    Notes: Summary The association between diurnal blood pressure variation and diabetic nephropathy was assessed in four groups of Type 1 (insulin-dependent) diabetic patients who underwent 24-h ambulatory blood pressure monitoring using an oscillometrie technique. Patients with nephropathy, who had never been treated for hypertension (group D3,n = 13), were individually matched for age, sex and diabetes duration to a group of microalbuminuric patients (D2,n = 26), to normoalbuminuric patients (D1,n = 26) and to healthy control subjects (C,n = 26). Group D3 was also compared to patients with advanced nephropathy receiving treatment for hypertension, mainly a combination of angiotensin converting enzyme inhibitors, metoprolol and diuretics (D4,n = 11). In group D3 24-h diastolic blood pressure (85 ± 8 mm Hg) was comparable to the results obtained in D4 (85 ± 8 mm Hg) but significantly higher than in D2 (78 ± 7 mm Hg), D1 (73 ± 7 mm Hg) and C (73 ± 7 mm Hg,p 〈 0.05, Tukey's test). The night/day ratio of diastolic blood pressure was higher in D3 (86 ± 5 %) and D2 (85 ± 7%) than in C (80 ± 7 %,p 〈 0.02). This ratio was also elevated in group D4 (94 ± 8%) compared to D3 (p 〈 0.05) corresponding to a marked smoothing of the diurnal blood pressure curve. The 24-h heart rate (beats per min) was significantly elevated in D3 (84 ± 8) and D2 (80 ± 10) compared with C (73 ± 11,p 〈 0.05 Tukey's test), suggesting the presence of parasympathetic neuropathy In conclusion the normal circadian variation of blood pressure was moderately disturbed in a group of microalbuminuric patients and patients with less advanced overt nephropathy. Patients with advanced diabetic nephropathy receiving antihypertensive therapy showed a marked reduction of nocturnal blood pressure fall, which can only be identified by the application of ambulatory blood pressure measurements to verify the 24-h effectiveness of blood pressure control.

    Notes: Summary Four patients are described presenting papilloedema, increased pressure and reduced CSF absorption—caused by either spinal tumours, leptomeningeal carcinomatosis or encephalitis. Remarkably they all had a normal CT without signs of hydrocephalus. A 24-hour intracranial pressure monitoring showed a mean pressure of 30–35 mm Hg, recurrent plateau waves and high occurrence of B waves. Conductance to CSF outflow studied by a constant perfusion test was severely reduced 0.010–0.026mlmin−1mm Hg−1 (normal 〉 0.12ml mm Hg−1 min−1). Despite these findings no ventricular enlargement was seen on serial CT scans. The reason therefore remains unknown. Disappearance of papilloedema and a variable clinical improvement followed shunt-insertion.

    Notes: We calculate the modifications of the unimolecular rate constants for free metallic clusters due to thermally exited electronic degrees of freedom. The effect is essentially taken into account by substituting Helmholtz's free energy of the electronic system for the ground state energy. The activation energy pertaining to the electronic ground states of the mother and daughter cluster is then replaced by the first difference in Helmholtz's free energy. © 1994 American Institute of Physics.

    Notes: The electronics for the microchannel plate photomultiplier tubes used in the LIDAR (Light Detection and Ranging) Thomson scattering diagnostic on the JET tokamak are described. It is shown how the microchannel plate photomultiplier tubes are gated with a very small coupling between gate signal and output, and how ≈300 V transient pulses with rise times of 230 ps are clipped to safe levels of ≈40 V.

