster cast was applied for one month and weightbearing was not allowed for 3 months. In three cases, a depression of the elevated fragment, at the most 2.8 mm, recurred during the first month after operation. This was not considered to be significant and more aggressive postoperative mobilisation may be warranted.

    Notes: Summary The results in relation to the correction achieved 1 year after surgery of a series of tibial osteotomies in 50 patients (52 knees) are presented with a total observation time of 10 years. Mean age at the time of osteotomy was 56 years. Overcorrected knees had a significantly better result after 10 years. Progress of gonarthrosis occured in 6/34 overcorrected knees compared with 3/4 in the normo- and undercorrected group. One out of 34 overcorrected knees recurred in varus. Five knees were revised by knee arthroplasty or reosteotomy, one of which was overcorrected.

    Description / Table of Contents: Résumé La fixation de prothèses du genou PCA Modular à tige tibiale cruciforme, cimentée ou non, a été étudiée par analyse stéréophotométrique, 6 semaines après l'intervention, chez 26 patients randomisés. Des contraintes externes physiologiques ont été appliquées en rotation externe et interne et l'accroupissement a simulé la mise en charge excentrique postérieure. Dans tous les cas un déplacement provoqué a pu être constaté. Une différence statistiquement significative, entre les deux groupes, n'a pu être mise en évidence ni pour le déplacement provoqué, ni pour la migration initiale, à l'exception de l'abaissement constaté entre les positions couché sur le ventre et debout (0,0 pour le groupe cimenté contre 0,2 mm pour le non-cimenté, p〈0.05). Dans certains cas le déplacement le plus important a été noté durant le passage de la position de repos à la position debout ou lors des contraintes en rotation interne-externe, alors que dans d'autres cas le déplacement le plus grand a été observé lors de l'accroupissement. Une différence statistiquement significative de l'abaissement de la prothèse dans cette position a été constatée, puisqu'il est nul pour les pièces cimentées et de 0,2 mm pour les non-cimentées (p〈0,05). La forme de la prothèse avec une queue cruciforme ainsi que l'utilisation d'une lame de scie à refroidissement interne peuvent expliquer que la différence ne soit que minime entre les deux groupes, cimenté et non-cimenté.
    Notes: Summary The fixation of cemented and uncemented PCA modular cruciform-stemmed tibial components was examined 6 weeks after operation using roentgenstereophotogrammetric analysis in 26 randomly selected patients. Physiological external forces were applied in external/internal rotation and eccentric posterior loading generated by squatting. All cases showed inducible displacement, but there were no statistically significant differences between the 2 groups in inducible displacement or in the initial migration, except for subsidence between the lying and standing position. In some, the greatest movement was found when changing from resting to standing, or during rotational stress; in others, the greatest movement was found during squatting. There was a statistical difference in subsidence on squatting which was greater in the uncemented group. The design of the cruciform stem and the use of an internally cooled saw may explain this small differences between the 2 groups.

    Notes: Six vacuum mixing systems, Cemvac®, Merck®, Mitvac®, Optivac®, Osteobond®, and Stryker®, were tested using prechilled Palacos R® bone cement to investigate the reduction of porosity compared to mixing at atmospheric pressure. In addition the Optivac®, Osteobond®, and Stryker® were tested using Simplex P® bone cement to find out if they were effective in reducing the porosity of a middle viscosity bone cement. All vacuum mixing systems reduced the number of macropores (〉1 mm) and micropores (0.1 mm 〈 voids 〈1 mm) and increased the density of both Palacos R® and Simplex P. But only the Optivac®, Stryker®, and Merck® systems reduced the area percentage of macropores with more than 50% compared to the control. When using Simplex P® bone cement, all three mixing systems tested reduced the numbers and the area percentage of macropores compared to the control. The results show that vacuum mixing is effective in reducing the porosity in both a high viscosity cement such as Palacos R® and a middle viscosity cement such as Simplex P®. Not all systems tested were effective in reducing the number and size of large voids. © 1996 John Wiley & Sons, Inc.
