    Description / Table of Contents: Summary The natural course of Osteonecrosis (ON) of the femoral head has yet not been evaluated sufficiently. Especially in the early forms of the disease (ARCO 0 to II) without collapse of the femoral head, useful information on the natural course could only be collected since the routine use of MR-imaging. The unspecific findings in ARCO stage I with negative radiographs are potential reversible. The “point of no return” already lies in the irreversible ARCO stage II in almost all cases. Prognosis for further progression for both early stages depends primarily on the extension and location of the lesion. Only the rare, small to medium sized lesions in the medial or central location may have a good prognostic course over a period of more than five years. The much more common large sized and lateral located lesions will have a probability of about 80 % to progress to femoral head collapse within two years. Conservative therapy with single protected weight bearing has shown bad results not significantly different from the natural course of the disease. Pulsed electromagnetic fields are still discussed controversially. Until now there is no MRI controlled study available in early ON, showing superior results compared to the easy and cost-effective core decompression therapy. The extra corporal shock wave therapy has shown spontaneous pain relief in early ON. Midterm results of this new therapeutical concept are not available yet. Hyperbaric oxygen therapy has shown to stimulate the repair process in an animal experiment. The preliminary clinical results of this time and cost consuming therapy are not convincing. Vasoactive drugs in combination with limited weight bearing may play a role in the conservative management of early ON (ARCO I and part of ARCO II) in the future. For the conservative therapy in ON several other substance are currently in clinical testing. The benefit of conservative therapeutical concepts in ON in the future can only be evaluated with prospective MRI controlled clinical studies. The use of cytokines in combination with surgical debridement and bone grafting of the necrotic area may be a possible therapeutical concept for the future.
    Notes: Zusammenfassung Der natürliche Verlauf der Osteonekrose (ON) des Hüftgelenkes im Erwachsenenalter ist bis heute noch nicht völlig geklärt. Besonders in den Frühstadien vor dem Gelenksflächeneinbruch (ARCO 0 bis II) sind erst seit dem Einsatz von Serien MRT-Studien entsprechende Kenntnisse gewonnen worden. Die unspezifischen Veränderungen im ARCO Stadium I mit negativem Röntgen sind potentiell reversibel. Der „point of no return“ liegt jedoch in den allermeisten Fällen bereits im irreversiblen Frühstadium ARCO II. Für beide Frühformen ist die Prognose einer weiteren Progression primär abhängig von der Ausdehnung und Lokalisation der Läsion. Lediglich bei den seltenen, kleinen bis mittelgroßen Läsionen mit medialer und zentraler Lokalisation ist ein prognostisch günstiger natürlicher Verlauf über einen Zeitraum von mehr als fünf Jahren zu erwarten. Bei den viel häufigeren, großen und lateral gelegenen Läsionen ist ohne Therapie in etwa 80 % mit einem Gelenkflächeneinbruch innerhalb von zwei Jahren zu rechnen. Die konservative Therapie mit alleiniger Entlastung des Hüftgelenks zeigt schlechte Ergebnisse, die sich nicht wesentlich vom natürlichen Verlauf unterscheiden. Der Einsatz der Magnetfeldtherapie wird bis heute kontrovers diskutiert, konnte jedoch bei Frühformen in keiner MRT kontrollierten Studie einen Vorteil gegenüber der einfachen und kostengünstigen Hüftkopfentlastungsbohrung zeigen. Die in jüngster Zeit eingesetzte Stoßwellentherapie zeigte eine spontane Schmerzerleichterung bei Frühformen. Mittelfristige Ergebnisse über den klinischen und bildgebenden Verlauf liegen jedoch noch nicht vor. Die Hyperbare Oxygenation kann die Reparaturkapazität positiv beeinflussen, die vorläufigen klinischen Ergebnisse dieser aufwendigen Therapie sind jedoch nicht überzeugend. Vasoaktive Medikamente könnten in Zukunft bei den Frühformen (ARCO I und eingeschränkt ARCO II) in Kombination mit mechanischer Entlastung eine Rolle spielen. Mehrere andere Substanzen zur konservativen Behandlung der ON sind derzeit in klinischer Erprobung. Zur exakten Standortbestimmung der konservativen Therapiekonzepte bei der ON sind prospektive MRT kontrollierte Studien erforderlich. Eine mögliches zukünftiges Behandlungskonzept könnte der Einsatz von Zytokinen in Kombination mit chirurgischer Sanierung des Nekroseareals darstellen.

