    Notizen: Computer based learning has enjoyed an increasing role in mainstream education with the development of more powerful personal computers available at a lower price. This articlereviews work on a specific form of computer software, virtual environments. It aims to illustrate the potential of this computer application in the education and rehabilitation of people with intellectual disabilities. Virtual environments appear to be a fruitful method of teaching skills for independent living to people with intellectual disabilities. Initial studies demonstrate that learning in this way transfers to the real life situation in which the skills are required. However, to effectively exploit their educational potential, interaction with virtual environments needs to be guided. This can be done using a human tutor and a software tutor and preliminary findings from current research suggest guidelines for the effective employment of both. Future directions involve ensuring the availability of the software, making the technology accessible to a wider range of users and researching staff support to ensure that use is facilitated and that this technology is appropriately exploited.

    Notizen: Surgical procedures that involve a local anaesthetic block are often accompanied by light general anaesthesia. It is possible that under these circumstances, the patients are more likely to register auditory events while apparently unconscious. Two groups of children were exposed to auditory stimuli during surgery; one group received a lighter level of anaesthesia than the other. Retention of the stimuli was tested using a cued recall technique, and the performance of the two experimental groups was compared with a control group which had not been exposed to the material but was tested in the same way. The light anaesthesia group retained more items than did the other two groups but this difference was significant (p 〈 0.05) only when compared with the other experimental group. This is not very strong evidence of auditory registration, but a greater effect might be found with exposure to emotionally significant material. It is suggested that patients should be protected from unfortunate theatre conversation.

    Notizen: A randomised double-blind study was conducted to compare the influence on postoperative pain of equal volumes of one of two concentrations of bupivacaine (0.25%, 0.5%), or saline 0.9%, injected into the knee joint after arthroscopy. The results showed that intra-articular bupivacaine had no significant analgesic effect in either concentration. Plasma bupivacaine concentrations were measured 20 minutes after injection in order to determine the extent of systemic absorption.

    Notizen: Abstract The results of three controlled trials performed on children with insulin-dependent diabetes mellitus were examined for evidence of seasonal variation in concentrations of glycosylated haemoglobin (HbA1). All three studies showed lower levels during the summer months. Multiple regression analysis showed that the month of sampling accounted for a significant proportion of the total variance in HbA1 levels (P〈0.001 in all three studies). We suggest that exercise, dietary changes and the frequency of minor illnesses may all contribute to this fluctuation which has important implications for the design of clinical trials in childhood diabetes.

    Notizen: Abstract In a double-blind, balanced crossover study, eight healthy male volunteers ingested either DL-308 (10 mg), DL-308 (20 mg), oxazepam (30 mg) or placebo. Subjective estimates of coordination and anxiety, objective performance measurements and cardiovascular measurements were taken at 1, 3, 5 and 8 h after ingestion. DL-308 (20 mg) exerted a strong sedative effect as judged by self-reported coordination scores and performance on logical reasoning and reaction time tests. The effect was evident almost immediately and, on the coordination and reasoning tests, lasted up to 8 h following ingestion. Attention is drawn to the need to extend performance testing in order to maximise test sensitivity. No drug had any consistent or strong influence on cardiovascular measures.

    Notizen: Summary This study investigated aspects of the community psychiatric care of psychotic patients living in inner city areas, and especially those of Afro-Caribbean ethnicity. Ethnicity was not found to be a major factor affecting complicance with depot injections. More important were variables such as time since first treatment and sex. Defaulters from treatment were more likely to be subsequently admitted to hospital.
