    Description / Table of Contents: Summary A new method for the measurement of renin in human plasma is described. The method is based on the introduction of the internationally available renin standard of the Medical Research Council (MRC) London, as a calibration system. Thus, some principal disadvantages of methods expressing results in renin reaction velocity (angiotensin generation rate) only are avoided. Both renins, unknown and standard, react with a sheep substrate preparation and are handled identically throughout the whole procedure including the angiotensin I radioimmunoassay (RIA). The plasma renin concentration (PRC) is given in 10−6 MRC-renin units (µU/ml). Results: the renin standard is free of angiotensin, angiotensinases, and angiotensinogen; it is stable on storage. Identical enzyme kinetics are shown for both renins. An interference between endogenous and exogenous substrate could be avoided. The potentially harmful influences of proteins from the enzyme incubation mixture on the RIA dose response curve are shown. The use of an angiotensin I calibration system could be omitted. Using a standard renin dilution from 250–0.9 µU/ml almost the full biological range is covered. When giving an unrestricted diet the preliminary normal values of PRC are 21.9±12.6 µU/ml in recumbent and 40.1±18.8 µU/ml in upright position (n=16,x±s, age 20–35 years). Earlier findings of age-dependancy of PRC were confirmed.
    Notes: Zusammenfassung Es wird eine neue Methode zur Messung von Renin in menschlichem Plasma beschrieben. Die Methode basiert auf der Einführung des Renin-Standards des Medical Research Council (MRC), London als Eichsystem und vermeidet damit die prinzipiellen Nachteile von Verfahren, die Renin nur als Angiotensinbildungsgeschwindigkeit erfassen. Prinzip der Methode ist es, Standard- und Plasma-Renin während der Enzymreaktion (mit Schafsubstrat) und während des Angiotensin I-Radioimmunoassays (RIA) identisch zu behandeln. Die Plasma-Renin-Konzentration (PRC) wird in 10−6 MRC-Renineinheiten (µU/ml) angegeben. Ergebnisse: Der Renin-Standard ist stabil und frei von Angiotensin, Angiotensinasen und Angiotensinogen. Er zeigt gleiche Enzymcharakteristika wie Plasma-Renin. Eine Interferenz von endo- und exogenem Substrat konnte vermieden werden. Mögliche, gravierende Störungen des RIA durch Proteine der Enzymreaktionsansätze wurden nachgewiesen. Eine Eichung über einen Angiotensin I-Standard kann vermieden werden. Bei Anwendung einer Standard-Renin-Verdünnung von 0,9–250 µU/ml wird nahezu der gesamte biologische Bereich erfaßt. Unter freier Kost betrugen die vorläufigen Normalwerte für 30–35jährige in Ruhe 21,9±12,6 µU/ml, nach Orthostase 40,1±18,8 µU/ml (x±s,n=16). Frühere Beobachtungen über eine Altersabhängigkeit der PRC wurden bestätigt.

    Notes: Abstract Indium oxide films doped with tin (ITO-films) have been hf-sputtered from an 80 at-%In2O3/20 at-%SnO2 target onto glass substrates. The sputter atmosphere contained mainly argon (10−2Torr) with addition of oxygen (0≦p O 2≦2·10−2Torr). The sputtered films aren-conductors. The conductivity and density of charge carriers depend on the oxygen content of the sputter gas. They could be varied by two orders of magnitude. In air or in oxygen atmosphere the films oxidize at the surface and for a certain depth beneath the surface, thus decreasing the conductivity. The Hall mobility of the sputtered films is smaller (≈10 cm2V−1 s−1) than one observes at ITO films produced by CVD sparaying or other methods. The conductivity of as sputtered films approached maximum values of about 1000Ώ−1cm−1.

    Notes: Abstract. A hydrodynamic thrombectomy catheter was prospectively evaluated for the treatment of recently thrombosed vessels. A total of 52 consecutive patients (42 males and 10 females; mean age 64 ± 15 years) presenting with acute or subacute occlusion of dialysis shunts (n = 25), peripheral bypass (n = 14) or native arteries (n = 15) were treated with the Hydrolyser (Cordis Europa NV, Roden, The Netherlands). Mean occlusion time was 4 days (range 1–17 days) and mean thrombus length 19 ± 11 cm. The Hydrolyser was effective and fast in removing thrombus, regardless of the thrombus length. No major complications were reported. The immediate procedure success rates were 82, 100, 87 and 79 % for Brescia Cimino, dialysis shunt, native arteries and bypass grafts, respectively. Adjunctive thrombolysis (applied for persistence of residual thrombus or thrombosed distal vessels too small for hydrolytic thrombectomy) was required in 4 % of thrombotic dialysis shunts, in 20 % of native arteries and in 50 % of bypass graft occlusions. On angiographic controls, distal embolizations were reported only in native arteries (13 %) and bypasses (14 %); all were successfully treated percutaneously, except for one case treated by Fogarty balloon. Cumulative primary patency rates were respectively at 6 months 56, 62, 78 and 65 % for each indication. We conclude from this preliminary clinical study that hydrodynamic thrombectomy with a Hydrolyser is a promising technique to treat acute occlusions. This device can reduce complications as well as the time required to remove large amounts of thrombus and the use of expensive thrombolytic drugs.

