    Notes: The skin application of EMLA cream under a Tegaderm dressing was compared in children with a new combined dressing/local anaesthetic patch—the EMLA patch. The analgesic effect during venepuncture was assessed using a visual analogue scale (patients) and a verbal rating scale (investigator). Skin adhesiveness and incidence of local skin reactions with the two types of application were also studied. The study was designed as an open randomised trial with two parallel groups. Sixty children, aged between 5 and 15 years were evaluated. After a minimum application time of 60 min an intravenous cannula was inserted. There was no difference in analgesia as assessed by the patients or the investigators. Mild discomfort at removal of the occlusive dressing/patch was observed in a few patients, but there was no difference in the adhesiveness of the Tegaderm dressing and the EMLA patch. Only mild local skin reactions (with paleness in the anaesthetised skin area) were observed in both groups. It was concluded that both the EMLA patch and the Tegaderm/EMLA cream dressing provide effective dermal analgesia for venepuncture with a 0.8 mm (outer diameter) cannula. The two types of application were indistinguishable but the ease of application of the patch is a distinct advantage.

    Notes: The gas phase high resolution x-ray photoelectron spectra of the Cr(CO)6 metal complex are presented. The valence electron spectrum and the core level spectra including the associated satellite structures from the Cr, C, and O atoms are discussed. Comparison to free CO and to CO adsorbed on metal surfaces are made. An assignment of the valence levels is achieved using an intensity model. The anomalous dispersion of the narrow band in CO adsorbed on Cu seems to be related to the localization properties of the corresponding 7a1g level in Cr(CO)6. The C1s and O1s shake-up structures are assigned using the Z+1 approximation, electron energy loss data and data from shake-up spectra of free CO. The Cr core level binding energy shifts relative to Cr metal are shown to vary with the subshell. Electronic structure and vibronic calculations are carried out for the ground and C1s and O1s core ionized states. The results for the vibronic coupling constants and for the vibrational excitations confer more with the observations in the electron energy loss spectra of free CO rather than with the photoelectron spectra of free CO. This supports the notion of a, close to complete, charge transfer screening accompanying ligand core ionization of Cr(CO)6.

    Notes: Pharmacokinetic data obtained after one dose of a 2-mg budesonide enema were compared with data obtained after the last dose of four weeks of daily treatment in 24 patients with active distal ulcerative colitis or proctitis. This open multicentre study involved 28 eligible patients. Sigmoidoscopy and biopsy scores improved significantly (P 〈 0.002) during the four-week treatment period. Maximal plasma concentration (Cmax) of budesonide was 2.1 nmol/L 1.3 h after the first dose and 2.5 nmol/L 1.2 h after the last dose; the difference was not significant. The area under the curve (AUC) of plasma concentration vs. time was after the first dose 9.7 nmol h/L and after the last dose 11.6 nmol h/L (P 〈 0.03). The small increase in AUC may be attributed to improved absorption. During the last dose interval, minimal plasma concentration was below the limit of quantitation in most subjects. The Cmax and AUC of budesonide increased slightly after four weeks of treatment, but budesonide did not accumulate. Mean morning plasma cortisol values did not change significantly during treatment (P= 0.083), although a small change in cortisol levels between the first visit (pre-treatment) and last visit was positively correlated to the C. of budesonide measured at the last visit (P= 0.012).

    Notes: Methods: The retrograde spread of two budesonide enema formulations with different viscosities was investigated. The study design was open, randomized and two-period crossover. Three female and two male patients (age range: 35–45 years) with distal ulcerative colitis or proctitis participated. Both enema formulations contained a dose of 2 mg budesonide/100 mL. An unabsorbable radioactive marker (99mTc-labelled human serum albumin microcolloid) was added to the enema just before administration. All doses were given in the evening with the patients lying in a supine position during the whole investigation. The intestinal spread was followed for 8 h using scintigraphic imaging. Plasma samples for budesonide assay were taken during the 12 h after administration of the low viscosity enema. Results: Budesonide plasma levels were measurable for up to 4–6 h. Cmax was 2.5 nmol/L (range:0.9–4.5 nmol/L) and was attained in 1.5 h (range 1–3 h). The patients had no difficulty in retaining the enemas. There was a statistically significant difference in spread between the low and high viscosity enema. The low viscosity enema spread over an area situated between the rectum and the splenic flexure. The spread occurred mainly in the first 15 min after administration. In contrast, the high viscosity enema, in most cases, spread only over a minor part of this area and the rate and extent of spreading was also more variable with this formulation. Conclusion: The spread of a low viscosity enema appears to be well suited for the treatment of proctitis and distal colitis.

