    Description / Table of Contents: Zusammenfassung. Zwischen 1980 und 1995 wurden an unserer Abteilung 207 Patienten mit 219 ausgedehnten Decubitalulcera im Beckenbereich operativ behandelt. Davon wurden 80 Patienten, welche im Zeitraum von 1992–1995 operiert wurden, nachuntersucht. Die postoperative Komplikationsrate lag bei 22 %, die Rezidivrate bei 27 %. Zur Qualitätssteigerung wurde das prä- und postoperative Management standardisiert. In dieser Arbeit wird der besondere Stellenwert dieser präoperativen Vorbereitung, die Kriterien der Lappenwahl, das chirurgische Vorgehen und die postoperative Nachbetreuung diskutiert.
    Notes: Summary. Between 1980 and 1995, 207 patients with 219 extensive pelvic pressure sores underwent reconstructive surgery at our department. Eighty patients operated upon between 1992 and 1995 were followed up. The postoperative complication rate was 22 % and the relapse rate 27 %. Pre- and postoperative management has been standardized for the purposes of quality control. The value of preoperative management, choice of flap, operative procedure and postoperative care are discussed in this paper.

    Description / Table of Contents: Zusammenfassung. Die notfallmäßige Versorgung schwerer Extremitätenverletzungen mittels freier Lappenplastik stellt ein modernes, aber nicht oft praktiziertes Behandlungskonzept dar. Die positiven Erfahrungen mit diesem Konzept und die Vorteile gegenüber dem konventionellen Vorgehen werden erläutert. Das initiale radikale Débridement ist der entscheidende Teil der Operation, gefolgt von der Osteosynthese. Der freie Gewebetransfer wird entsprechend den Erfordernissen des Extremitätendefekts ausgewählt und mikrochirurgisch angeschlossen. Durch dieses moderne Behandlungskonzept des freien Gewebetransfers im Akutstadium konnte eine frühe Rehabilitation der Extremitätenfunktion erzielt werden. Der Nachbeobachtungszeitraum reicht von 3 Monaten bis zu 8 Jahren. Es wurde weder ein Lappenverlust beobachtet noch traten tiefe Wund- oder Knochenentzündungen auf. Durch das vorgestellte Behandlungskonzept konnte eine Reduktion der Lappenverlustrate, eine verringerte assoziierte Morbidität und funktionell verbesserte Wiederherstellung verglichen mit den Literaturangaben bei konventionellem Vorgehen erzielt werden.
    Notes: Summary. The concept of emergency free tissue transfer for severe extremity injuries represents a cutting-edge technology. We discuss our very positive results with this technique. The conceptual reasons for these favorable results, compared with conventional approaches, are also discussed. An initial, radical debridement is the most important part of the operation. This is then followed by osteosynthesis. The correct type of free tissue transfer is chosen according to the requirements of the soft tissue defect. Qualitatively better results with earlier definitive rehabilitation were achieved with free tissue transfer performed during the acute stage of limb wound treatment. The follow-up period ranged from 3 months to 8 years. We experienced neither flap loss, osteomyelitis, nor severe wound infection. Using the modern concept of emergency free tissue transfer, we reduced the rate of flap loss and associated morbidity while achieving a functionally improved reconstruction.

    Notes: Abstract. A technique to facilitate correct preoperative marking for an en bloc resection in superiorly based mammaplasty is presented. A temporary triangular suture is placed to control the estimated resection lines for the removal of the medial and lateral tissue block. The described technique helps to avoid the formation of medial and lateral dog ears and the unpleasant and visible scars that result.

