    Notes: Abstract A 16-year-old girl with disseminated molluscum contagiosum (MC) was found to have a very low level of serum IgM, elevated levels of IgG and IgA, and a high level of IgE. She had normal numbers of peripheral blood IgM+, IgG+ and IgA+ B-lymphocytes but their terminal differentiation into plasma cells could not be induced by pokeweed mitogen (PWM) in vitro. On the other hand, the patient's T-cells showed normal helper functions in the PWM system and normal interferon (IFN) production in vitro. However, the IgM+ B-cells can be induced to differentiate into IgM secreting cells by Epstein-Barr virus (EBV), suggesting that the genetic mechanism for synthesis of the component immunoglobulin proteins is present. T-cell functions were impaired, as shown by delayed type cutancous hypersensitivity (DTH) and mitogen response. The data suggest that the selective IgM deficiency of the patient is due mainly to defects in B-cells at the terminal differentiation stage, but immunological abnormalities are present in both B and T-cell systems. Neutrophil functions examined were normal. MC was treated by intravenous injection of IFN without any side effects; however, no clinical improvement was achieved.

    Notes: Summary This study was carried out to investigate the effects of an expanding supratentorial mass on auditory brain-stem responses (ABRs). A balloon was inserted into the supratentorial epidural space of seven baboons (in two cases, in the right occipital area; in five cases, in the right temporal area). The balloons were inflated at a rate of 0.2 ml/minute to increase intracranial pressure (ICP). ICP (right frontal epidural pressure) and blood pressure (BP) were continuously recorded. Recordings of ABRs (vertex to mastoid on both sides) were made serially. Pupillary changes were also recorded. At 30mmHg ICP, the amplitude of wave V on the right side was observed to be significantly attenuated (p〈0.02). At 50 mm Hg ICP, the latency of wave V on the right side was significantly prolonged compared with that at 30mmHg ICP (p 〈 0.02). At 70mmHg ICP, significantly decreased amplitude of wave V on the left side was also observed (p〈0.02, from the control), associated with significant increased latency of wave IV on the right side (p〈0.01, from the control; p 〈0.05, from 50 mm Hg ICP). Finally, waves IV and V on both sides substantially disappeared at 100 mm Hg ICP. Anisocoria appeared in four animals at 30–50 mm Hg (mean ±SD; 45±8.7) ICP. The amplitude of wave V was significantly decreased in these circumstances (p〈0.05). Bilaterally fixed pupils occurred in five animals at 70–100 mm Hg (mean±SD; 76 ±12) ICP. wave IV latencies on both sides were significantly prolonged (p〈0.02 right; p〈0.05, left) and disappearance of wave V (four out of five on the right side, three out of five on the left side) was observed in these circumstances. These findings suggest that serial measurement of ABRs can be useful for evaluation of the progress of transtentorial herniation.

    Notes: A new procedure was established for determining fat-binding capacity of proteins, using a nonpolar fluorescent probe diphenylhexatiene dissolved in corn oil. The amount of bound oil was measured from the fluorescence intensity (FI) of the bottom layer after centrifuging the blended protein-oil-probe mixture. This method classified proteins into two groups, i.e., group 1 which had FI increasing linearly with protein concentration and group 2 which had FI increasing then decreasing. The linearly fitted slope of the FI-protein plot revealed better correlation with anilinonaphthalenesulfonate hydrophobicity than with cis-parinarate hydrophobicity. It also showed a significant correlation with emulsifying activity index.

    Notes: Abstract Overdosage intoxication of sulindac, tiaramide and diclofenac caused excitability of central nervous system, followed by unconsciousness. The case was treated with ordinary therapies and direct hemoperfusion (DHP). Serum concentrations of these drugs and their metabolites were correlated well with the clinical symptoms. DHP may be effective to eliminate these drugs and their metabolites.

    Notes: Abstract Intracranial pressure (ICP) was measured in six patients following cardiopulmonary resuscitation (CPR). The causes of cardiac arrest were respiratory or circulatory problems and the primary intracranial pathology was not detected. The measurement of ICP started 3 to 10 h following CPR except one patient in whom it started on the day 7. Duration of ICP measurement ranged from 2 to 7 days. In five out of six patients, ICP persistently remained below 20 mmHg. In the remaining one patient, ICP elevation associated with seizure activity was observed and ICP ultimately increased to 57 mmHg. Among these, four patients died and two remained in a persistent vegetative state. These results suggest that ICP following CPR does not necessarily increase if the patient has no primary intracranial pathology or seizures.
