    Description / Table of Contents: Zusammenfassung Es wird über Homotransplantation der Lunge bei zwei jungen Patienten berichtet, bei denen eine pulmonale Insuffizienz bestand. Bei dem einen war die Lunge durch Salzsäuredämpfe zerstört, bei dem anderen war die pulmonale Insuffizienz durch Pneumonie bedingt. Es konnte zunächst eine Verbesserung der pulmonalen Funktion erreicht werden, aber, die unterschiedlichen Widerstände zwischen dem Transplantat und der eigenen verbliebenen Lunge führten zu einer Verteilungsstörung der Ventilation und Perfusion zwischen den beiden Lungen. Damit werden die klinischen Schwierigkeiten und Probleme der Ventilation nach der Transplantation demonstriert. Wir glauben, daß die getrennte künstliche Beatmung des Transplantates und der eigenen Lunge mit unterschiedlichen Beatmungsdrucken sowie die Blockierung der Pulmonalarterie, der eigenen Lunge mit einem Ballonkatheter die therapeutischen Konsequenzen darstellen könnten.
    Notes: Summary The authors report on homotransplantation of a lung in two young patients who suffered from pulmonary insufficiency. In one patient the lung was destroyed by hydrochloric acid vapors and in the other patient the pulmonary insufficiency was caused by pneumonia. Initially improvement of pulmonary function could be achieved but the different resistances between the transplanted organ and the patient's own remaining lung resulted in a distribution disorder of ventilation and perfusion between the two lungs. This demonstrates the clinical difficulties and problems of ventilation after transplantation. We believe that separate positive pressure ventilation of the transplant and of the patient's own lung with different ventilation pressures as well as blocking of the pulmonary artery of the patient's own lung with a ballooncatheter could be of therapeutic value.

    Description / Table of Contents: Zusammenfassung Es wird über zwei erfolgreiche Replantationen eines unteren Extremitäten bei einem erwachsenen Mann und eines Oberarmes bei einem Knaben berichtet. Das Interval zwischen Durchtrennung und Wiederherstellung der Zirkulation betrug 3 bzw. 5 Std. Bei dem ersten Patienten konnte nach einer primären Nervennaht ca. 1 1/2 Jahre nach dem Unfall eine partielle Wiederkehr der motorischen und sensiblen Nervenfunktionen nachgewiesen werden, die ihm das selbständige Gehen ermöglicht. Bei dem zweiten Patienten ist die Replantation des Oberarmes erfolgreich verlaufen. Bereits 6 Monate nach der sekundären Nervennaht ist die motorische und sensible Reinnervation des replantierten Armes nachweisbar. Die Beobachtung der Funktion und Elektrotherapie der Muskulatur dauert an.
    Notes: Summary This report includes the results of two successful replantations of an almost completely amputated leg in an adult man and of a severed arm in a young boy. In the first case the replantation procedure was started less than one hour and in the second one two hours after amputation and the arterial circulation was restored in 3 an 5 hours, respectively. The important points are as follows: 1. Preservation of the severed extremity with hypothermia and by this increase of ischemic tolerance. 2. Exact debridement to remove the necrotic part. 3. Restoration of blood circulation with sufficient arterial and venous anastomosis after elimination of microthrombosis and blood cell aggregations by lavage of the vessels with heparin-ringer solution. 4. Stable osteosynthesis. 5. Primary nerve repair. 6. Physical therapy and electrical stimulation of denerved muscles.

    Notes: Two simple sample preparation methods for the speciation analysis of triphenyltin and butyltin compounds in marine biotissues, using tetramethylammonium hydroxide (TMAH) solubilization and enzymic hydrolysis, have been developed and compared with conventional acid digestion. Derivatization was carried out in situ using sodium tetraethylborate (NaBEt4) without prior separation of the analytes from the tissue matrix. Separation and detection was performed using capillary gas chromatography (GC) coupled to microwave-induced plasma atomic emission spectrometry (MIP AE) allowing detection limits of 2 ng g-1 (as tin) to be reached. The accuracy of the presented methods was demonstrated by the analysis of a fish reference material (NIES No. 11). the necessity for sample clean-up is discussed and examples of the analysis of mussel tissue are shown.
