Zusammenfassung Die Diagnose einer kompletten acromio-claviculären Luxation (Typ III) stellt eine Operationsindikation dar, wobei in der Literatur verschiedene Operationsmethoden angegeben werden. In der Zeit vom 1.1.1970 bis 31.12.1978 sind in der Berufsgenossenschaftlichen Unfallklinik Tübingen 33 Patienten mit frischen AC-Sprengungen Typ Tossy III durch Zuggurtungsosteosynthesen und Bandnaht versorgt worden. Die Ergebnisse dieser Operationsmethode werden mitgeteilt und auf intra- und postoperative Komplikationen wird eingegangen. Die Vorzüge einer Freilegung des AC-Gelenkes durch einen bogenförmigen Hautschnitt schildert man, nicht zuletzt aus kosmetischen Erwägungen.

Summary The diagnosis of a complete acromio-clavicular separation (Tossy III) is a indication for a surgical repair. Many and different methods are reported in the literature. 33 patients with a fresh, complete acromioclavicular separation were treated at the BG-Unfallklinik Tübingen from 1. 1. 1970 to 31.12.1978 by suturing the ligaments, inserting pins across the joint and tension wire bending. With this method good results have been achieved, possible intra- and postoperative complications are reported. The incision along the clavicle quite often gives scar problems. Therefore we now use an arched incision across the AC-joint.
