    Notes: We have developed a simulation technique, based on a dynamic mean field theory to describe the evolution of a melt of liquid crystalline polymers that is quenched below its isotropic–nematic transition temperature. The polymers are described by chains characterized by a bending energy depending on the angles between consecutive chain segments and a Maier Saupe term accounting for the effect of mutual alignment of the segments in the melt. The equations for the time evolution of the system depend on coarsened variables, which are the density ρ(r) and the orientation distribution S(underbar)(r). The density field ρ evolves according to a diffusion-type equation, whereas the orientation field S(underbar) follows a growth equation. © 2002 American Institute of Physics.

    Description / Table of Contents: Zusammenfassung. Bei einem 71 jährigen Patienten mit einem seit 8 Wochen bestehenden schmerzlosen Verschlußikterus war sonographisch und computertomographisch eine 7 × 6 cm große Raumforderung im Bereich des Pankreaskopfes nachweisbar, die in Zusammenhang mit einem CA 19–9 von 329 U/l hochgradig verdächtig auf ein Pankreascarcinom war. Nach endoskopischer Einlage einer Pigtail-Drainage in den mit 2,7 cm deutlich dilatierten Ductus hepatocholedochus (DHC) machte eine anhaltende kreislaufwirksame Blutung aus dem DHC eine notfallmäßige partielle Duodenopankreatektomie nach Whipple erforderlich. Die histopathologische Untersuchung ergab ein sklerosierendes folliculäres centroblastisch-centrocytisches Lymphom i. S. eines niedrig malignen Non-Hodgkin-Lymphoms (NHL) des B-Zell-Systems. Eine sichere Abgrenzung des primären NHL des Pankreas zum Pankreascarcinom ist weder klinisch noch durch laborchemische oder bildgebende Verfahren sicher möglich. Die Impression des Ductus pancreaticus ohne Infiltration bei relativ großen Tumorprozessen sollte an eine extranodale Manifestation eines NHL denken lassen und eine histologische Abklärung herbeiführen.
    Notes: Summary. A 71-year-old patient had been suffering from pain-free obstructive jaundice for 8 weeks. Ultrasonography and computed tomography revealed an inhomogeneous mass (diameter 7 × 6 cm) in the head of the pancreas. In combination with a CA 19–9 of 329 U/l, the findings were highly suggestive of a pancreatic carcinoma. Endoscopic implantation of a pigtail drain into the dilated choledochal duct was performed. A partial duodenopancreatectomy (Whipple's procedure) became necessary because of continuous bleeding with hemodynamic disorders after endoscopic papillotomy. In the histopathological examination a low-grade malignant non-Hodgkin lymphoma of the pancreas (follicular centroblastic-centrocytic) was diagnosed. The differential diagnosis of primary pancreatic lymphoma from pancreatic carcinoma is usually impossible. Neither clinical nor laboratory nor imaging methods indicate to the correct diagnosis. In cases of relatively large pancreatic tumor masses and impression of the pancreatic duct without infiltration, a primary pancreatic lymphoma should be considered and a histological diagnosis by biopsy should be performed.

    Notes: Abstract: Tilt of hydrocarbon chains of lipid molecules with respect to membrane plane is commonly considered to characterize the internal structure of a membrane in the crystalline state. However, membranes in the liquid state can also exhibit tilt resulting from packing constraints imposed on the lipid molecules in diverse biologically relevant structures such as intermediates of membrane fusion, pores in lipid bilayers and others. We analyze the energetics of tilt in liquid membranes and its coupling with membrane bending. We consider three contributions to the elastic energy: constant tilt, variation of tilt along the membrane surface and membrane bending. The major assumption of the model is that the core of a liquid membrane has the common properties of an elastic continuum. We show that the variation of tilt and membrane bending are additive and that their energy contributions are determined by the same elastic coefficient: the Helfrich bending modulus, the modulus of Gaussian curvature and the spontaneous curvature known from previous studies of pure bending. The energy of a combined deformation of bending and varying tilt is determined by an effective tensor accounting for the two factors. In contrast, the deformation of constant tilt does not couple with bending and its contribution to the elastic energy is determined by an independent elastic constant. While accurate determination of this constant requires additional measurements, we estimate its value using a simplified approach. We discuss the relationships between the obtained elastic Hamiltonian of a membrane and the previous models of membrane elasticity.

    Notes: Summary To determine the effect of a taurine-enriched drink “Red Bull” on performance, 10 endurance-athletes performed three trials. After 60 min. cycling at approximately 70% VO2 max, the subjects pedalled to exhaustion on a cycle ergometer. During each exercise, the subjects received 500 ml of a test-drink after 30 min. submaximal cycling: “Red Bull” without taurine, without glucuronolacton (U1), “Red Bull” without taurine, without glucuronolacton, without caffeine (U2) and “Red Bull” original drink containing taurine, glucuronolacton and caffeine (U3). The heart rate level was significantly lower in U3 (p = 0,0031) 15 min. after application. The plasma catecholamines increased slightly from begin of exercise to 15 min. after application of the drinks in all trials but remained on a significantly lower level in U3 (epinephrine (p = 0,0011) and norepinephrine (p = 0,0003). Endurance time was significantly longer with “Red Bull” original in U3 (p = 0,015). The results of this study show a positive effect of a taurine-containing drink on hormonal responses which leads to a higher performance.

    Description / Table of Contents: Zusammenfassung Anhand von 5 Fallberichten wird über die Therapie der extrinsischen Harnleiterobstruktion durch die Einlage von 2 ipsilateralen inneren Doppel-J-Harnleiterschienen berichtet, nachdem die Einlage einer singulären inneren Harnleiterschiene trotz korrekter Lage zu keiner suffizienten Entstauung des Harnleiters und Nierenbeckenkelchsystems geführt hatte. Durch die erhöhte Festigkeit von 2 parallelen inneren Schienen konnten Obstruktion und Kinking vermindert und eine Rückbildung der Stauung des proximalen Hohlsystems erzielt werden. Ferner ermöglicht diese Technik neben dem Urinfluß innerhalb der Schienen einen vermehrten Fluß zwischen den Schienen, der als entscheidender Mechanismus der Urinpassage bei liegenden Harnleiterschienen angesehen wird. In Fällen extrinsischer Harnleiterkompression, in denen durch die Einlage einer singulären inneren Schiene keine suffiziente Rückbildung der Ektasie des vorgeschalteten intra- und extrarenalen Hohlraumsystems erreicht werden kann, sollte die simultane Einlage von 2 ipsilateralen inneren Doppel-J-Harnleiterschienen als technisch einfache, komplikationsarme Therapiealternative in Betracht gezogen werden, die einigen Patienten die Einlage eines perkutanen Nephrostomiekatheters oder invasivere Maßnahmen ersparen kann.
    Notes: Summary We present our experiences with the approach of placing 2 parallel ureteral Double-J stents simultaneously in extrinsically obstructed ureters. In all 5 reported patients therapy with single ureteral stents had failed despite correct stent position. With the increased stiffness of 2 parallel ureteral silicon 7F Double-J stents ureteral kinking and luminal compression could be reduced leading to sufficient reduction of hydronephrosis. This approach with potential space between the stents preserves urinary flow through as well as around the stents which is considered to be the most important mechanism in stented ureters. In cases of extrinsic ureteral obstruction with failure of a single stent the simultaneous use of 2 parallel Double-J ureteral stents should be taken into account as a technically easy therapeutic option. It may obviate percutaneous nephrostomy tube placement or more invasive therapy.
