Summary Arterial and coronary venous blood samples of 26 patients, in whom routine preoperative cardiac catherization was performed, were analysed before and after the administration of propranolol. The values for blood pH, pCO2, standard bicarbonate, O2-saturation and pO2 as well as glucose, free fatty acids, pyruvate, lactate, hydrocybuturate, and acetacetat of cardiac and general metabolism were determined. Changes of general metabolism were observed by comparing the arterial level of substrates before and after the administration of propranolol. The behaviour of the substrates of cardiac metabolism was studied by comparing arterio-coronary venous substrate differences as well as their share in oxygen consumption of total oxydative cardiac metabolism. The arterial oxygen pressure decreased from 91,62 mm Hg before to 85,69 mm Hg after the administration of propranolol (p 0,001); accordingly the arterio-coronary venous difference of oxygen pressure decreased from 75,8 mm Hg before to 70,3 mm Hg after the administration of propranolol (p 0,005). After blockade of beta-receptors the values for free fatty acids (p 0,01) and lactate (p 0,0025) decreased significantly.

Zusammenfassung Bei 26 Patienten wurden bei routinemäßig präoperativen Herzkatheteruntersuchungen im arteriellen und coronarvenösen Blut vor und nach Verabreichung von Propranolol Bestimmungen der Blutgase und der energieliefernden Substrate des Herzstoffwechsels sowie des allgemeinen Stoffwechsels durchgeführt. Im einzelnen wurde gemessen: Blut-pH, pCO2, Standard-Bicarbonat, O2-Sättigung und pO2; Glucose, freie Fettsäuren, Pyruvat, Lactat, Hydroxybutyrat, Acetacetat. Durch Vergleiche des arteriellen Spiegels der Substrate vor und nach Propranolol wurden die Veränderungen im Bereich des allgemeinen Stoffwechsels beobachtet. Durch Vergleich der arterio-coronarvenösen Differenzen der Substrate und deren Sauerstoff-Anteile am gesamten oxydativen Stoffwechsel des Herzens wurde ihr Verhalten in Hinsicht auf den Myokardstoffwechsel geprüft. — Der arterielle Sauerstoffdruck sank von 91,62 vor auf 85,69 mm Hg nach Propranolol ab (p〈0,001); entsprechend sank die arterio-coronarvenöse Differenz des Sauerstoffdruckes von 75,8 vor auf 70,3 mm Hg nach Propranolol ab (p〈0,005). Die freien Fettsäuren (p〈0,01) und das Lactat (p〈0,0025) im arteriellen Blut sanken nachβ-Receptoren-Blockade signifikant ab. Die übrigen Substrate änderten sich nicht signifikant. Die arterio-coronarvenösen Differenzen von Lactat (p〈0,05) und von Acetacetat (p〈0,05) sanken nach Propranolol signifikant ab. Der Sauerstoffanteil am gesamten oxydativen Stoffwechsel des Herzens für Lactat sank nach Propranolol ebenfalls signifikant ab (p〈0,05). Die Sauerstoffanteile der übrigen Substrate änderten sich unterβ-Receptoren-Blockade nicht signifikant.