We examined possible age- and gender-specific differences in the function and mass of left (LV) and right (RV) ventricles in 36 healthy volunteers using cine gradient-recalled echo magnetic resonance imaging. Subjects were divided into four groups (nine men and nine women in each): men aged under 45 years (32 ± 7), women aged under 45 (27 ± 6), men aged over 45 (59 ± 8), and women aged over 45 (57 ± 9). Functional analysis of cardiac volume and mass and of LV wall motion was performed by manual segmentation of the endocardial and epicardial borders of the end-diastolic and end-systolic frame; both absolute and normalized (per square meter body surface area) values were evaluated. With age there was a significant decrease in both absolute and normalized LV and RV chamber volumes (EDV, ESV), while LV and RV masses remained unchanged. Gender-specific differences were found in cardiac mass and volume (for men and women, respectively: LV mass, 155 ± 18 and 110 ± 16 g; LV EDV, 118 ± 27 and 96 ± 21 ml; LV ESV, 40 ± 13 and 29 ± 9 ml; RV mass, 52 ± 10 and 39 ± 5 g; RV EDV, 131 ± 28 and 100 ± 23 ml; RV ESV, 53 ± 17 and 33 ± 15 ml). Normalization to body surface area eliminated differences in LV volumes but not those in LV mass, RV mass, or RV function. Functional parameters such as cardiac output and LV ejection fraction showed nonsignificant or only slight differences and were thus largely independent of age and gender. Intra- and interobserver variability ranged between 1.4 % and 5.9 % for all parameters. Cine magnetic resonance imaging thus shows age- and gender-specific differences in cardiac function, and therefore the evaluation of cardiac function in patients should consider age- and gender-matched normative values.

    Notes: Abstract. Isotopes of dubnium (element 105) with mass numbers A = 256, 257, and 258 were produced by the reaction 209Bi(50Ti,xn) 259-xDb (x = 1, 2, 3) at projectile energies of (4.59-5.08) AMeV. Excitation functions were measured for the 1n, 2n and 3n evaporation channels. The same position of the excitation function was observed for the 1n channel as for the previously measured 1n channel of the reaction 208Pb(50Ti,1n)257Rf. The measured α-decay data of 257Db and its daughter products resulted in the identification of α-decaying isomeric states in 257Db and 253Lr. Two new isotopes, 256Db and 252Lr, were produced at the highest bombarding energies of 4.97 AMeV and 5.08 AMeV. They were identified by delayed α-α coincidences. The measured half-lives are (1.6+0.5 -0.3) s for 256Db and (0.36+0.11 -0.07) s for 252Lr. Besides α-decay, a spontaneous fission activity of T 1/2 = (2.3+1.1 -0.6) s was observed and attributed to an electron-capture branch of 256Db, which feeds the fissioning nucleus 256Rf. A branching ratio of 0.36±0.12 was obtained. The isotope 255Rf was produced by the reaction 207Pb(50Ti,2n)255Rf. Improved decay data have been obtained by means of α- and α-γ spectroscopy.

To study basic properties of the fusion reaction dynamics for heavy compound systems the partial-wave distribution $\sigma_\ell $ can be employed as an alternative to the classically used fusion/fission excitation functions. A variety of reactions leading to compound nuclei (CN) in the Pb region can be used to investigate features like the fusion-fission competition, the role of deformation in the fusion of heavy systems and a possible effect of the Z = 82 shell on the enhancement of evaporation residue (ER) production.

    Notes: Abstract. Alpha-decay of the new nuclide 184Bi has been studied in the complete-fusion reaction 93Nb(94Mo, 3n)184Bi at the velocity filter SHIP. The evaporation residues were separated in-flight and subsequently identified on the basis of recoil- $\alpha$ , recoil- $\alpha$ - $\gamma$ analysis and excitation functions measurements. Two $\alpha$ -decaying isomeric states in 184Bi with half-life values of 13(2) ms and 6.6(1.5) ms were identified. The $\alpha$ -branching ratio of 180Tl was deduced for the first time as $b_\alpha = (2 --12)\%$ . Improved data on the fine-structure $\alpha$ -decay of 186Bi were obtained in the 93Nb(95Mo, 2n)186Bi reaction. A similarity of the decay energies and half-life values of 184,186Bi is pointed out and a possible explanation for this effect is suggested.

    Notes: Abstract. Detailed fine-structure $\alpha$ -decay studies of 188,190Bi were performed using the complete-fusion reactions of 50,52Cr ions with a 142Nd target at the velocity filter SHIP. The evaporation residues were separated in-flight and subsequently identified on the basis of recoil- $\alpha$ , recoil- $\alpha$ - $\gamma$ /X-ray and excitation function measurements. Improved data on the $\alpha$ -decay of 188,190Bi were obtained and a number of new low-lying excited states in the daughters 184,186Tl were observed. An intruder, presumably 10- state was identified in 184Tl, extending the systematics of such states in the odd-odd Tl nuclei beyond the neutron mid-shell at N = 104. Some technical aspects of experiment at recoil separators are also discussed.

    Notes: Abstract: The even-even nucleus 270110 was synthesized using the reaction 64Ni + 207Pb. A total of eight α-decay chains was measured during an irradiation time of seven days. Decay data were obtained for the ground-state and a high-spin K isomer. The new nuclei 266Hs and 262Sg were identified as daughter products after α-decay. Spontaneous fission of 262Sg terminates the decay chain. The measured data are in agreement with calculations using the macroscopic-microscopic model and with self-consistent HFB calculations with Skyrme-Sly4 interaction.
