Summary A case of Trigeminal neuralgia associated with a meningioma of the tentorium is described because of the unusual feature that the pain was deceptively similar to that of a glossopharyngeal neuralgia. The difficulties in differentiating preoperatively between these two neuralgias is discussed. Dandy's operative approach under local anaesthesia via the posterior fossa with the employment of an electrical stimulator is recommended as the method of choice in cases where it is not clinically possible to decide whether one is dealing with Trigeminal or with Glossopharyngeal neuralgia.
Abstract:
Résumé Un cas de névralgie trigéminale liée à un méningiome de la tente est décrit; La particularité de cette observation est représentée par la douleur qui simule une névralgie glosso-pharyngienne. Les difficultés du diagnostic différentiel préopératoire entre ces 2 névralgies sont étayées. La voie d'abord de Dandy sous anesthésie locale, vers la fosse postérieure, avec l'emploi d'un stimulateur électrique, est recommandée comme la meilleure méthode dans les cas où cliniquement il n'en est pas possible de faire le diagnostic entre névralgie trigéminale et névralgie glosso-pharyngienne.

Zusammenfassung Ein Fall von Trigeminusneuralgie bei Meningeom des Tentorium wurde beschrieben, der dadurch bemerkenswert ist, daß die Schmerzattacken einer Glossopharyngeusneuralgie täuschend ähnlich waren. Die Schwierigkeiten der präoperativen Differentialdiagnose zwischen diesen beiden Neuralgiearten werden besprochen. Der operative Zugang durch die hintere Schädelgrube nachDandy in Lokalanästhesie unter Benutzung eines elektrischen Reizgerätes wird als Methode der Wahl für solche Fälle empfohlen, in denen es klinisch nicht möglich ist zu entscheiden, ob es sich um eine Trigeminuso der Glossopharyngeusneuralgie handelt.
