    Notes: Background The production of IgE in B lymphocytes is down-regulated by IFN-γ. IL-12 induces IFN-γ production by T lymphocytes and natural killer cells by binding to its specific receptor. RNA editing is a post-transcriptional modification.Objective Here we show that the RNA editing of IL-12 receptor (R) β2 is associated with atopy.Methods Atopic patients and non-atopic healthy controls were studied. Fragments of IL-12R β2 cDNA and genomic DNA were amplified and sequenced. Furthermore, the function of the IL-12R β2 chain was investigated.Results Sequence analysis of the cDNA clones representing IL-12R β2 mRNA transcripts revealed a C-to-U conversion at nucleotide 2451 (Ala 604 Val) on exon 13 in some atopic patients. Surprisingly, sequence analysis of their genomic DNA showed no 2451 C-to-T (Ala 604 Val) mutation. We concluded that the observed C-to-U mismatch in the cDNA clone is due to a post-transcriptional modification, RNA editing. The C-to-U conversion was observed in 21 (20.6%) of 102 atopic patients, whereas this conversion was observed in only 4 (3.8%) of 104 non-atopic subjects (P〈0.001). IFN-γ production by peripheral blood mononuclear cells (PBMCs) stimulated with IL-12 in the subjects with the C-to-U conversion was significantly lower than that in the subjects without the C-to-U conversion. In atopic patients with the C-to-U conversion, PBMCs faintly showed the tyrosine phosphorylation of Stat4, and the IgE production by PBMCs was not suppressed by IL-12 whereas it was suppressed by IFN-γ.Conclusions The RNA editing of IL-12R β2, 2451 C-to-U (Ala 604 Val) conversion causes impairment of the IL-12 signal cascade and the subsequent reduction in IFN-γ production, resulting in the impaired down-regulation of IgE production. This is the first report indicating that atopy is associated with RNA editing.

    Notes: Increased sensitivity of cough reflex is a fundamental feature of bronchodilator resistant non-productive cough associated with eosinophilic tracheobronchitis. Our hypothesis is that cough sensitivity is increased by airway allergic reaction characterized by airway eosinophilic inflammation. The aim of this study was to elucidate the hypothesis and clarify the characteristics of the increased cough sensitivity.Number of coughs elicited by inhalation of increasing concentrations of capsaicin (10−8, 10−6 and 10−4 m) was counted 24 h after an aerosolized antigen or saline in actively sensitized or non-sensitized (naive) conscious guinea pigs and then bronchoalveolar lavage was performed. The cough response was also measured 1 day before and 1, 2, 3, 5 and 7 days after an aerosolized antigen challenge in sensitized or naive animals. In addition, effect of procaterol (0.1 mg/kg), atropine (1 or 10 mg/kg), phosphoramidon (2.5 mg/kg) given intraperitoneally 30 min before the capsaicin challenge or capsaicin desensitization on the cough response was examined. Furthermore, the thromboxane A2 (TXA2) receptor antagonist S-1452 in a dose of 0.01 or 0.1 mg/kg or vehicle (saline) was given intraperitoneally at 24 and 1 h before the measurement of cough response.Number of coughs caused by capsaicin was extremely increased 24 h after an antigen challenge in sensitized guinea pigs compared with a saline or an antigen challenge in naive animals or a saline challenge in sensitized animals. The increased cough response disappeared at 3–7 days after the antigen challenge. Eosinophils in bronchoalveolar lavage fluid obtained after the measurement of capsaicin-induced coughs, which was performed 24 h after the antigen challenge, were significantly increased in sensitized guinea pigs. The eosinophil count was significantly correlated to the number of capsaicin-induced coughs. Procaterol or atropine did not alter the antigen-induced increase of cough sensitivity, whereas atropine did reduce the cough response in naive animals. Phosphoramidon increased the number of capsaicin-induced coughs in naive guinea pigs but not in sensitized and antigen-challenged animals. Capsaicin desensitization decreased the cough response in both antigen-challenged sensitized guinea pigs and naive animals. S-1452 reduced the antigen-induced increase of cough response in sensitized guinea pigs, but not in naive animals.Airway allergy accompanied with airway eosinophilia induces transient increase in cough sensitivity, which is not mediated by bronchoconstriction. The increased cough sensitivity may result in part from inactivation of neutral endopeptidase and TXA2, one of the inflammatory mediators.