    Notes: Abstract. The aim of this study was to evaluate mid- and long-term results of ultrasonically guided ethanol injection into parathyroid adenomas. From 1988 to 1996, 27 patients (mean age 77 ± 13 years) were treated for parathyroid adenomas by percutaneous ethanol injection. The survey included clinical information, plasma calcium, phosphorus and PTH (1–84) evaluation. Follow-up lasted 22.6 ± 10 months. No major complications were observed for 63 ethanol injections. Biochemical recovery was 58 %, biochemical improvement at 3 months was 33 %, and failure was 7 %. Four of 15 cured patients presented a recurrence of the disease 1 or 2 years after the first treatment. Ultrasonically guided ethanol injection can be useful in the treatment of parathyroid adenomas when surgery is not possible. The immediate results are interesting but not as good as those obtained with surgery. A regular biochemical survey is necessary so that recurrences can be recognized and treated at an early stage.

    Description / Table of Contents: Summary Ochronosis is a rare disorder of tyrosin catabolism with an autosomal, recessive trait. Clinical signs predominantly affect the locomotor system. Inflammatory episodes in large joints can mimick Rheumatoid Arthritis (RA). Brownish discoloration of sclerae and cartilage is characteristic, diagnosis is confirmed by typical changes of spinal X-rays and the finding of homogentisic acid in urine samples. We describe a case of a 53 year old female patient with ochronosis and a second diagnosis of RA.
    Notes: Zusammenfassung Die Ochronose ist eine seltene, autosomal rezessiv vererbte Störung des Tyrosinabbaus. Ihre Symptome manifestieren sich überwiegend am Bewegungsapparat. Entzündungsschübe insbesondere an den großen Gelenken können eine Chronische Polyarthritis (CP) vortäuschen. Charakteristisch sind bräunliche Verfärbungen der Skleren und Knorpel, die Diagnose kann dann durch typische Röntgenveränderungen der Wirbelsäule und den Nachweis von Homogentisinsäure im Urin gesichert werden. Wir beschreiben den Fall einer 53-jährigen Patientin mit Ochronose und einer zusätzlichen CP.

    Notes: SYNOPSIS. Electron microscope observations on Thelohania sp. in the shrimp Pandalus jordani support the view that the Golgi complex in Microsporida is a “classical” one, composed of vesicular, vacuolar, and cisternal components. During development of the sporoblast, a portion of the Golgi complex is seen as an electron-dense reticulum enmeshing the core of the polar filament. Associated with the reticulum are electron-dense bodies. The reticulum and “dense bodies,” reported in several previous publications, have not been well understood and have been given a variety of names. The evidence favors the view that these structures have secretory activity in which the reticulum concentrates or synthesizes material, some of which takes the form of membrane-bounded granules. It is suggested that the most appropriate name for the reticulum is “reticulum golgien,” and that the correct name for the “dense bodies” is the standard cytologic term, “secretion granules.” The secretion granules apparently remain in the posterior part of the spore, and may be stored there for some as yet undetermined use.

    Description / Table of Contents: Summary Case: A newborn infant was treated in hospital because of suspected pneumonia and renal insufficiency at the third day post partum. Seven days later, shortly after having been nursed by her mother, the newborn vomited an eucalyptus oil-smelling liquid, which was collected in a diaper. This liquid and the liquid obtained from a gastrolavage performed immediately afterwards were secured in air-tight bottles. The family of the newborn laid a charge against the hospital for bodily harm caused by negligence reproaching the hospital for having wilfully given the newborn a toxic substance. Discussion: Only by means of advanced analytical methods (mikroheadspace GC/MS and GC/MS) which permit a subtle separation of mixtures of volatile oils, the reproach against the hospital could be refuted and the suspicion against the mother confirmed. In connection with the clinical picture which is discussed with reference to the relevant literature, and the investigation results of the police, the mother was deprived of the custody of her child, and the child was placed into the custody of the youth welfare office.
    Notes: Zusammenfassung Fall: Berichtet wird über ein Neugeborenes, bei dem in der Klinik am 3. Tag post partum eine plötzliche Verschlechterung des Gesundheitszustands mit Verdacht auf Pneumonie und Niereninsuffizienz auffiel. 7 Tage später erbrach das Neugeborene, kurz nachdem es erstmalig wieder durch die Mutter versorgt worden war, eine stark nach Eukalyptus riechende Flüssigkeit, die in einer Windel aufgefangen wurde. Diese wurde ebenso wie die unmittelbar danach gewonnene Magenspülflüssigkeit in luftdicht schließenden Behältnissen gesichert. Mit dem Vorwurf der vorsätzlichen Beibringung einer toxischen Substanz konfrontiert, erstattete die Familie des Neugeborenen Anzeige gegen die Klinik wegen fahrlässiger Körperverletzung. Diskussion: Nur durch den Einsatz moderner Analysenmethoden (Mikroheadspace-GC/MS und GC/MS) mit der Möglichkeit der subtilen Trennung von Gemischen ätherischer Öle konnte der Vorwurf gegen die Klinik entkräftet und der Verdacht gegen die Mutter bestätigt werden. Im Zusammenhang mit dem klinischen Bild, das im Vergleich zur einschlägigen Literatur diskutiert wird, und den polizeilichen Ermittlungsergebnissen wurde der Mutter das Sorgerecht entzogen und das Kind in die Obhut des Jugendamts entlassen.