    Notes: By combining the time-of-flight or LIDAR principle with a Thomson backscatter diagnostic, spatial profiles of the electron temperature and density are measured in a magnetically confined fusion plasma. This technique was realized for the first time on the JET tokamak. A ruby laser (3-J pulse energy, 300-ps pulse duration, 0.5-Hz repetition rate) together with a 700-MHz bandwidth detection and registration system yields a spatial resolution of about 12 cm. A spectrometer with six channels in the wavelength range 400–800 nm gives a dynamic range of the temperature measurements of 0.3–20 keV. The stray light problem in the backscatter geometry is overcome by spectral discrimination and gating of the photomultipliers. A ruby filter in the spectral channel containing the laser wavelength allows calibration of the vignetting along the line of sight by means of Raman scattering, enabling the measurement of density profiles. The low level of background signal due to the short integration time for a single spatial point yields low statistical errors (ΔTe /Te ≈6%, Δne /ne ≈4% at Te =6 keV, ne =3×1019 m−3 ). Goodness-of-fit tests indicate that the systematic errors are within the same limits. The system is described and examples of measurements are given.

    Notes: Anthocyanin extraction from elderberry pomace is improved by mechanical disintegration and use of aqueous HCl in place of acidified ethanol as extractant, with pH being important in determining extractant effectiveness. Extraction at low solvent:pomace ratios gives extracts of higher concentration and lower volume, but with reduced efficiency. Enzyme pretreatments do not improve extractability from exten-sively crushed pomace, and if of extended duration, can result in anthocyanin break-down. Vacuum evaporation of extracts gave little loss of anthocyanins, and combined with freeze drying, produced materials with high anthocyanin retention.

    Notes: Carrier concentration and mobility of unintentionally doped InP layers, grown directly on Si using metal-organic vapor-phase epitaxy, have been studied. The formation of antiphase domains (APDs) was found to depend on annealing of the Si substrate in an AsH3 flow prior to epitaxial growth. Dislocation densities determined by the wet chemical delineation technique were (8±1)×107 cm−3, seemingly uncorrelated to APDs in the layers. In addition to a shallow donor and a compensating acceptor, a deep donor was observed affecting the temperature dependence of the free-electron concentration between 77 and 300 K. The electron mobility in this temperature range could be described in terms of the scattering mechanisms which are dominant in homoepitaxial InP, namely, scattering due to polar optical phonons, to ionized impurities, and to space charges. Electron scattering due to either of these mechanisms was strongly influenced by the occurrence of antiphase boundaries (APBs). The space-charge density as well as the degree of compensation of the epitaxial layers increases with the density of APBs. Degraded 300 K mobilities were obtained indicating the effect of local stress at the APB.

    Notes: The design of a photoemission microprobe beamline at the MAX laboratory is presented. The beamline will utilize the radiation from a 26 period undulator on the 550-MeV MAX storage ring. The beamline will be capable of providing about 1010 photons/s of monochromatized radiation (bandwidth better than 0.25 eV) in the energy range of 20 to 150 eV focused into a submicrometer focal spot. It will be used for scanning photoemission experiments with a lateral resolution better than 1 μm. The optical system is comprised of a plane-grating monochromator with a Kirkpatrick–Baez objective and a specially designed ellipsoidal focusing mirror.

    Notes: A computer controlled acquisition and evaluation system for electrostatic probes is presented. Fully automatic evaluation of the probe characteristics yields electron temperature, plasma potential, and density during the recording. Two-dimensional probe positioning is accomplished with stepping motors. The system is characterized by 12-bit probe current resolution and extremely wide probe voltage range (±130 V). It is tested in a collisionless argon magnetoplasma and compared to standard wave diagnostic techniques for magnetized plasmas, i.e., the resonance cone technique. The influence of the probe radius on the electron current in a magnetized plasma is studied experimentally. The two techniques yield identical density and temperature values within 20% accuracy and therefore show the reliable applicability of the automatic evaluation system.

    Notes: Summary. The clinical significance of placental perforation and bloodstained amniotic fluid was studied in a group of 7238 Danish women undergoing mid-trimester amniocentesis for prenatal diagnosis under ultrasound guidance. The risk of spontaneous abortion was significantly increased both in pregnancies where the placenta was perforated and in those with blood-stained amniotic fluid. The risk estimate nearly doubled after placental perforation and more than doubled with a bloody tap. It is concluded that for women at relatively low risk of a fetal genetic abnormality, the indication of the amniocentesis should be reconsidered if a placental perforation is unavoidable.