    Notes: Background Several studies have shown that interleukin (IL)-4 and interferon-gamma (IFN-γ) are important for the regulation of immunoglobulin E (IgE) production and that IL-18 and IL-12 induce IFN-γ.Objective IFN-γ production in response to IL-18 or IL-12 stimulation was investigated in peripheral blood mononuclear cells (PBMCs) of atopic patients with various levels of serum IgE.Methods Cytokine production from PBMCs was measured following stimulation with a non-specific stimulator (phytohemagglutinin: PHA), IL-18 or IL-12 in 12 healthy controls and 26 atopic patients with various serum IgE levels.Results IFN-γ production by IL-18-stimulated PBMCs was positively correlated with IFN-γ production by IL-12-stimulated PBMCs (P 〈 0.05). However some atopic patients showed discrepancy between the levels of IFN-γ production stimulated by IL-12 and by IL-18.Conclusions The results shown here suggest the presence of abnormalities in the IL-12 and/or IL-18 signalling pathways, such as genetic defects in the atopic patients.

    Notes: Common variable immunodeficiency (CVID) is a primary antibody deficiency syndrome characterized by defective B-cell maturation and antibody formation resulting in low serum antibody levels of all immunoglobulin (Ig) isotypes. To investigate the pathogenesis of CVID, we developed a set of competitive polymerase chain reaction for membrane-bound Ig heavy chain (mHC) mRNAs for IgM, IgG and IgA. Data on three children with CVID in group A of Bryant's classification were analysed. All the three mHC mRNA levels in Patient 1 were almost same as those in healthy controls. In Patient 2, mHC mRNA for IgM was detected at a level similar to that in controls, but mHC mRNAs for IgG and IgA heavy chains were not detected. In Patient 3, all the three mHC mRNAs were undetectable. Our data suggest that a different molecular basis exists in these patients with CVID even though all belong to group A of Bryant's classification. Use of our method facilitates a better understanding of molecular events in CVID patients and may be useful for precise classifications of CVID.

    Description / Table of Contents: Résumé  On a examiné les résultats oncologiques et fonctionnels de 33 patients ayant reçu une radiothérapie peropératoire pour un ostéosarcome d’une extrémité. Sur les sept cas de récidive locale identifiés, six se situaient dans une région apparemment non irradiée, ce qui indique que la défaillance locale pourrait être due à un défaut de détermination du territoire irradié. Sur les 33 patients, 14 ont reçu une endoprothése et 7 été amputés après la radiothérapie. Les tumeurs irradiées ont été laissées in situ chez les 12 patients restants. Dans ce dernier groupe, l’examen radiologique n’a révélé aucune dégénérescence au niveau des articulations. Des complications locales ont été observées chez 26 patients sur 33. La complication la plus problématique était la fracture de l’os irradié. La consolidation par enclouage médullaire a donné des résultats encourageants pour la prévention des fractures. Bien que la radiothérapie peropératoire soit efficace pour la maitrise locale de l’ostéosarcome des extrémités, la sélection judicieuse des patients et l’amélioration de la procédure sont nécessaires pour obtenir un membre fonctionnel à long terme.
    Notes: Abstract  The outcome following intra-operative radiation therapy in the treatment of osteosarcoma in the extremity in 33 patients was evaluated for oncological and functional results. Local recurrence occurred in seven cases, six of which were in a non-irradiated region, indicating inappropriate planning of the radiation field. Twenty-one patients underwent either prosthetic replacement (14) or amputation (7). Irradiated tumours were left in situ in the remaining 12 patients. In this latter group no degenerative joint changes were observed radiologically. Twenty-six patients experienced local complications, of which fracture of the irradiated bone was the most significant. Associated intramedullary nailing showed encouraging results in preventing fracture. Although IORT is effective for the local control of osteosarcoma in extremities, critical patient selection and improvements of treatment protocol are required in order to obtain a satisfactory outcome.
