Zusammenfassung An 15 weißen Ratten wurden Muskeln mit 0,65 mm dicken Injektions- und EMG-Nadeln punktiert. Die histologische Untersuchung sofort nach der Punktion bzw. in Abständen von 4 Std bis zu 28 Tagen ergab „myopathische“ fokale Veränderungen. Diese bestanden in Einzelfasernekrosen, Faserphagocytosen, zelligen, gemischt polymorphkernigen und lymphocytären Infiltraten, in Faserregenerationen und reaktiven Kernveränderungen. Vom 14. Tage an waren keine Veränderungen mehr zu sehen mit der einzigen Ausnahme von einmaligen reaktiven Kernen nach 19 Tagen. Die Rolle der Technik der Punktion, von deren Zahl und der Verweildauer der Nadel sowie des Nadelmaterials wird diskutiert. Die praktischen Schlußfolgerungen für die bioptische Diagnostik neuromuskulärer Erkrankungen werden besprochen.

Summary In the rat the puncture of a muscle with a 0.65 mm injection needle or EMG needle causes histological changes consisting of necrosis of single fibers, phagocytosis, cell infiltration, reactive changes of nuclei as well as regeneration of fibers. These changes disappear after 14 days with one exception, where reactive nuclei were still present after 19 days. At later stages histological changes were never observed. The changes seem to be independent of the number of punctures and the length of time the needle was left in the muscle. It is concluded that muscle biopsy should not be carried out on a muscle which had been punctured during the previous 19 days. We cannot tell if the additional injection of a substance in the muscle might even produce longer lasting histological changes. In addition the myopathologist should be aware of the possibility of a needle myopathy when observing certain well localized changes in his specimens. Focal changes, similar to those described above, can be observed in the early stages of muscular dystrophy, in carriers of Duchenne dystrophy as well as in some chronic neurogenic muscular atrophies.