    Notes: Abstract  The rate of seroconversion for antibody to Chlamydia pneumoniae was analysed in blood samples of 26 vertically HIV-1 infected children and 14 seroreverter children (HIV-negative children born to HIV-positive mothers) during a 3-year study period. Seroconversion for Chlamydia pneumoniae was found in 13 of 26 HIV-1 infected children and in 1 of 14 in the seroreverter group (P=0.013). A lower mean CD4+ cell count and p24 antigen positivity at enrolment were significantly associated with seroconversion for Chlamydia pneumoniae. Signs and symptoms of acute respiratory infection were recorded in the 30 to 40 days preceding collection of the blood samples showing seroconversion for Chlamydia pneumoniae in 8 of 13 HIV-1 infected children and in the single seroreverter. This study confirms the potential role of Chlamydia pneumoniae in the pathogenesis of respiratory tract infections in HIV-1 infected subjects.

    Notes: Abstract Introduction: A simple technique is presented here for temporarily covering massive, full-thickness, abdominal-wall defects, when they cannot be closed directly. Methods: The exposed viscera can be covered with a meshed split-thickness skin graft to close the wound and seal off the abdominal cavity from the outside. Once the patient's general condition improves, the epidermal layer of the mesh graft should be removed by dermabrasion to minimize the risk of epidermal cysts, and the defect should be closed either by primary closure or by a local or free flap, depending on the defect size.

    Notes: Abstract PURPOSE: We report the case of a 69-year-old man who had suffered a third-degree burn injury of the buttocks close to the perianal region at the age of 2 years. Although initial attempts for conservative treatments failed, wound healing was achieved after numerous skin grafts. However, after prolonged healing, the patient developed scar contracture, resulting in total effacement of the gluteal folds and natal crease, and formation of a gluteal pseudoampulla and a pseudoanus. As a result of misinterpretation of the local anatomy, for the next 50 years the patient had to defecate into the gluteal pouch and empty this pseudoampulla by manually compressing the buttocks and irrigating with chamomile tea. METHOD: After a thorough examination revealed the presence of a normal, uninjured anus hidden by the scarred pouch, we excised this burn scar to expose the unburned perianal tissue and covered the resulting defect with a meshed, split-thickness skin graft. RESULT: Anorectal function was preserved, the anatomic contour was restored, and primary wound healing was achieved after one single-stage operation. CONCLUSION: To prevent such deformities, we recommend early excision and skin grafts as initial treatment, as opposed to conservative treatment, and thorough follow-up examinations to avoid misinterpretation of such a complex, altered local anatomy.

    Notes: The reaction of the carbonyl-functionalized phosphaalkenes RC(O)P=C(NMe2)2 [R = tBu (2a), Ph (2b)] with protic acids and alkylating reagents occurred at the two-coordinate phosphorus atom to give the phosphanyl-substituted carbocations 3a,b and 4a,b. In contrast, treatment with Me3SiOSO2CF3 resulted in attack at the oxygen atom by the silyl group, and the formation of [RC(OSiMe3)=PC(NMe2)2]SO3CF3 (5a,b). Similarly, the Lewis acids B(C6F5)3, Al(tBu)2Cl and AlMe3 were ligated to the oxygen atom of the carbonyl group. Two equivalents of GaMe3 were added to the oxygen and phosphorus atom of the phosphaalkene to yield the thermolabile complexes [RC(OGaMe3)=P(GaMe3)C(NMe2)2] (10a,b). In contrast, one molecule of InMe3 was bound to the phosphorus center of the phosphorus compound. Reaction of the phosphaalkenes with [Ni(CO)4], [Fe2(CO)9] or [{(Z)-cyclooctene}Cr(CO)5] also took place at the pnictogen atom, resulting in complexes of the type [RC(O)P{M(CO)n}C(NMe2)2] (R = tBu, Ph; M = Ni, n = 3; Fe, n = 4; Cr, n = 5). The chemical transformations reported here underline the versatile chemistry of phosphaalkenes and emphasize a relationship between carbonyl-functionalized phosphaalkenes and the well-investigated class of phosphorus ylides. X-ray structures of compounds 6b, 7b*, 10a, 11a and 12a are reported.
