    Notes: Abstract Melorheostosis is a benign sclerosing dysplasia with a very unusual and characteristic roentgenographic appearance. We reported a patient with melorheostosis in the left lower extremity followed up for 14 years. Although the extraosseous lesions only slightly enlarged, the bone scintigraphy showed the activity of the metabolism to be still high.

    Notes: Abstract The purpose of this investigation was to study the fundamental compositional dependence of bioactivity for bioactive glasses by adding a third component such as Na2O, B2O3, Al2O3, Fe2O3, P2O5 or F− to binary calcium silicate glass. Rectangular specimens of these six kinds of calcium silicate glasses containing each 3 wt ratio third component were implanted into the tibias of mature rabbits for 8 or 25 weeks. Contact microradiograms and SEM-EPMA showed that all glasses, except for Fe2O3-containing glass, formed a Ca, P-rich layer in combination with a Si-rich layer on their surfaces within 8 weeks. The other four kinds of glass except that containing Al2O3 became attached to bone through these layers within 8 weeks, and even Al2O3-containing glass became attached to bone by 25 weeks. Fe2O3-containing glass did not form these layers, nor did it attach to bone even at 25 weeks. A detachment test, performed 8 weeks after implantation, showed that only glasses containing B2O3, P2O5 and F− bonded tightly with bone. Glass containing B2O3 maintained a tight bond at 25 weeks, but the other two kinds of glass showed decreased loads at 25 weeks.

    Description / Table of Contents: Zusammenfassung Wir operierten 10 Patienten mit den folgenden seltenen Wirbelsäulentumoren: drei solitäre Plasmacytome, eine aneurysmale Knochenzyste, ein Chordom, einen Riesenzelltumor, ein Hämangiom, ein gutartiges Osteoblastom und ein Chondrosarkom. Diese Tumoren fanden sich in der Halswirbelsäule (ein Fall), in der Brustwirbelsäule (fünf Fälle), in der Lendenwirbelsäule (zwei Fälle) und im Sakrum (zwei Fälle). Es waren fünf männliche und fünf weibliche Patienten mit einem Durchschnittsalter von 43,1 Jahren. In sechs Fällen wurde Harrington-Instrumentation verwendet, in zwei Fällen Zielke-Instrumentation, eine Kombination von beiden in einem Fall und Kirschner-Draht für den letzten Fall. In zwei Fällen wurde nach totaler Resektion des Sakrum eine ilio-lumbale Verbindung mittels Harrington-Instrumentation durchgeführt. Die Fälle wurden durchschnittlich über fünf Jahre nachverfolgt. Der vorliegende Bericht beschreibt diese seltenen Fälle und betont die Wirksamkeit instrumenteller Chirurgie bei der Rekonstruktion der Wirbelsäule nach ausgedehnten Eingriffen.
    Notes: Summary We performed instrumentation surgery on ten patients with the following rare primary spinal tumors; three solitary plasmacytomas, an aneurysmal bone cyst, a chordoma, a giant-cell tumor, a hemangioma, a solitary bone cyst, a benign osteoblastoma, and a chondrosarcoma. These tumors were found in the cervical spine (one case), thoracic spine (five cases), lumbar spine (two cases), and sacrum (two cases). The patients consisted of five men and five women, and their average age was 43.1 years. Harrington instrumentation was used in six cases, Zielke instrumentation in two, a combination of both in one case, and a Kirshner wire in one. In two cases, iliolumbar fusion was performed with Harrington instrumentation, after total resection of the sacrum. Average duration of the follow-up period was 5 years. This report describes these rare cases and emphasizes the effectiveness of instrumentation surgery in the reconstruction of the spine after extensive surgery.

    Notes: Summary Seven cases of intraspinal inclusion cysts, surgically treated between 1970 and 1984, are reported in this paper. Four were epidermoid cysts and three dermoid. Two cysts were located in the thoracic spine (one dermoid and one epidermoid) and five in the lumbosacral region (three epidermoid and two dermoid). Associated spinal dysraphia was noted in five cases. Congenital dermal sinus was found in two cases, spina bifida cystica in one, spina bifida occulta in one, and diastematomyelia in one case. The average postoperative follow-up period was 5 years. No neurological symptoms were found in the five patients who received early surgical treatment, but for the two patients whose operations were late, there were some residual neurological deficits. Therefore, early and accurate diagnosis and radical surgical extirpation are emphasized.

    Description / Table of Contents: Résumé  On a examiné les résultats oncologiques et fonctionnels de 33 patients ayant reçu une radiothérapie peropératoire pour un ostéosarcome d’une extrémité. Sur les sept cas de récidive locale identifiés, six se situaient dans une région apparemment non irradiée, ce qui indique que la défaillance locale pourrait être due à un défaut de détermination du territoire irradié. Sur les 33 patients, 14 ont reçu une endoprothése et 7 été amputés après la radiothérapie. Les tumeurs irradiées ont été laissées in situ chez les 12 patients restants. Dans ce dernier groupe, l’examen radiologique n’a révélé aucune dégénérescence au niveau des articulations. Des complications locales ont été observées chez 26 patients sur 33. La complication la plus problématique était la fracture de l’os irradié. La consolidation par enclouage médullaire a donné des résultats encourageants pour la prévention des fractures. Bien que la radiothérapie peropératoire soit efficace pour la maitrise locale de l’ostéosarcome des extrémités, la sélection judicieuse des patients et l’amélioration de la procédure sont nécessaires pour obtenir un membre fonctionnel à long terme.
    Notes: Abstract  The outcome following intra-operative radiation therapy in the treatment of osteosarcoma in the extremity in 33 patients was evaluated for oncological and functional results. Local recurrence occurred in seven cases, six of which were in a non-irradiated region, indicating inappropriate planning of the radiation field. Twenty-one patients underwent either prosthetic replacement (14) or amputation (7). Irradiated tumours were left in situ in the remaining 12 patients. In this latter group no degenerative joint changes were observed radiologically. Twenty-six patients experienced local complications, of which fracture of the irradiated bone was the most significant. Associated intramedullary nailing showed encouraging results in preventing fracture. Although IORT is effective for the local control of osteosarcoma in extremities, critical patient selection and improvements of treatment protocol are required in order to obtain a satisfactory outcome.

    Notes: Zirconia has received special attention, mainly because of its high strength and toughness. However, there is some controversy about the time-dependent deterioration of its mechanical properties. To examine the change in mechanical properties of zirconia ceramics in vivo and in vitro, tetragonal zirconia polycrystal pieces were introduced into the medullary cavity of the tibia in Japanese rabbits and animals were sacrificed after 2, 4 and 6 weeks and 6, 12, and 30 mo, respectively. Alumina ceramic and hydroxyapatite (HAP) pieces were used as controls to investigate the differences in biocompatibility. Zirconia showed a bending strength of over 1000 MPa initially, and little time-dependent change in strength was found in both in vivo environments. x-Ray analysis showed little change in the transformation rate, i.e., less than 5 mol % in vivo and in vitro over a period of 3 years. To estimate time-dependent changes in zirconia over a longer period, zirconia pieces were placed in 95°C saline solution for over 3 years and their mechanical properties examined at chosen intervals. No serious decrease of bending strength was found over the 3-year period under these conditions. It is concluded that zirconia can be used clinically because it retains a bending strength of over 700 MPa under any experimental conditions for over 3 years. © 1993 John Wiley & Sons, Inc.