    Notes: Summary Consumption of fatty fish species, like salmon and herring, from the Baltic Sea is an important source of human exposure to persistent organochlorine compounds, e.g. polychlorinated dioxins (PCDDs), dibenzofurans (PCDFs) and biphenyls (PCBs). Many of these compounds show immunotoxic and hepatotoxic effects in animals. We have now studied immunological competence, including lymphocyte subsets, in 23 males with a high consumption of fish from the Baltic Sea and in a control group of 20 males with virtually no fish consumption. The high consumers had lower proportions and numbers of natural killer (NK) cells, identified by the CD 56 marker, in peripheral blood than the non-consumers. Weekly intake of fatty fish correlated negatively with proportions of NK cells (r s = −0.32, P = 0.04). There were also, in a sub-sample of 11 subjects, significant negative correlations between numbers of NK cells and blood levels of a toxic non-ortho-PCB congener (IUPAC 126; r s = −0.68, P = 0.02) and a mono-ortho congener (IUPAC 118; r s = −.76, P = 0.01). A similar correlation, in 12 subjects, was seen for p,p′-DDT (r s = −0.76, P = 0.01). The corresponding negative correlation, in 13 subjects, with blood levels of PCDD/Fs was not significant (r s = −0.57, P = 0.07). No significant association was seen between organic mercury in erythrocytes and NK cells. Fish consumption was not associated with levels of any other lymphocyte subset. Neither were there any correlations with plasma immunoglobulins or liver enzyme activities. Our study indicates that accumulation of persistent organochlorine compounds in high consumers of fatty fish may adversely affect NK cell levels.

    Notes: Abstract Using the ARGUS detector at thee + e − storage ring DORIS II, we have measuredB decays into exclusive final states containing aD orD * meson plus several pions, or containing aJ/ψ or ψ′ meson plus a strange particle. Some of these channels have not been seen before, while others represent updated measurements of previous results. The branching ratios are compared with the predictions of the model of Bauer, Stech and Wirbel. Using the cleanest decay channels, the mass of the charged and neutralB meson are found to bem B −=(5280.5±1.0±2.0) MeV/c2 andm B 0=(5279.6±0.7±2.0) MeV/c2 respectively, yielding a mass differencem B 0−m B − =(−0.9±1.2±0.5) MeV/c2.

    Notes: Abstract A recently proposed multiplicity measure is used to study the properties of QCD cascades. We show that it is possible to define anomalous dimensions locally in rapidity space for the QCD cascade ine + e −-annihilation events and in deep inelastic lepton scattering. In this way it will be possible to differentiate between different suggested models for multiple gluon emission. We also show that the properties of an event are to a surprisingly large degree deterimined by the first one or two gluons. Thus e.g. the multiplicity fluctuations ine + e −-annihilation at theZ 0-pole are to about 90% determined by the hardest gluon. This implies that it may be principally difficult to make a distinct separation between the hard perturbative phase and the soft hadronization phase.

    Notes: Abstract Particle production in small rapidity or angular intervals have fractal structures similar to a Cantor dust. In this paper we present analytical result for multifractal dimensions valid for high energies. For high moments the dimension is given by $$\sqrt {6\alpha _s /\pi } $$ . The scaling properties seen in the partonic state are not so well reflected in the hadronic multiplicity moments or factorial moments. We show how to define new observables on the final hadronic state, which do scale well. This means that the multifractal dimensions can be well determined and compared with results from QCD.

    Notes: Abstract The level structure of92Ru has been studied by means of γ-ray spectroscopy. The nucleus was produced by the58Ni(40Ca, α2p)92Ru reaction at beam energies of 147 and 187 MeV. The NORDBALL detector system including particle selection was used. A large number of new levels with excitation energies up to 11.3 MeV and spin values up to 22 or 23 units of angular momentum have been established. The level scheme is compared with recent shell model calculations using100Sn as a core. Some systematics of the g 9 2/−2 configuration is discussed and a strong correlation between the levels in90Mo and92Ru is found.
